Bennett fracture is the most common fracture of the thumb. Choosing the appropriate approach to fracture fixation requires a thorough knowledge of the anatomy surrounding the first carpometacarpal joint, which is necessary to prevent injury to local sensory nerves and tendons. Although no study has shown superior outcomes compared with open reduction internal fixation and fluoroscopically guided closed reduction and percutaneous pinning, arthroscopic-assisted fixation allows for debridement of the carpometacarpal joint, direct visualization of the articular surface during reduction, and has mi

The ideal method of irrigation and débridement for severe extremity wounds has yet to be determined. This report demonstrates the use of hydrosurgical débridement in the treatment of highly contaminated acute forearm fractures in a 22-year-old man ejected during a motor vehicle crash in a farm area. The result was rapid, selective, and effective débridement of deeply embedded material, which allowed for expeditious reconstruction with internal fixation, tendon transfers, and groin flap coverage while avoiding infection and injury to vital structures. The technique is described in detail and th

This report presents a case of direct injury to the dorsal sensory branch of the ulnar nerve caused by arthroscopic repair of the triangular fibrocartilage complex. The dorsal sensory branch of the ulnar nerve was strangulated by one of the three pull-out sutures of the joint capsule, just ulnar to the extensor carpi ulnaris tendon. Pain and dysaesthesia of the ulnar side of the wrist was completely relieved after excision of the injured nerve segment. This complication can be avoided by careful exploration of the dorsal sensory branch of the ulnar nerve prior to suturing or passage of instrum

The history of modern surgery of the hand in Japan, along with the establishment of the Japanese Society for Surgery of the Hand in 1957, is described in the following article. Major and unique contributions of Japanese surgeons in various aspects of hand surgery are introduced. These contributions encompass microvascular and replantation surgery, tendon repair and grafting, neurotization in brachial plexus injury, reconstruction of nail-bed and paronychial tissues for nail regrowth and that of post-traumatic contracture, congenital anomalies, and osteonecrosis of the lunate. These contributio

Three cases of musculocutaneous nerve injury distal to its innervation of the coracobrachialis muscle are reported. The syndrome typically features painless weakness of the biceps brachii and brachialis muscles, sensory loss in the distal volar forearm, a history of recent vigorous upper extremity resistive exercise, and resolution of the problem on resting the affected extremity. In all three reported cases the dominant arm was involved. The condition should not be confused with C-5 or C-6 radiculopathy, brachial plexopathy, or rupture of the biceps brachii muscle belly or tendon.

BACKGROUND: Loss of finger motion after zone II flexor tendon repair remains common and is largely caused by adhesion formation. Although intrinsic vascular and cellular contributions to tendon healing have been examined in canine, avian, and primate models, prior studies have emphasized biochemical or perfusion-based endpoints without comprehensive assessment of functional and mechanical outcomes after clinically relevant repair. Consequently, the influence of intrinsic healing elements on early postoperative function remains unclear. In this study, we employed a novel avian model of intrasyn

Transection of the radial nerve is frequently associated with humeral shaft fractures that are part of a very complex upper extremity injury. In the presented case, a 19-year-old man with a 10-cm radial nerve defect with a need for nerve grafting to recover complete sensory and motor deficit of the radial nerve. In our case, at the same time we provided the tendon transfer of musculus (m.) pronator teres to m. extensor carpi radialis brevis, m. flexor carpi ulnaris to m. extensor digitorum communis, m. palmaris longus to m. extensor pollicis longus, and long sural nerve graft because of an ext

Initial imaging evaluation for a variety of knee pathologies often begins with a radiographic series. Depending on the specific indication, this will include at least two different projections of the knee. In most cases, these are the anteroposterior and lateral radiographs of the affected knee, and sometimes with the contralateral knee for comparison. Typically, knee pathologies visible on lateral view can also be appreciated on the anteroposterior view. However, several pathologic processes occur in anatomic locations typically obscured on other projections because of superimposed osseous st

Tears of peroneus brevis tendon represent a cause of underdiagnosed lateral ankle pain and instability. The typical clinical presentation is retro-malleolar pain, in some cases associated with palpable swelling around the fibular malleolus, pain during activities and difficulty in walking. We present a case of peroneus brevis split lesion with superior peroneal retinaculum avulsion in a young athlete who referred to the emergency ward of our hospital for left ankle pain after an inversion injury. An early diagnosis allowed treating the injury and promptly resuming sport activity, after rehabil

The subscapularis is the strongest rotator cuff muscle in the shoulder. Until recently, most surgical repairs were performed through an open approach. Furthermore, the reporting of arthroscopic repair techniques and outcomes is fairly limited. This is likely due to the difficulty involved in all-arthroscopic repairs and the relative rarity of this injury compared with injuries of the other rotator cuff tendons. Arthroscopic subscapularis repair can be challenging, even for an expert surgeon, and attempting a double-row repair adds further complexity. Given these issues, this article provides t

The purpose of this study is to present the results of a new intramedullary fixation technique for metacarpal fractures. A J-shaped nail, which is a curved 2.0 mm diameter Kirschner wire sharply bent at the proximal end, was inserted from the dorsal aspect of the metacarpal base. Twenty-one metacarpal fractures (five transverse shaft fractures and 16 neck fractures) in 19 hands of 18 patients with were operated by this technique. All fractures had successful unions. Clinical and radiographic results were excellent in all ten hands that had excluded concomitant injuries. There was no mechanical

Although permanent sequelae of deep burns always persist, they can be very favourably influenced if we start soon after the injury. There are several possibilities: very early rehabilitation by positioning and supports prevents the shortening of tendons and ligaments surrounding the large joints and thus reduces post-traumatic oedema. By selecting the correct surgical technique at the right moment, we achieve an optimal course of healing and scar formation. Great attention must be paid to infection and its prevention. After healing it is important to apply compressive aids soon, preferably in

Fractures of the humerus, scapula and clavicle usually result from a direct blow or a fall onto an outstretched hand. Most can be treated by immobilization. Dislocation of the humerus, strain or sprain of the acromioclavicular and sternoclavicular joints, and rotator cuff injury often can be managed conservatively. Recurrence is a problem with humerus dislocation, and surgical management may be indicated if conservative treatment fails. Rotator cuff tears are often hard to diagnose because of muscle atrophy that impairs the patient's ability to perform diagnostic maneuvers. Chronic shoulder pr

Background The anterior cruciate ligament (ACL) is one of the primary stabilisers of the knee, and injury to it often requires surgical intervention. ACL injuries are common sports injuries that tend to occur during sudden deceleration, pivoting, or landing movements, resulting in knee instability and decreased functionality. Different graft options have been proposed over the years for reconstructing the ACL. Objective This study aimed to evaluate the short-term functional, clinical stability, and biomechanical (muscle strength) outcomes following arthroscopic ACL reconstruction using a quadr

BACKGROUND: The metacarpophalangeal joint (fetlock) is the most commonly affected site of racehorse injury, with multiple observed pathologies consistent with extreme fetlock dorsiflexion. Race surface mechanics affect musculoskeletal structure loading and injury risk because surface forces applied to the hoof affect limb motions. Race surface mechanics are a function of controllable factors. Thus, race surface design has the potential to reduce the incidence of musculoskeletal injury through modulation of limb motions. However, the relationship between race surface mechanics and racehorse lim

OBJECTIVE: Achilles tendon ruptures lead to long-term structural and functional deficits. Prior research that sought to identify optimal rehabilitation techniques was fundamentally limited by the inability to continuously monitor Achilles tendon loading during rehabilitation. Our objective was to develop a data-driven model that predicts per-step peak Achilles tendon loading from only a single, boot-mounted accelerometer.
METHODS: Nineteen patients recovering from an acute Achilles tendon rupture completed in-lab walking trials while wearing an instrumented immobilizing boot. A boot-mounted i

Percutaneous Bunnell repair and open modified Kessler repair remain debated options for acute Achilles tendon rupture (AATR). We retrospectively compared a minimally invasive percutaneous Bunnell technique (Group A) with an open modified Kessler repair (Group B) within a standardized early functional rehabilitation (EFR) protocol at a single center. Fifty-five adults with closed AATR treated between January 2021 and December 2022 were analyzed (Group A, n = 25; Group B, n = 30). Between-group comparisons used Welch t tests for continuous variables and χ² or Fisher exact tests for categorical v

Latissimus dorsi (LD) tendon transfer is widely used to treat irreparable rotator cuff tears, particularly in cases with posterosuperior and anterosuperior tear patterns. We conducted a comprehensive anatomical analysis of the attachment of the LD tendon to the humerus, focusing on its morphological patterns and morphometric features, which are critical for optimizing the surgery. Dissection of 32 LD tendons in 16 Korean cadavers revealed three insertion patterns: fully combined (40.6%), partially combined (34.4%), and fully separated (25.0%). The mean width, length, and thickness of the tendo

OBJECTIVE: To improve postoperative outcomes in patients with distal Achilles tendon rupture by using of a new method of surgical treatment.
MATERIAL AND METHODS: The authors proposed a new method to restore the Achilles tendon for its distal rupture (within 2 cm from attachment to the calcaneus). This approach implies forming a canal in the calcaneus and minimally invasive harvesting of the plantar tendon. At the next stage, the plantar tendon is passed through the canal of the calcaneus in U-shaped fashion and proximal part of Achilles tendon with maximum approximation of its fragments and
