Solifenacin succinate for the treatment of symptoms of overactive bladder.

BACKGROUND: Overactivity of the bladder detrusor muscle can result in urinary urgency, frequency, and incontinence. Antimuscarinic agents are the treatment of choice, as they reduce the contractility of this muscle. Solifenacin succinate (SOL) is a competitive muscarinic-receptor antagonist approved by the US Food and Drug Administration in late 2004 for the treatment of overactive bladder (OAB) with symptoms of urge urinary incontinence, urgency, and urinary frequency. OBJECTIVE: This article reviews the current primary literature concerning the pharmacokinetics, efficacy, and tolerability o…

Single-institution experience in 110 patients with botulinum toxin A injection into bladder or urethra.

OBJECTIVES: To detail, in a review, one institution's 6-year experience using botulinum toxin A (BTX-A) in the bladder and urethra in 110 pa … ications had occurred at last follow-up. Two women with multiple sclerosis and mild baseline stress urinary incontinence reported increased leakage with stress after BTX-A external sphincter injection, and one woman with multiple sclerosis noted new onset stress urinary incontinence after external sphincter injection. However, they all reported significant improvement …

The Role of Urodynamics in Post-Prostatectomy Incontinence.

PURPOSE OF REVIEW: Radical prostatectomy (RP) is the standard surgical treatment for localized prostate cancer. One of the side effects after RP impacting quality of life is urinary incontinence. Because of the impact in quality of life, more patients are seeking treatment for post-prostatectomy incontinence. Urodynamics are an important tool to help understand how prostate surgery changes bladder and urethral function. We reviewed the current literature in order to determine the role of urodynamics in post-prostatectomy incontinence. RECENT FINDINGS: Studies have been done to demonstrate the…

[Devastating course-good outcome! Excellent results thanks to artificial urinary sphincter implantation despite perineal urethrostomy-a case report].

In this case report, we describe, for the first time, the implantation of an AMS-800 sphincter (Boston Scientific Corporation, Marlborough, MA, USA) in a patient with stress incontinence following radical prostatectomy, who was ultimately treated with an perineal urethrostomy due to a complicated course of previous incontinence therapy. The continence status and postoperative quality of life after treatment with the newly implanted sphincter system clearly demonstrate that a boutonnière deformity is not a contraindication for treatment with an artificial urinary sphincter.

The functional anatomy of the female pelvic floor and stress continence control system.

This paper provides an overview of the functional anatomy of the structures responsible for controlling urinary continence under stress. The stress continence control system can be divided into two parts: the system responsible for bladder neck support, and the system responsible for sphincteric closure. Age- and injury-related changes in each of these systems are discussed. Understanding the pathophysiology of incontinence on the anatomical level will help to lead to identification of specific defects, thereby allowing better individualized treatment for the incontinent patient.

Nonsurgical therapy for stress incontinence.

This article discusses the therapies that have been developed for the treatment of stress incontinence due to female pelvic-floor dysfunction. A combination of pelvic muscle exercises, biofeedback, behavioral modification, and electrical stimulation are all treatment options that do not involve surgery. When physiotherapy proves successful, and surgery is avoided, it is necessary for the patient to be put on a maintenance program to avoid relapse. The authors also discuss the link between urinary stress incontinence and women involved in sports.

[Burch laparoscopic procedure for repairing proven stress incontinence--report of 32 cases].

There are more than 200 procedures for repairing stress urinary incontinence. We evaluated the safety and efficiency of the Burch laparoscopic procedure in 32 women with urodynamically proven genuine stress incontinence. Mean operating time was 40 minutes and mean hospitalization time after the procedure was 30 hours. The cure rate was 97%, similar to that reported in other studies (80-95%). The major complications were 2 cases (6.2%) of unintended bladder injury, diagnosed and repaired laparoscopically. Although follow-up has only been for 3-42 months, the high cure rate and safety and advant…

Suprapubic-vaginocutaneous fistula 18 years after a bladder-neck suspension.

BACKGROUND: Several complications are associated with healing after pelvic reconstructive surgery for stress urinary incontinence. These include infection, hemorrhage, erosion, and fistula formation. CASE: A 67-year-old woman presented with simultaneously draining vaginal and suprapubic sinuses. Examination revealed a vagino-abdominal fistula. Surgical excision found an abscess around synthetic material from a previous bladder-neck suspension. CONCLUSION: Unusual fistulation can occur remotely from anti-incontinence surgery, especially when graft materials are used.

Prevention and management of moisture-associated skin damage.

Disruption to the integrity of the skin can reduce patient wellbeing and quality of life. A major cause of skin breakdown is prolonged exposure to moisture, but this is often overlooked. When skin is wet, it becomes more susceptible to damage from friction and shearing forces, and skin flora can penetrate the disrupted barrier, causing further irritation and inflammation. If untreated, moisture-associated skin damage (MASD) can rapidly lead to excoriation and skin breakdown. MASD includes incontinence-associated dermatitis (IAD), which is caused by prolonged skin exposure to urine and stool, p

Incontinence-Associated Dermatitis: Pathogenesis, Contributing Factors, Prevention and Management Options.

Incontinence-associated dermatitis (IAD) is a common type of irritant contact dermatitis, seen in patients with urinary or faecal incontinence. Mechanical factors such as traumata and friction may aggravate the lesions. The fragile skin in elderly patients is more prone to developing IAD. The clinical picture of IAD consists of persistent redness on the one hand and skin loss on the other. Both categories may be associated with clinical signs of major colonisation or infection. It is important to distinguish IAD from pressure ulcers and other dermatoses in the genital region. Due to the lack o

Physiology and management of bladder and bowel continence following spinal cord injury.

Patients with spinal cord injury commonly develop bladder and bowel symptoms that can affect their quality of life. The level of spinal cord injury is an important factor in considering bladder and bowel management strategy options to facilitate re-establishment of some level of elimination control. Bladder management involves choosing a drainage method appropriate to individual capabilities; options include clean intermittent or indwelling catheterization, Credé's method, reflex voiding, and surgical options. Bowel management strategies incorporate schedule, nutrition, stimulation, and surgic

Spinal cord tethering.

Tethering of the spinal cord is an insidious cause of worsening incontinence that requires urgent medical assessment and possibly surgical intervention to prevent any further deterioration. Clinicians monitoring young people and adults with spina bifida need to maintain a high degree of suspicion to ensure the early detection of tethered cord syndrome to help promote and maintainindependent living.

Detecting and monitoring incontinence associated dermatitis: Does impedance spectroscopy have a part to play?

In this review, current understanding of the prevention and treatment of Incontinence Associated Dermatitis (IAD) is discussed. The need for preventative measures which target specific faecal/urinary irritants is highlighted, including the role of urease inhibitors. There is no existing internationally and clinically accepted method to diagnose and categorise the severity of IAD. Diagnosis currently relies on visual inspection; non-invasive techniques to assess skin barrier function could remove subjectiveness, particularly in darker skin tones. Impedance spectroscopy is a non-invasive techniq

Genitourinary applications of sacral neuromodulation.

Sacral neuromodulation provides a new option for the management of voiding dysfunction. For patients with intractable urge-incontinence, interstitial cystitis and non-obstructive urinary retention, this procedure has resulted in significant improvement in urinary frequency, voided volume and pelvic pain. We provide a review of the current literature on sacral neuromodulation and the West Virginia University experience with this procedure.
