The increasing incidence of ciprofloxacin resistance in Streptococcus pneumoniae may limit the efficacy of the new quinolones in difficult-to-treat infections such as meningitis. The aim of the present study was to determine the efficacy of clinafloxacin alone and in combination with teicoplanin and rifampin in the therapy of ciprofloxacin-susceptible and ciprofloxacin-resistant pneumococcal meningitis in rabbits. When used against a penicillin-resistant ciprofloxacin-susceptible strain (Clinafloxacin MIC 0.12 microg/ml), clinafloxacin at a dose of 20 mg/kg per day b.i.d. decreased bacterial c…

Acute bacterial rhinosinusitis (ABRS) is a well-recognised and common problem confronting many primary care physicians but the abuse of antibiotic therapy for viral aetiologies of ABRS has lead to widespread bacterial resistance. The once easily-eradicated pathogens have developed many mechanisms to resist antimicrobial therapies. The most common pathogens Streptococcus pneumoniae, Haemophilus influenzae and Moraxella catarrhalis are still found in cultures of sinus cavities but Staphylococcus and possibly some anaerobes play an important role in the development of the chronic stage of rhinosi…

The discriminatory power, speed, and interlaboratory reproducibility of tRNA intergenic length polymorphism analysis (tDNA-PCR) combined with capillary electrophoresis was evaluated for the identification of streptococci. This method was carried out in three different laboratories under highly standardized conditions for 54 strains belonging to 18 different species. It was concluded that interlaboratory reproducibility of tDNA fingerprints produced by means of capillary electrophoresis was sufficiently high to permit the exchange between different laboratories and the construction of common li

Brain abscess is a serious, life-threatening infection. The infection can originate from contiguous sites of existing infections, such as chronic otitis media, dental infection, mastoiditis, or sinusitis, where anaerobic bacteria predominate. The infection can also occur in children with cyanotic congenital heart disease, in whom the predominant organisms are viridans, microaerophilic, or anaerobic streptococci, or after head trauma, in which case Staphylococcus aureus, viridans cocci, and Streptococcus pneumoniae are the most prevalent isolates. Enterobacteriaceae, Pseudomonas aeruginosa, yea…

Twelve patients hospitalized with acute exacerbations of chronic bronchitis (CB) and infected in the lower respiratory tract with H. influenzae (HI) or Streptococcus pneumoniae (SP) were examined. Bacteria, isolated from the expectorate caused an IgE-mediated histamine release from the patient's own blood leukocytes, indicating that all were sensitized to their own bacteria. Sensitization was only observed in some of the patients when tested with a standard panel of HI or SP obtained from other patients, indicating the importance of using the patient relevant bacterial antigenic determinants. …

Although antimicrobial agents from a number of classes have been used with a good degree of clinical success for acute bacterial exacerbations of chronic bronchitis, the incidence of resistance by beta-lactamase-producing strains to certain penicillins and cephalosporins continues to increase and represents a growing clinical problem. There also have been reports of significant resistance by Streptococcus pneumoniae to tetracyclines and of treatment failures caused by this organism among patients receiving fluoroquinolones. The emergence of penicillin-resistant pneumococci in North America als…

Streptococcus pneumoniae has recently developed resistance to almost every agent that has been used for therapy, including the extended-spectrum cephalosporins. The empiric therapy of penicillin-resistant pneumococcal meningitis should include cefotaximine or ceftriaxone plus vancomycin pending cephalosporin susceptibility results. Intermediate penicillin-resistant pneumococcal infections outside the central nervous system will usually respond to high dose intravenous beta-lactam antibiotic therapy. Highly penicillin-resistant pneumococcal infections may not respond to penicillin therapy, in w…

Eighty-four pediatric patients with bacterial meningitis were prospectively evaluated while receiving cefuroxime (200 mg/kg/day in four equal intravenous doses) as single-drug therapy for 9 to 13 days. Six cases were admitted in extremis and died within a few hours because of irreversible central nervous system damage or shock. The remaining 78 patients were cured, and prompt bacteriological and clinical responses were noted. The pathogens were Haemophilus influenzae b (43 cases), Neisseria meningitidis (20 cases), Streptococcus pneumoniae (10 cases) and unknown (five cases). All pathogens wer…

A total of 348 cases of meningitis caused by Streptococcus pneumoniae were reported to and investigated by the New York City Department of Health during the six years 1973-1978. The descriptive epidemiology and completeness of reporting of this disease entity are presented. The total crude incidence was estimated at 1.46 cases per 100,000 population per year. The high-risk age categories were less than 5 years and greater than or equal to 65 years. The incidence of pneumococcal meningitis for blacks with sickle cell disease was much greater than that for other blacks, who, in turn, had a highe…

Auditory brainstem response (ABR) was used to assess possible hearing loss in 60 patients recovering from bacterial meningitis. The ABR resu … s. As expected, otitis media occurred significantly with conductive hearing loss. Type of pathogen (Streptococcus pneumoniae) and hospitalization greater than two weeks were significantly correlated with sensorineural hearing loss. As meningitis typically affects young children who are difficult to test with conventional audiometry, ABR provides an effective means of test …

Streptococcus pneumoniae causes substantial morbidity and mortality. Incidence and severity are increased among populations with some chronic diseases. The currently available polyvalent polysaccharide vaccine induces antibody production among immunologically competent recipients against the 14 serotypes responsible for 80% of pneumococcal bacteremia in the efficacious in clinical trials with healthy young men in epidemic conditions and in patient with sickle cell anemia. Similar trials in two other high-risk populations had inconclusive results. Decisions on vaccine use now largely rest on in…

Intubation of the tympanic membrane as a treatment for acute otitis media is extremely controversial. This study was designed to determine the effect of tympanic membrane intubation in an experimental model for acute purulent otitis media. The right tympanic membrane of the chinchilla was intubated with Paparella Type I tympanostomy tubes. This procedure was followed in seven days by nasopharyngeal inoculation of Streptococcus pneumoniae and in an additional three days by nasopharyngeal inoculation of influenza A virus. Nasopharyngeal inoculation with these microorganisms results in acute puru…

BACKGROUND: Mechanically ventilated patients are highly susceptible to poor oral hygiene and ventilator-associated pneumonia, mainly due to pathogenic bacterial colonization in the oral cavity. While antiseptic mouthwashes are commonly used, they may disrupt natural oral microbiota. Probiotics offer a potential alternative by promoting beneficial microbial colonization, enhancing oral hygiene, and reducing the risk of infection. This study aimed to evaluate the effect of probiotic colonization on oral hygiene and the incidence of ventilator-associated pneumonia in mechanically ventilated patie

Identifying conserved, immunogenic proteins that confer protection against Streptococcus pneumoniae (pneumococcus) colonization could enable development of serotype-independent vaccines. In our controlled human infection model, no individual IgG or cytokine/chemokine response correlated significantly with protection against colonization with pneumococcus, suggesting that effective immunity reflects a coordinated, multi-antigen response. To capture these complex patterns, we trained independent Random Forest models on humoral and cellular datasets. The humoral model identified IgG responses to …

Bacterial meningitis represents a devastating inflammatory disease of the central nervous system (CNS), characterized by the invasion of pat … lay between bacterial virulence factors and neuroinflammatory cascades, with particular emphasis on Streptococcus pneumoniae as a model pathogen. By integrating recent advances in molecular pathogenesis and translational immunology, this review provides a framework for developing precision therapies to mitigate inflammation-mediated CNS damage in bacterial meningitis.

Streptococcus pneumoniae (pneumococcus) remains a major global cause of invasive human disease with clear seasonal variation suggesting a role for environmental factors. The pneumococcus includes >100 serotypes and many lineages with different disease patterns. Interactions between microbial diversity, environmental factors, and invasive pneumococcal disease (IPD) remain unclear. We examined the effects of air pollution and meteorological factors on 59,017 IPD cases over 15 years, including 4,350 genome-sequenced isolates, using Bayesian spatio-temporal hierarchical models that link environmen…

The complement system is a central component of innate immunity, mediating opsonization, chemotaxis, cytolysis, and shaping adaptive responses. Deficiencies in complement proteins, whether inherited or acquired, predispose to severe infections, particularly with encapsulated bacteria such as Neisseria meningitidis and Streptococcus pneumoniae. Although rare, inherited defects affect different pathways and may also present with autoimmune or renal diseases. Diagnosis relies on functional and quantitative assays, especially in patients with early-onset or recurrent infections. Complement inhibit…
