John Caffey Award. Sonography of the caudal spine and back: congenital anomalies in children.

Articulated-arm, B-mode 3.5-5.0 MHz sonograms from 27 children with congenital anomalies of the caudal spine were analyzed retrospectively and correlated directly with patient appearance, preoperative myelograms, intraoperative photographs, and pathologic specimens to determine how effectively sonography could display the major pathologic features known to be present. Anterior spina bifida, posterior spina bifida, and (partial) sacral agenesis were displayed as focal absence of normal spinal echoes and distortions of the paraspinal/gluteal muscles. Subcutaneous anechoic spaces continuous with …

Spinal instability as defined by the three-column spine concept in acute spinal trauma.

This article is a presentation of the concept of the three-column spine. The concept evolved from a retrospective review of 412 thoracolumbar spine injuries and observations on spinal instability. The posterior column consists of what Holdsworth described as the posterior ligamentous complex. The middle column includes the posterior longitudinal ligament, posterior annulus fibrosus, and posterior wall of the vertebral body. The anterior column consists of the anterior vertebral body, anterior annulus fibrosus, and anterior longitudinal ligament. Major spinal injuries are classified into four d…

Anterior decompression of the spine for metastatic epidural cord compression: a promising avenue of therapy?

Most metastatic epidural tumors arise in a vertebral body and invade the anterior epidural space. Therefore, it is logical to decompress the spine anteriorly and not by traditional laminectomy. Surgical decompression is indicated if relapse occurs after radiotherapy and further radiation cannot be administered, if there is neurological deterioration during radiotherapy, and when histological diagnosis of the primary tumor is lacking. This pilot study consists of eleven consecutive anterior decompressions of the spine performed in nine patients. In seven instances other treatment modalities had…

[Our experience with vertebral grafts in the surgery of recent severe fractures of the dorsolumbar spine].

The authors report their experience of spine fusion in the management of fracture dislocations of the thoracic and lumbar spine. In a series of over a hundred surgical stabilization, plate instrumentation alone was orthopedically unsuccessful in 20% of cases after plate removal. Instrumentation is but a tutor, whereas spine fusion is an element which brings definite spine stability. Clinical and roentgenographie results are given. Based on the different types of spine injuries, indication are discussed as well as surgical approach and technique with a possible association of several methods ap…

Repair of major defects of the chest wall and spine with the latissimus dorsi myocutaneous flap.

The latissimus dorsi myocutaneous flap is a remarkably durable and versatile flap. Flap necrosis did not occur in any of our patients. One can safely carry with it skin segments as narrow as 3 cm, or as wide as 30 cm. In addition to the 5 cases presented, we have used the flap to repair axillary burn contractures, for breast reconstruction after a transverse incision, and for coverage of the upper arm and shoulder. The applications of this flap challenge the creative imagination of the surgeon and allow a simplified reconstruction, compared to other good methods. The newly described posterior

[Inflammatory and tumorous metastases of the thoracic spine: operative management and results (author's transl)].

133 patients with inflammatory or tumorous metastases of the thoracic spine were operated upon within the last 11 years. Radical removal of inflammatory lesions with consecutive corticalis-spongiosa-plasty, taken from the christa pelvis, and chemotherapy are a therapeutical unit. Excision of the tumor with supplement bone grafting or "Pallacosplombe" and stabilization according to the principals of osteosynthesis are rewarding in individual cases. Four out of nine tumor patients survived two years after surgery.

[Nystagmus as an objective assessment of the cervical spine syndrome and its treatment (author's transl)].

Nystagmus together with the history, X-ray and palpation findings were used to confirm the diagnosis of the cervical spine syndrome. In patients with this disorder the nystagmus was controlled by manual treatment in 63%. Nystagmus therefore can be used objectively to assess the success of therapy in cervical spine syndrome.

Therapeutic percutaneous embolization for extra-axial vascular lesions of the head, neck, and spine.

Therapeutic percutaneous embolization of extra-axial vascular tumors and arteriovenous malformations was performed 41 times in 27 patients. Twenty-one patients (78%) had a clinically favorable result. In 11 of these patients, the procedure was preoperative and caused a dramatic reduction of surgical blood loss. In the remaining 10 patients with a favorable result, therapeutic embolization alone resulted in a significant clinical amelioration documented by a detailed follow-up varying from 2 to 5 years. In patients with uncontrollable epistaxis, the procedure was life-saving. The guidelines and
