The number of reported cases of tuberculosis is on the increase. TB is a curable disease that usually affects the lungs and is transmitted through inhalation of droplets spread by coughing. Christine Bell describes how the nurse's role in the control of TB is vital to prevent further spread of the disease and to ensure that people who are affected successfully complete their course of treatment.

OBJECTIVE: To determine factors associated with no contacts identified for homeless patients in New York City.
DESIGN: Culture-confirmed pu … meless patients (n = 2836) (1 vs. 4, P < 0.001). Among homeless patients, having AFB smear-positive sputum with cavitary lesions reduced the likelihood of having no contacts identified. Homeless patients who lived on the street at the time of diagnosis were more likely to have no contacts identified compared to those with contacts identified (61.4% vs. 5 …

It has become clear in the last few decades that the primary underlying pathology of asthma is airway tissue inflammation. In asthma, airway remodeling occurs during chronic inflammation, even in very young children. One of the key goals in treating asthma is to identify those young children with an asthmatic phenotype and initiate early treatment to avoid irreversible airway remodeling. Distinguishing asthma from other conditions that induce wheezing is a daunting but critical step in the appropriate treatment of asthma. In some children, hallmark mediators of inflammation, such as increased

The purpose of this study, initiated in 1964 and concluded in 1967, was to define the distribution of Pseudomonas (now Burkholderia) pseudomallei in Thailand, to evaluate its importance as an etiologic agent, and to survey the presence of antibody in people that might indicate prior infection and/or contact with the microorganism.

A women, 50 years old, has been observed for 4 years because of recurrent infiltrations in both lungs. Biopsy of those lesions revealed sarcoid-like granulomas. Tubercle baccilli were not present in those lesions nor in the sputum or bronchial washings. Anti-tuberculosis therapy was without effect. On admission to our hospital she was in good performance state. There was an infiltration in the base of the left lung. Hepatosplenomegaly was observed on USG examination. Aspartate aminotransferase was 49 UI/l, alanine aminotransferase 70 UI/l. Alkaline phosphatase was 167 UI/l and the titer of ant…

In the mid-1980s, the incidence of tuberculosis (TB) in the United States increased. Recently, TB became an index diagnosis for AIDS. Chest radiographs in patients with AIDS and TB may be atypical; various features are addressed in this article. Radiologists should be aware of the possibility of TB because this disease is not only communicable to health care workers and other patients but more recently has become difficult to treat because of multidrug-resistant Mycobacterium tuberculosis.

The hemolysin/bacteriocin produced by some strains of Enterococcus faecalis is active in the lysis of human, rabbit, and horse erythrocytes, but not those from sheep. In this study, we determined that 20% of clinical enterococcal isolates tested in the clinical microbiology laboratory produced hemolysin and that pathogenic human E. faecalis were more likely to be hemolysin-producing isolates. Among the organisms isolated from different anatomic sites, variability in the degree of hemolysin production existed. We used an isogenic pair of E. faecalis organisms to demonstrate that hemolysin produ

This was a randomized, third-party-blinded, multicenter study that compared once-daily azithromycin (500 mg on day 1, followed by 250 mg on days 2-5) to cefaclor (500 mg three times daily for 10 days) in the treatment of patients with acute bronchitis or pneumonia. A total of 546 patients were entered into the study and 272 patients were evaluable for efficacy analysis. Of these, 249 (176 azithromycin, 73 cefaclor) had bronchitis and 23 (15 azithromycin, 8 cefaclor) had pneumonia. The combined clinical cure and improvement rate, as determined by the investigator, was 96% for azithromycin and 9

The incidence of bacteraemia and fungaemia was determined in 29 adults with cystic fibrosis (CF) during 50 consecutive admissions to hospital for management of infective exacerbations of pulmonary disease. Blood was drawn for aerobic, anaerobic and fungal cultures from all patients who were febrile on admission or who became febrile during treatment. The population included eight patients who had indwelling venous access systems in situ. The overall incidence of positive blood cultures in febrile patients was 3.5% [95% confidence interval (C.I.), 1-6%]. We recorded one case of Pseudomonas aeru

Human immunodeficiency virus (HIV) infection is associated with abnormalities of humoral immunity that result in an increased incidence of bacterial pneumonia. From 2% to 10% of acquired immunodeficiency syndrome (AIDS)-associated pneumonia is caused by encapsulated bacteria. Clinical features are usually typical of community-acquired pneumonia and include fever, productive cough, and chest pain. Focal radiographic infiltrates, an elevated WBC count, and mild hypoxemia are commonly observed. Streptococcus pneumoniae, Haemophilis influenzae, other Streptococcus species, and Branhamella catarrha

The bacteriological study of sputa, nasopharyngeal smears and bronchial washings taken from pneumonia patients has shown that the leading etiological agent was Streptococcus pneumoniae isolated in the diagnostic titre (10(7) bacteria per ml) in 78.1% of the cases. Staphylococcus aureus, Haemophilus influenzae, enterobacteria and yeast-like fungi have been found to play an insignificant role in the etiology of acute pneumonia (2.5 +/- +/- 0.9%). The results of the serological diagnosis by means of the complement fixation test have revealed that, alongside S. pneumoniae, the following infective

Meningiomas are the most common benign intracranial neoplasms, and although the otolaryngologist rarely sees these lesions, he or she may encounter them in surgery of the skull base. Surgical removal is the primary treatment modality for these meningiomas. To avoid recurrent disease, the best initial therapy is the most extensive procedure that can reasonably be effected. Chemotherapy and irradiation therapy have, at best, only limited roles in treatment.

Cytologic classification of subtypes of small-cell carcinoma of the lung (SCCL) was retrospectively performed on respiratory material of 90 cases of lung carcinoma cytologically diagnosed as SCCL. Additionally, the cytologic material was reexamined for the presence of variants of SCCL in 68 cases of lung carcinoma whose types had not been conclusively defined by cytology. The type of lung cytology specimens reviewed were sputa, bronchial washings and brushings and fine needle aspirations. The study included review of pretreatment tumor histology, when present, and examination of the ultrastruc

The author reports the observations over 190 patients referred to the Tuberculosis Institute for the present or suspected pulmonary tuberculosis of the lower lobe localization. In 47 (24.7%) of the patients the diagnosis of tuberculosis was rejected and lung cancer was diagnosed, central--in 5 of them, peripheral--in 42. Clinical manifestations in patients with tuberculosis and cancer were similar, weakness, cough, poor appetite being predominent; however, in cancer patients these were more frequently observed and more manifest with a tendency to aggravation and showing no response to tubercul

To evaluate the possibility of separate pulmonary infections in human beings by different strains of Mycobacterium tuberculosis, a search for multiple phage types within a single host was under-taken. Culture isolates from 2 or more distinct anatomic sites of infection in the same patient were obtained from 87 persons. In 3 subjects, 2 distinct phage types were found. The possible explanations for 2 types in the same patient and the epidemiologic implications are discussed.
