Congenital esophageal stenosis: a rare case of dysphagia.

Congenital esophageal stenosis (CES) is a rare anomaly, resulting from incomplete separation of the respiratory tract from the primitive foregut at the 25th day of life. First clinical signs are abnormalities of the swallowing mechanism caused by the intrinsic narrowing of the esophagus. Diagnosis is usually delayed, requiring an accurate history and high level of suspicion, alongside with an esophagogram. Definite diagnosis is only confirmed by histological examination. Treatment usually involves surgery, depending on the severity, location and type of stenosis. We report the case of an 18 mo

[Colon cancer after colon interposition for oesophageal replacement].

The authors report the case of a colon adenocarcinoma developed on the neck at the anastomosis of the skin tube and colon 44 years following a corrosive oesophageal injury. This patient suffered a moderately severe oesophageal, stomach and laryngeal injuries due to drinking hydrochloric acid 44 years ago. He underwent serial laryngoplasties, then needed a tracheostomy, oesophagectomy, pyloroplasty and ileocolon transposition. An antethoracal oesophagus formation was performed with ileocolon and skin tube amendment. 44 years later an ulcerated adenocarcinoma developed in the transposed colon, w

Sudden dysphagia in an elderly, quadriparetic patient.

CLINICAL CASE: We report the case of a 92-year-old quadriparetic patient who suddenly presented a major swallowing disorder following trauma to the neck. A diagnosis of myasthenia gravis was suggested by single-fibre EMG of the extensor digitorum communis muscle. In view of the continued absence of dysphagia after 6 months of pyridostigmine treatment, this diagnosis was considered as definitive. DISCUSSION: We review the various characteristics of myasthenia gravis and ways of investigating dysphagia. CONCLUSION: It is important to be aware of and investigate all the direct and indirect sign

[Madelung's disease].

Madelung's disease (Md) is a rare metabolic disorder which presents with multiple symmetrical nonencapsulated fatty masses situated mainly around the head, neck and nape. The main indications for Md treatment are: shortness of breath, head's movement restrictions, swallowing disorders (dysphagia), aesthetic considerations, and the fear of cancer. The standard treatment of Md is the surgical excision of pathological tissues. The disease is progressive, relapses after removal of tissue are often observed. The discussion is based on a three-year observation of Md patient operated by the authors.

Human papillomavirus and head and neck cancer: a growing concern.

Head and neck cancer is increasing in incidence in patients without the usual risk factors for the disease. Practitioners need to be aware that young, non-smoking patients are also at risk for certain types of head and neck cancer. Head and neck cancer in this patient group is likely due to infection of the tonsil and tongue with high-risk strains of human papillomavirus (HPV). There is strong epidemiological and laboratory evidence that HPV is a cause of head and neck cancer. Therefore, any patient with persistent lesions, ulcers, swallowing difficulty, change in voice, or neck mass needs pro

Presentations of agenesis of the hemidiaphragm in an adult.

A 67-year-old man presented with chest tightness and vomiting of one-week duration. On physical examination bowel sounds were heard on the left chest. Plain chest x-ray, barium swallow, and CT scan suggested gastric volvulus with diaphragmatic hernia. At laparotomy there was complete agenesis of the left hemidiaphragm with no diaphragmatic remnants seen and no associated lung hypoplasia. This is an extremely rare condition and careful assessment is needed to differentiate between diaphragmatic hernia and agenesis of the diaphragm as more cases could be diagnosed in the future as a result of go

Gastrointestinal disorders in elderly patients.

Gastrointestinal disorders are common in elderly patients, and the clinical presentation, complications, and management may differ from those in younger patient. Most impairment occurs in the proximal and distal tract of the gastrointestinal system. Swallowing abnormalities with a wide span of symptoms and pelvic floor pathologies involving all the pelvic compartments are common. Acute abdomen, often from small bowel obstruction or mesenteric ischemia, can pose a diagnostic challenge, because a mild clinical presentation may hide serious visceral involvement. In this setting, the radiologist o

Large pyogenic granuloma: a case report.

Though pyoginic granuloma is a common occurrence in the oral cavity, it has got particular significance because of its unexpected clinical course. Here a case of large pyogenic granoloma is presented in a 30-year-old female reported at the dental OPD of VSS Medical College and Hospital, Burla with the complaint of a growth in the mouth from the right upper last tooth since one and half years associated with difficulty in swallowing and breathing. The lesion was excised under local anaesthesia along with extraction of upper right second molar. The patient recovered without any complication and

Dysphagia and hoarseness associated with painless aortic dissection: a rare case of cardiovocal syndrome.

Cardiovocal syndrome (Ortner's syndrome) is characterized by left recurrent laryngeal nerve palsy due to cardiovascular disease, but in rare cases it can also be caused by aortic dissection. An 81-year-old man with hypertension was admitted to the hospital with aspiration pneumonia. He had been developing progressive dysphagia and hoarseness for several months before admission. A videofluoroscopic swallowing study showed supraglottic penetration with barium paste and liquid. Laryngoscopy and electromyography revealed left vocal cord palsy caused by left recurrent laryngeal neuropathy, and a co

Pediatric postcricoid vascular malformation: a diagnostic and treatment challenge.

Hemangioma of the upper aerodigestive tract is a rarely reported occurrence in the pediatric literature. To date, there have been three published case reports of postcricoid hemangiomas contributing to unexplained dysphagia and respiratory distress. We present three children with a history of swallowing difficulty and stridor who were found to have an occult postcricoid mass. Valsalva maneuvers confirmed the suspicion of a vascular malformation in both patients. Transoral laser therapy (KTP and CO2) was used to ablate the lesions. The patients are symptom-free at 5 months, 5 years, and 2 weeks

[Intralingual dermoid cyst: imaging features of a giant cyst].

Dermoid cysts of the oral cavity are rare and most commonly involve the floor of mouth. Intralingual dermoid cysts are extremely rare and usually large at the time of diagnosis in adult patients. We report the case of 6 year old girl with macroglossia and swallowing and respiratory difficulties due to a giant intralingual dermoid cyst. Ultrasonography showed a large cystic intralingual lesion. MRI allowed accurate evaluation of the size, the extension and the relationship of the cystic mass with adjacent structures, the fat component of the cystic mass confirmed the dermoid origin. Progressive bulbur paralysis (Fazio-Londe disease).

Progressive bulbar paralysis of childhood is characterised by progressive paralysis of muscles innervated by cranial nerves. The authors report a case of progressive bulbar paralysis of childhood in a 12-year-old child. Child was admitted with the complaints of drooping of eyelids, difficulty in swallowing and hoarse voice. She had involvement of III, VII, IX, X, XI and XII cranial nerves and the corticospinal tracts. Electromyography revealed spontaneous activity in the form of fasciculations, giant motor unit potential and discrete recruitment of motor neurons suggestive of denervation patte

Arteria lusoria as a cause of dysphagia.

Aberrant right subclavian artery is a rare vascular anomaly. Dysphagia caused by this anomaly usually develops in older patients due to increased rigidity of the oesophagus itself or vessel wall and elongation of the aorta. In this paper, a 10-year-old boy patient suffering from difficulty in swallowing since his infancy is presented. Barium contrast examination of the oesophagus showed indentation on the oesophagus and MRI angiography revealed an aberrant right subclavian artery. Surgical correction totally relieved the symptom of the dysphagia. It was concluded that this kind of vascular ano

Analysis of 683 drug packages seized from "body stuffers".

The wrappings and contents of 683 drug containers (bubbles) seized from 154 street dealers have been analysed since 2001, when the law enforcement policy in Hamburg, Germany, allowed the application of emetics to be used by so-called body stuffers for the recovery of swallowed drug packages. Of the bubbles recovered 96% contained cocaine. Cases of intoxication have not yet been observed but a 20-year-old man with a previously unknown toxic cardiomyopathy died 3 days after a sudden cardiac arrest which followed the forced application of ipecacuanha via correctly placed nasogastral tube.

[Aneurysm of the internal carotid artery--a differential diagnosis of paralysis of the caudal cranial nerves].

Paralysis of the caudal cranial nerves, e. g. the nervus glossopharyngeus, vagus and accessorius, may cause disorders in swallowing and speaking leading to a reduction in the patient's quality of life. Glomus tumors or malignant lesions of the skull base are a frequent cause of such lesions. We report on the case of a 48 year old patient who presented an acute lesion of these cranial nerves in combination with paresis of the nervus hypoglossus as a result of an aneurysm of the internal carotid artery directly underneath the base of the skull. The aneurysm was treated by parent vessel occlusion

Tonsillar tuberculosis associated with pulmonary and laryngeal foci.

Tonsillar tuberculosis is one of the uncommon forms of extrapulmonary tuberculosis. We report a case of tonsillar tuberculosis associated with pulmonary and laryngeal foci. A 23-year-old female was admitted for evaluation of hoarseness and difficulty in swallowing. Bilateral palatine tonsils were enlarged, and a tonsillectomy was performed. Since a histological study revealed tonsillar tuberculosis, antituberculous agents were administered. After the treatment the pulmonary lesions detected with chest computed tomography were improved, and her symptoms were relieved. The possibility of tonsill

We report the case of a hyoid syndrome caused by a developmental anomaly of the second branchial cleft, presenting at adult age by dysphagia without any abnormality detected at the barium swallow and at naso-pharyngeal endoscopy, first examinations to perform in case of dysphagia. The MRI findings of this anomaly showed a hyperintense well-limited vallecular mass syndrome. The diagnosis of hyoid bone anomaly was established at spiral CT with 3D reconstructions showing an incurvated and elongated lesser cornua causing persistant impingement on the lateral wall of the oropharynx. Patient with purely extraocular manifestations from a pit viper snakebite (Agkistrodon halys brevicaudus).

Snake venom can cause myriad local and systemic signs and symptoms. Neurotoxic effects include difficulty seeing, diplopia, difficulty in opening the mouth, speaking, or swallowing, and difficulty getting out of bed the morning after the snakebite. Because of the unique structure of the extraocular muscles, they are particularly susceptible to the neurotoxin. Rarely, though, are symptoms of snakebite confined to the extraocular muscles. This case report describes a patient who experienced only extraocular manifestations of envenomation. It is important for clinicians to recognize this unique n

Diagnosis and management of diffuse leiomyomatosis of the oesophagus.

Diffuse leiomyomatosis of the oesophagus is a rare entity among oesophageal diseases. Histopathologically it is characterized by diffuse hypertrophy of the muscular layer extending to the whole oesophagus predominantly in the lower third, where it can result in tumour formation. Leiomyomatosis can involve the upper part of the stomach and is frequently associated with genital or tracheobronchial (bronchitracheal) muscular localizations. Also, it can be associated with Alport's syndrome in familial cases. For diagnosis, barium swallow, computerized tomography (CT) scan and, in recent years, end
