Low muscle mass in patients with stroke on admission reduces walking ability at discharge.

BACKGROUND & AIMS: Reduced skeletal muscle mass may negatively influence postural retention and walking function. This study aimed to examine the influence of the skeletal muscle mass index on walking function in patients with stroke. METHODS: This study included patients with cerebral infarction aged ≥65 years. The Asian Working Group for Sarcopenia's skeletal muscle mass index criteria were used to classify the participants into the low and high skeletal muscle mass index groups. The patient characteristics of the two groups were compared. The primary and secondary outcome measures were ind…

Role of Body Composition in Patients with Resectable Pancreatic Cancer.

This study investigates the role of body composition parameters in patients with pancreatic cancer undergoing surgical treatment. The research involved 88 patients diagnosed with pancreatic cancer who underwent surgery at the Modena Cancer Center between June 2015 and October 2023. Body composition parameters were obtained from CT scans performed before and after surgery. The percentage of sarcopenic patients at the time of diagnosis of pancreatic cancer is 56.82%. Of the patients who died between the first and second CT evaluated, 58% were sarcopenic, thus confirming the role of sarcopenia on…

Younger Older Americans and Sarcopenic Obesity: The Moderating Role of Living Alone.

OBJECTIVES: Previous studies have indicated that compared to older adults, younger older adults (e.g., baby boomers) are more susceptible to obesity, but their risk decreases as they age. However, there is a lack of research on how individuals experience sarcopenic obesity, which increases in later life and is a mortality risk factor. This study examined how younger cohorts of older Americans and their demographic traits are related to sarcopenic obesity. METHODS: Generalized estimating equations were used to analyze participants aged 65 years and older, stratified by sex using data from the

Neck circumference as a potential indicator of pre-sarcopenic obesity in a cohort of community-based individuals.

BACKGROUND & AIMS: ESPEN/EASO advocates screening for sarcopenic obesity based on the concomitant presence of an elevated body mass index (BMI) or waist circumference. Neck circumference (NC) is another simple and reliable anthropometric measurement for estimating obesity; however, its ability to detect sarcopenic obesity has not yet been established. The aim of the present study was to explore the association between NC and sarcopenic obesity in a Shanghai community population. METHODS: The study included 1542 participants (622 men and 920 women) with a mean age of 58 years who underwent an

The impact of machine-learning-derived lean psoas muscle area on prognosis of type B aortic dissection patients undergoing endovascular treatment.

OBJECTIVES: The aim of this work was to investigate the impact of machine-learning-derived baseline lean psoas muscle area (LPMA) for patients undergoing thoracic endovascular aortic repair. METHODS: A retrospective study was undertaken of acute and subacute complicated type B aortic dissection patients who underwent endovascular treatment from 2010 to 2017. LPMA (a marker of frailty) was calculated by multiplying psoas muscle area and density measured at L3 level from the computed tomography. The optimal cut-off value of LPMA was determined by the Cox hazard model with restricted cubic splin

European guideline on obesity care in patients with gastrointestinal and liver diseases - Joint ESPEN/UEG guideline.

BACKGROUND: Patients with chronic gastrointestinal (GI) disease such as inflammatory bowel disease (IBD), irritable bowel syndrome (IBS), celiac disease, gastroesophageal reflux disease (GERD), pancreatitis, and chronic liver disease (CLD) often suffer from obesity because of coincidence (IBD, IBS, celiac disease) or related pathophysiology (GERD, pancreatitis and CLD). It is unclear if such patients need a particular diagnostic and treatment that differs from the needs of lean GI patients. The present guideline addresses this question according to current knowledge and evidence. OBJECTIVE: T

Identification of sarcopenic obesity in adults undergoing orthopaedic surgery: Relationship between "a body shape index" (ABSI) and fat-free mass. A cross -sectional study.

BACKGROUND: Sarcopenic obesity is a condition characterised by the coexistence of low muscle mass and function (sarcopenia) and excessive fat mass (obesity). The aim of this study was to determine the prevalence of this condition in patients undergoing orthopaedic surgery by gender and type of orthopaedic surgery. In addition, this study investigated the suitability of a waist circumference-based anthropometric measure, body shape index (ABSI), for predicting sarcopenic obesity and the predictive power of ABSI for fat-free mass index (FFMI), a surrogate marker of lean body mass. METHODS AND F…

[Impact of osteoporosis on physical performance parameters of middle-aged and elderly individuals-a cross-sectional study].

BACKGROUND: Osteoporosis (OP) in the elderly is accompanied by reduced muscle mass and reduced muscle strength, also known as sarcopenia. This results in functional limitations and a high risk of falls and injuries. The determination of physical performance parameters such as grip strength and trunk strength on the one hand, and balance abilities on the other, provide information about the individual's general neuromuscular condition and serve as an indicator of physical performance in the elderly. The extent to which previous osteoporotic vertebral fractures (VFs) restrict an individual's phy…

Association between Sarcopenic Obesity and Nonalcoholic Fatty Liver Disease and Fibrosis detected by Fibroscan.

BACKGROUND AND AIMS: Nonalcoholic fatty liver disease (NAFLD) and sarcopenic obesity share several pathophysiologic backgrounds. No prior studies have determined a plausible association between sarcopenic obesity and NAFLD and NAFLD-associated fibrosis. We aim to investigate the association between sarcopenic obesity and NAFLD, and NAFLD-associated fibrosis detected by transient elastography. METHODS: In a cross-sectional study from the 2017-2018 National Health and Nutrition Examination Survey, 1,925 participants were identified. NAFLD was defined by controlled attenuation parameter (CAP) sc

Molecular Mechanisms of Muscle Fatigue.

Muscle fatigue (MF) declines the capacity of muscles to complete a task over time at a constant load. MF is usually short-lasting, reversible, and is experienced as a feeling of tiredness or lack of energy. The leading causes of short-lasting fatigue are related to overtraining, undertraining/deconditioning, or physical injury. Conversely, MF can be persistent and more serious when associated with pathological states or following chronic exposure to certain medication or toxic composites. In conjunction with chronic fatigue, the muscle feels floppy, and the force generated by muscles is always

Nutrition support and clinical outcome in advanced cancer patients.

Newly diagnosed cancer patients are frequently found suffering from a metastatic disease, which poses additional challenges to the delivery of effective therapies. Chemotherapy and radiotherapy are associated with side effects which reduce tolerance to treatment and likelihood of tumour response. Identifying preventable factors of reduced response to therapy would translate into better care of cancer patients. Among other factors, malnutrition, as diagnosed by non-volitional weight loss, and cachexia, as revealed by sarcopenia, are universally recognised negative prognostic factors. Less certa…

Quality of life assessment in musculo-skeletal health.

Musculoskeletal disorders affect morbidity, quality of life and mortality, and represent an increasing economic and societal burden in the context of population aging and increased life expectancy. Improvement of quality of life should be one of the priorities of any interventions to prevent and treat musculoskeletal disorders in the ageing population. Two main approaches, namely generic and disease-specific instruments, can be applied to measure health-related quality of life. Among the generic tools available in scientific literature, the short form 36 questionnaire (SF-36) and the Euroqol f

Identification of Sarcopenic Obesity by Fat-to-Muscle Ratio in Older Adults: A Cohort Study.

BACKGROUND: The diagnosis of sarcopenic obesity has been established in Europe and Japan, but screening tools remain inconsistent and lack standardization. The fat-to-muscle ratio (FMR) is a potential screening measure for sarcopenic obesity; however, its diagnostic accuracy compared with other tools has not been evaluated. This study compared the diagnostic performance of several screening tools for sarcopenic obesity. METHODS: This cross-sectional analysis used data from the National Center for Geriatrics and Gerontology Study of Geriatric Syndromes (NCGG-SGS), a national cohort study condu

Relationship of preoperative low muscle mass with postoperative loss of independence after hepato-biliary-pancreatic surgery in elderly patients: a single-center retrospective study.

BACKGROUND: Numbers of patients undergoing hepato-biliary-pancreatic (HPB) surgery are increasing worldwide. However, elderly patients may experience loss of independence (LOI) postoperatively due to the marked invasiveness of surgical procedures. This retrospective cohort study aimed to investigate clinical characteristics of elderly patients including preoperative low skeletal muscle mass before HPB open surgery for malignancy, and the association between low skeletal muscle mass and LOI postoperatively. METHODS: Patients aged 65 or older who underwent open curative resection for HPB maligna

Dysregulation of the ubiquitin-proteasome system in aging skeletal muscle.

Protein homeostasis (proteostasis) is essential for maintaining skeletal muscle integrity, and its disruption is a central feature of aging related sarcopenia. The ubiquitin-proteasome system (UPS) is the primary pathway responsible for selective protein degradation in muscle. However, its regulation during physiological aging remains incompletely understood. Most studies have focused on muscle-specific E3 ubiquitin ligases, particularly MuRF1 and MAFbx/atrogin-1, which are widely used as molecular markers of muscle atrophy. However, changes in E3 ligase expression do not consistently correspo…

Cellular and molecular pathways linking obesity to skeletal muscle dysfunction.

Obesity is increasingly recognized as a condition that directly impairs skeletal muscle structure, metabolism, and endocrine function through complex molecular and cellular mechanisms extending beyond the classical concept of sarcopenic obesity. This narrative review aimed to synthesize current evidence regarding the intracellular signaling pathways, metabolic alterations, and endocrine interactions involved in obesity-induced skeletal muscle dysfunction independent of overt sarcopenia. Relevant literature from experimental, clinical, and review studies was identified through searches of PubMe…
