The Successful Use of Bilateral 2-Level Ultrasound-Guided Stellate Ganglion Block to Improve Traumatic Brain Injury Symptoms: A Retrospective Analysis of 23 Patients.

PURPOSE: The purpose of the study was to determine whether performing ultrasound-guided, bilateral stellate ganglion blocks (SGBs; performed on subsequent days) improved traumatic brain injury (TBI) symptoms. METHODS: A retrospective chart review was conducted for the time period between August 2022 and February 2023 to identify patients who received bilateral, 2-level (C6 and C4) SGBs for PTSD symptoms but who also had a history of TBI. Neurobehavioral Symptoms Inventory (NSI) scores were collected at baseline, 1 week, and 1 month post-treatment in 14 males and 9 females. RESULTS: Out of 23

Mild Traumatic Brain Injury: Striking Postconcussion Symptoms Due to Inadequate Management.

Pediatric mild traumatic brain injury is a frequent cause for emergency consultations. Very often, management decisions focus only on acute neurological problems, without considering possible long-term impairment. Our case describes a 14-year-old girl who developed a pronounced and prolonged postconcussive syndrome and subsequent posttraumatic stress symptoms after (mild) traumatic brain injury. Early discharge without adequate instructions about the appropriate time to return to school and daily life activities promoted these striking problems. Only the delayed interventions including reducti

Neural differences underlying face processing in veterans with TBI and co-occurring TBI and PTSD.

BACKGROUND: Traumatic brain injury (TBI) is common in military personnel and associated with high rates of posttraumatic stress disorder (PTSD). TBI impacts widely-distributed neural patterns, some of which influence affective processing. Better understanding how TBI and PTSD/TBI alters affective neural activity may improve our understanding of comorbidity mechanisms, but to date the neural correlates of emotional processing in these groups has been relatively understudied. METHODS: Military controls, military personnel with a history of TBI, and military personnel with both TBI and PTSD (N =

[Clinical aspects of mental disorders in children with burns acquired in a railroad accident].

The authors provide the results of the follow-up and examination of 57 children with burn disease due to the railway accident. Describe psychopathological disorders which developed in the victims at different stages of burn disease: burn shock, toxemia, septic toxemia , and convalescence. Lay emphasis on the fact that in formation of the psychopathological symptomatology in the acute period, of importance were thermal, traumatic, intoxication and psychogenic factors; during convalescence, of great importance was the appearance of postburn scars on the open parts of the body. The authors distin

Health Outcomes of Traumatic Brain Injury Among Refugee Survivors of Torture.

OBJECTIVE: To compare spontaneous reporting of health complaints in a sample of refugee survivors of torture with a history of moderate/severe traumatic brain injury (TBI) with survivors of torture without TBI and analyze the contribution of posttraumatic stress disorder symptoms to health outcomes. PARTICIPANTS: Treatment-seeking refugee survivors of torture with a moderate/severe TBI (n = 85) and a control group (n = 72) of survivors who suffered a physical injury during their persecution but had no history of a head injury. MEASURES: Health outcomes included a self-report of general physi

Enhancing quality of mental health service for veterans with seizures.

Psychiatric disorders are commonly identified in people with seizures (PWS). Comorbid mood, anxiety, and trauma- and stressor-related disorders are highly prevalent and not consistently recognized or treated promptly. Veterans with seizures are particularly vulnerable to neuropsychiatric comorbidities, including a higher prevalence of posttraumatic stress disorder and traumatic brain injury. In this article, we describe an overview of common mental health issues, their impact on quality of life, and available resources from the US Department of Veterans Affairs (VA) Epilepsy Centers of Excelle

Neuropsychiatric sequelae after liver transplantation and their possible mechanism via the microbiota-gut-liver-brain axis.

Patients after liver transplantation are often impacted by mental and even neuropsychiatric disorders, including depression, sleep disorders, anxiety, and post-traumatic stress disorder. Neuropsychiatric sequelae have an adverse impact on rehabilitation and can even incapacitate people, reducing their quality of life. Despite screening tools and effective treatments, neuropsychiatric sequelae after liver transplantation (NSALT) have not been fully diagnosed and treated. Current research suggests that NSALT may be partly related to intestinal microbial variation, but the detailed mechanism rema

COVID-19 Pandemic and Impending Global Mental Health Implications.

The increase in organisms transference and infectious pandemics across the globe have been accelerated by an increase in travel, international exchange and global changes in earth's climate. COVID-19, a virus caused by the novel coronavirus that was initially identified on December 2019, in Wuhan city of China is currently affecting 146 territories, states and countries raising distress, panic and increasing anxiety in individuals exposed to the (actual or supposed) peril of the virus across the globe. Fundamentally, these concerns ascend with all infections, including those of flu and other a

Life Stressors and Posttraumatic Stress Disorder in a Seriously Mentally Ill Jail Population.

Inmates represent a vulnerable population with increased rates of trauma and posttraumatic stress disorder (PTSD). However, little is known about the rates of trauma and PTSD among male inmates with acute psychiatric illness. This prospective, randomized study was conducted to assess the current rates of trauma and PTSD in this population. The sample consisted of 48 patients admitted to a hospital jail psychiatry service in New York City. Subjects were administered the Life Stressor Checklist-Revised and the Structured Clinical Interview for DSM-IV-TR Disorders, PTSD Module (SCID-I). The rate

[Treatment with psychedelics : potential benefits in Parkinson's disease].

Psychedelics, such as psilocybin and lysergic acid diethylamide, modulate neuroplasticity and brain connectivity via 5-HT2A receptors. Their efficacy has been demonstrated in depression and anxiety, where they are particularly interesting because of their rapid and long-lasting effect. They could also be beneficial for addictions, post-traumatic stress, and obsessive-compulsive disorder. This article explores their therapeutic potential in Parkinson's disease (PD), both symptomatically in PD depression, anxiety, and impulse control disorders, and in terms of neuroprotection. Psychedelics actua

Mental Health in the Youth Athlete.

Though research is inconclusive in being able to determine if young athletes are more or less likely to suffer from mental health disorders compared with their non-athlete peers, there are important psychological considerations that are unique to the athletic population. This includes depression in the context of overtraining and burnout, performance anxiety, perfectionism, psychological sequalae of concussion, and injury as an independent risk factor for depression, anxiety, post-traumatic stress, and high-risk behaviors. Optimization of mental health care in youth athletes requires continued

Interventional Mental Health: A Transdisciplinary Approach to Novel Psychiatric Care Delivery.

Mental health disorders are among the most common health conditions in the United States. Traditional clinical treatments rely on psychiatric counseling and, in many cases, prescription medications. We propose an innovative model, Interventional Mental Health, which employs a combination of modalities through a multifaceted approach to treat conditions that have exhibited limited responsiveness to traditional methods and individuals afflicted with multiple comorbidities simultaneously. We hypothesize that creating a unique treatment algorithm combining current therapeutic modalities such as St

Temporal Structure of Mixed States: Does Sensitization Link Life Course to Episodes?

Susceptibility to combined depressive and manic syndromes correlates strongly with arousal-related symptoms including impulsivity, anxiety and agitation. This relationship to a driven, "mixed" activation-depression state, generated by a life-long process, was described in classical times. Course of illness in mixed states includes increased episode frequency, duration, earlier onset, and association with addiction- and trauma/stress-related disorders. Mixed episodes have catecholamine and hypothalamic-pituitary-adrenocortical activity increased beyond nonmixed states of similar symptom severit

Cannabinoid use generalizes stress responses by altering the astrocyte plasticity through extracellular matrix signaling in the nucleus accumbens core.

The rising legal acceptance of cannabis and the high comorbidity between cannabis use disorder (CUD) and post-traumatic stress disorder (PTSD) highlight the importance of understanding how stress and cannabis influence the brain. We recently discovered that cannabis use promotes two PTSD-like symptoms: avoidance coping behaviors and the generalization of stress-coping responses to a neutral stimulus not previously linked to stress. To investigate the neuroadaptations behind these changes, we used in vivo zymography and confocal microscopy to examine how stress and cannabinoid use influence mul

Childhood trauma and inflammatory biomarker effects on cortical thinning in schizophrenia spectrum disorders.

Patients with schizophrenia spectrum disorders (SSD) have higher risks for early and adult life traumatic events and suffer from a heightened body response to stress and increased inflammatory activities. We hypothesize that in SSD, the effect of stress is associated with prolonged activation of the inflammatory system and causes elevation in immune markers. We examined the effects of childhood trauma and adult stressful life events on a C-reactive protein (CRP) and their combined contribution to cortical thickness thinning in 49 SSD patients and 26 healthy controls. Participants with SSD repo

Heart period and variability findings in preschool children with posttraumatic stress symptoms.

BACKGROUND: Traumatic experiences for young children might result in profound neurodevelopmental changes, compared with adults. Our aim was to examine autonomic control of heart rate in traumatized young children. METHODS: Sixty-two children who had suffered traumas and 62 nontraumatized control children, aged 20 months to 6 years, were assessed for posttraumatic stress disorder (PTSD) symptoms, interbeat interval, respiratory sinus arrhythmia (RSA), family rehearsal of the trauma, and parent-child relationship quality. RESULTS: Traumatized children with PTSD and traumatized children without

Intranasal insulin administration improves anxiety- and depressive-like behaviors in PTSD-exposed rats.

BACKGROUND: Post-traumatic stress disorder (PTSD) is a debilitating mental illness that arises after experiencing traumatic events. Insulin is involved in several nervous system functions through widespread receptors across the central nervous system (CNS). We aimed to study the effects of intranasally delivered insulin on behavioral impairments in an animal model of PTSD. METHODS: The single prolonged stress (SPS) protocol, which involved 2 h of restraint, 20 min of forced swimming, and exposure to vaporized diethyl ether, was used to induce PTSD-like symptoms in adult male rats. Insulin was

Effect of acute posttrauma propranolol on PTSD outcome and physiological responses during script-driven imagery.

INTRODUCTION: Animal and human research suggests that the development of posttraumatic stress disorder (PTSD) may involve the overconsolidation of memories of a traumatic experience. Previous studies have attempted to use pharmaceutical agents, especially the β-adrenergic blocker propranolol, to reduce this overconsolidation. AIMS: In this randomized, placebo-controlled study of the efficacy of propranolol in reducing the development of PTSD, we optimized dosages and conducted both psychophysiological and clinical assessments 1 and 3 months after the traumatic event. Daily associations between sleep quality, stress, and cannabis or alcohol use among veterans.

United States military veterans face heightened vulnerability to sleep disturbances due to factors such as irregular sleep schedules, combat-related stress, and co-occurring mental health disorders. These sleep disturbances are often exacerbated by substance use, including alcohol and cannabis, as veterans may rely on these substances to self-medicate for stress and sleep issues. However, the interplay between sleep quality, substance use, and perceived stress remains poorly understood, particularly on a day-to-day basis. This study aimed to explore the dynamic associations between these facto
