Dementia is a growing problem of global relevance, currently affecting over 55 million people worldwide. The number of new dementia cases is still increasing, primarily due to the aging of society. Dementia is defined as a substantial decline in cognitive function, and it is inherently associated with neurodegenerative diseases such as Alzheimer's disease, Parkinson's disease, dementia with Lewy bodies, frontotemporal dementia, and vascular dementia. Of note, most patients suffering from neurodegenerative conditions, in addition to cognitive impairment, often experience various types of sleep …

Antidepressants are increasingly prescribed for pediatric patients to treat a vast array of mental health conditions, including anxiety disorders, depressive disorders, and obsessive-compulsive disorder (OCD). While there is extensive research supporting the efficacy of antidepressants in managing these conditions in the pediatric population, their effects on sleep, an essential component of overall well-being, remain a critical area of interest. This review explores the clinical effects on sleep disorders and the impact of antidepressants on sleep architecture and overall sleep quality in chi…

Paediatric sleep medicine has rapidly evolved and expanded over the past half century as it became increasingly recognised as a unique field related to but distinct from adult sleep medicine. In looking forward to the next years, the focus of the following discussion is two-fold: to summarise a brief history of the field, recent developments and current trends, and to present a blueprint for the future across various key domains. Using Bronfenbrenner's Ecological Systems Theory as a model for the interaction between the five interconnected ecosystems and sleep in children, we discuss a variety…

Obstructive sleep apnea (OSA) is common during pregnancy and linked to adverse outcomes. While oxidative stress is a proposed pathogenic mechanism, evidence in pregnant populations remains limited. This multicenter, prospective study evaluated oxidative stress through protein carbonyl levels in 171 pregnant women and 86 cord blood samples. Polysomnography (PSG) performed during pregnancy categorized participants with the apnea-hypopnea index (AHI) in OSA, rapid eye movement (REM) OSA, and supine OSA. Protein carbonyl levels were measured by the dinitrophenyl hydrazine (DNPH) method. No signifi…

The preoptic area of the hypothalamus is key for the control of sleep onset and sleep homeostasis. Although traditionally considered exclusively somnogenic, recent studies identified a group of preoptic glutamatergic neurons that promote wakefulness. Specifically, our previous investigations demonstrated that chemogenetic stimulation of glutamatergic neurons within the medial-lateral preoptic area (MLPO_VGLUT2) promotes wakefulness, fragments non-rapid eye movement sleep (NREMs), and suppresses REM sleep (REMs). This evidence is further supported by recent work showing that preoptic glutamater…

Sleep disturbances are among the most prevalent and persistent consequences of traumatic brain injury (TBI), yet they remain underutilized as clinical indicators of injury status. In this perspective, we propose that sleep fragmentation-defined as the frequency of transitions between sleep and wakefulness-represents a functional, scalable, and underrecognized diagnostic biomarker of TBI. Drawing on empirical findings from a mouse model of diffuse TBI, we show that summary measures of sleep fragmentation and duration can reliably distinguish injured from uninjured animals using dimensionality r…

Parkinson's disease is a neurodegenerative disorder with an increasing prevalence worldwide. The development of disease-modifying therapies remains a critical priority; however, early intervention is limited by the paucity of robust biomarkers for the prodromal stage. Sleep disturbances-particularly isolated rapid eye movement sleep behavior disorder (iRBD)-are emerging as key clinical markers of prodromal synucleinopathy, offering opportunities for early detection and risk stratification. This article highlights advances in clinical and biofluid biomarkers, including speech abnormalities, α-s…

Sleep disturbances are highly prevalent in people with major depressive disorder (MDD) and contribute to a vicious cycle that exacerbates both conditions. Electroencephalographic (EEG)-based sleep features of people with MDD are well described in systematic reviews and meta-analyses. However, sleep abnormalities in mouse models of MDD remain poorly characterized. We conducted a systematic review to evaluate the face validity of mouse models of MDD in relation to sleep alterations. Among the 22 articles we identified, the most consistently represented features of sleep disturbances in mouse mod…

Sleep is a vital biological function governed by neuronal networks in the brainstem, hypothalamus, and thalamus. Disruptions in these circuits contribute to the sleep disturbances observed in neurodegenerative disorders, including Parkinson's disease, epilepsy, Huntington's disease, and Alzheimer's disease. Oxidative stress, mitochondrial dysfunction, neuroinflammation, and abnormal protein accumulation adversely affect sleep architecture in these conditions. The interaction among these pathological processes is believed to modify sleep-regulating circuits, consequently worsening clinical symp…

PURPOSE OF REVIEW: Sleep disordered breathing (SDB) is a direct consequence of tetraplegic spinal cord injury (SCI), is highly prevalent in both tetraplegia and paraplegia, and is associated with worse daytime functioning and reduced quality of life. Despite this, most people with SCI are undiagnosed and untreated for the disorder. This narrative review summarises research from the last 5 years on the epidemiology, pathophysiology, and consequences of SDB in SCI, as well as the current approaches to screening, diagnosis, and treatment of SDB in this population.
RECENT FINDINGS: Previous resea…

Obstructive sleep apnea (OSA) is a common disorder characterized by recurrent episodes of upper airway obstruction during sleep, resulting in apnea and hypopnea. While anatomical factors, such as increased upper airway collapsibility, significantly contribute to OSA, emerging evidence highlights the importance of non-anatomical mechanisms, including impaired pharyngeal muscle responsiveness, low arousal threshold, and elevated ventilatory loop gain. The interplay of these factors leads to respiratory instability, fragmented sleep, and systemic complications. Additional pathophysiological influ…

Opioid use in patients with sleep disordered breathing (SDB) presents therapeutic challenges within chronic pain and sleep medicine. Opioids impair respiratory drive through μ-opioid receptor activation in brainstem respiratory centers, exacerbating both obstructive and central apneas. Chronic opioid use is also linked to a high prevalence of central sleep apnea and increased nocturnal hypoventilation. Simultaneously, SDB contributes to heightened pain sensitivity via intermittent hypoxia, systemic inflammation, and alterations in neural plasticity. These mechanisms may influence opioid effica…

Idiopathic central sleep apnea or primary central sleep apnea (ICSA) has been an officially recognized sleep disorder since the 2005 International Classification of Sleep Disorders 2nd edition (ICSD-2) and remains in the International Classification of Sleep Disorders 3rd edition (ICSD-3). The literature supports the etiology as "high loop gain or increased controller gain," along with central sleep apnea (CSA) associated with common diagnoses (e.g. heart failure). Available data place the adult population prevalence at about 0.05% (men) and 0.003% (women), while up to 11% of patients diagnose…
