Experiences of Healthcare Professionals in a Street Clinic in a Municipality in Southern Brazil.

The Street Clinic (Consultório na Rua-CnR) is a strategic component of Primary Health Care in Brazil, aimed at populations experiencing homelessness, a group characterized by high levels of social and health vulnerability. This study critically analyzes the experiences of healthcare professionals working within a CnR team, identifying the meanings attributed to their work, the challenges encountered, and the strategies developed within the territory. This is an exploratory study with a qualitative approach, grounded in health narratives and the philosophical hermeneutics of Hans-Georg Gadamer.

Coronary Computed Tomography Angiography Identifying a Single Coronary Artery With a Critical Left-Main-Equivalent Stenosis Treated by Elective PCI.

Single coronary artery (SCA) is a rare congenital anomaly in which the entire myocardium is supplied by a single coronary ostium, and clinical significance depends largely on the anatomical course and presence of concomitant atherosclerotic disease. We report the case of a 58-year-old man presenting with exertional chest pain whose coronary computed tomography angiography revealed an SCA arising from the right coronary sinus with a critical 98% stenosis in the left main equivalent trunk supplying both the left anterior descending and circumflex territories. Although coronary artery bypass graf

Double Jeopardy - A Case of Overlapping Takotsubo Syndrome and Spontaneous Coronary Dissection.

Acute coronary syndrome (ACS) includes a spectrum of clinical conditions, with increasing recognition of myocardial infarction with non-obstructive coronary arteries (MINOCA). Among its potential etiologies, Takotsubo syndrome (TTS) and spontaneous coronary artery dissection (SCAD) present with overlapping clinical features, posing diagnostic challenges. We present a case of a 56-year-old postmenopausal woman with recurrent chest pain and elevated troponin levels, initially diagnosed with TTS based on apical ballooning and a lack of obstructive coronary artery disease. However, persistent symp

[Interarterial anomalous right coronary artery in a competitive athlete: case report].

Anomalous origin of the right coronary artery from the left sinus of Valsalva with an interarterial course is a congenital coronary anomaly associated with myocardial ischemia and sudden cardiac death, particularly in athletes exposed to intense hemodynamic stress. In this setting, risk is dynamic and results from the interaction between anatomical features and exercise. We report the case of a 37-year-old competitive endurance athlete in whom anomalous origin of the right coronary artery from the left sinus of Valsalva with an interarterial course and an intramural segment was identified duri

Improved accessibility and long-term effects of clinical networks: A mixed methods analysis of the implementation journey of a mature cancer network in Italy.

BACKGROUND: Clinical networks have emerged as strategic approaches to address complex coordination problems in health and social care; however, real-world evidence on the activities and long-term contributions of mature networks remain limited. OBJECTIVE: This study analyses the implementation of a mature cancer network in Italy, examining changes in access for newly diagnosed cancer patients over a ten-year period and assessing the role of management and policy decisions in shaping the network's development and consolidation. METHODS: A mixed-method approach is used, combining analyses of a

Hoverfly responses to looming stimuli depend on elevation and speed.

An object on immediate collision course generates a rapidly expanding visual stimulus on the retina, which will typically trigger a fast reaction, such as an evasive behaviour. In hoverflies, for example, visual looming stimuli may be generated if the insect is about to collide with a stationary object in the surround, by an approaching predator, or by conspecifics during territorial interactions. Supporting these behavioural responses is a diverse range of looming-sensitive descending neurons that project information from the optic lobes and central brain to the motor control centres in the t

Brainstem vascular syndromes.

The brainstem is a heterogeneous assembly of well-organized nuclei and pathways that play a critical role in motor, sensory, arousal, and autonomic functions. Because of its extensive reciprocal connections with the cortex, basal ganglia, and cerebellum, the brainstem is involved in generating, controlling, and regulating all eye movements. Brainstem vascular supply, which is especially complex in the midbrain, comes entirely from the vertebrobasilar system. Brainstem infarcts are infrequent, and hemorrhages are even less common. The pons is the section most often affected by ischemic or hemor

The factory enters the fray: how mitochondrial protein trafficking shapes the host response to infection.

Beyond textbook functions in homeostatic metabolism, mitochondria are now recognized as central coordinators of cell-intrinsic and cell-extrinsic immune responses to infection. Directed trafficking of proteins and other molecules between mitochondria and the rest of the cell underlies a growing catalog of these activities. Some are pro-host; others are antagonized by viral effectors or co-opted by viruses entirely. How host and viral factors rewire the mitochondrial proteome during infection to shape these outcomes remains incompletely understood. The evolutionary history of this system adds a

Predicting disease progression and mortality in prostate cancer using real-world data-driven time-inhomogeneous Markov models.

Prostate cancer progression varies across patients. Understanding disease trajectories and mortality risk is important for improved healthcare. This study developed a time-inhomogeneous Markov model using territory-wide electronic medical records from Hong Kong to estimate 10-year disease progression. The model was constructed using data from 3,274 patients newly diagnosed in 2010-2012. Their electronic medical records defined baseline covariates (age, Charlson Comorbidity Index [CCI], and prostate-specific antigen [PSA] level) and time-varying health states. Mild cases (age≤65, CCI = 0, PSA≤4

Spatial organization of AQP4 water channel expression in the human brain.

Aquaporin-4 (AQP4) water channels support the glymphatic system, a brain-wide pathway that clears cerebral waste products. Here, we use AQP4 gene expression to reconstruct a whole-brain glymphatic-related topography and link it to vascular physiology, glioma, and vulnerability to neurodegenerative diseases. We find that AQP4 expression is highly organized across the brain, peaking in subcortical, ventral, and periventricular territories, consistent with a clearance axis near cerebrospinal fluid reservoirs and perivascular interfaces. Linking AQP4 expression to vascular organization, AQP4-enric

Stenting the Right, Sparing the Left: Optical Coherence Tomography-Guided Management of Simultaneous Multivessel Coronary Thrombosis.

Simultaneous multivessel coronary thrombosis is uncommon, and its optimal management is undefined, particularly when one culprit is non-occlusive. Optical coherence tomography (OCT) can distinguish plaque rupture from erosion and thereby identify lesions amenable to a stent-sparing, antithrombotic strategy. We report a 34-year-old smoker of Mongolian origin who sustained an inferior myocardial infarction from simultaneous thrombosis of the right coronary artery (RCA, totally occluded) and the proximal left anterior descending artery (LAD, non-occlusive). After emergency thromboaspiration and s

Autoantibodies neutralizing type I interferons underlie a third of cases of Chikungunya virus encephalitis or myelitis.

Chikungunya virus (CHIKV) infection is typically not life-threatening but may, in rare cases, affect the central nervous system (CNS). In three cohorts of confirmed CHIKV cases from Martinique (French overseas territory) and Brazil (patients aged 0-89 years, n = 245), 20 patients had CNS infection (aged 17-87 years). Autoantibodies neutralizing type I interferons (AAN-I-IFN) were found in 35% of patients (4 male and 3 female, aged 17-87 years) with encephalitis (4/15), encephalomyelitis (1/2), or myelitis (2/3), but were absent in 225 patients without CNS infection. They neutralized high conce

Molecular characterization of Echinococcus granulosus sensu lato genotypes circulating in small ruminants in north India.

The etiological agents of cystic echinococcosis comprise a species complex, Echinococcus granulosus sensu lato (s. l.). Despite high endemicity, the disease remains neglected in most parts of the globe, including India. The present study aimed at investigating the genotypes of Echinococcus granulosus s. l. circulating in sheep and goats in two states (Haryana and Himachal Pradesh) and a union territory (Chandigarh) of north India. Out of 1049 slaughtered small ruminants (n = 314, 394, and 341 from Chandigarh, Himachal Pradesh, and Haryana, respectively), 85 (n = 53, 17, and 15 from Chandigarh,

Self-assembled EDIII/NS1 nanoparticle vaccines elicit protective immune responses against Japanese encephalitis virus.

Japanese encephalitis virus (JEV) is a mosquito-borne orthoflavivirus that can cause severe neurological disease in humans and reproductive disorders in swine, posing a persistent threat to public health and the swine industry. Despite being vaccine-preventable, the virus remains endemic in 24 countries and territories throughout Asia and Oceania, underscoring the urgent need for safer and more effective vaccines to counter this ongoing threat. In this study, we developed the nanoparticle vaccines by covalently conjugating 60 copies of EDIII or NS1 protein subunits to the self-assembling mi3 p

Interdisciplinary and Participatory Research for the Elimination of Socially Determined Diseases in the Amazon: A Narrative Review.

This narrative review examines how interdisciplinary and participatory research has advanced the understanding and control of socially determined diseases in the Brazilian Amazon. The integration of social sciences into the Tropical Medicine Foundation Dr. Heitor Vieira Dourado marks a shift from purely biomedical approaches toward those recognizing the cultural, environmental and territorial dimensions of health. Qualitative studies on snakebite envenoming among Indigenous populations revealed that treatment integrates biomedical and spiritual practices, highlighting the need for intercultura

Evolutionary inference reveals global natural histories and predicted pathways of antimicrobial resistance in Klebsiella pneumoniae.

Antimicrobial resistance (AMR) is a substantial and growing global health burden. Understanding, and predicting, its evolution in specific pathogens will help responses across scales from individual patient cases to large-scale policy. Here, we use global data on AMR features, predicted from 47k Klebsiella pneumoniae genomes, with hypercubic transition path sampling to infer the evolutionary pathways by which AMR features in K. pneumoniae (KpAMR) are acquired across 102 countries, territories, and areas. We identify "globally consistent" evolutionary behaviors that hold across countries, and "
