An audit of the cervical cancer screening histories of 246 women with carcinoma.

OBJECTIVE: Women with cervical carcinoma and residing in the Calgary Health Region between 1996 and 2001 were audited to characterize factors in the opportunistic cervical cancer screening pathway contributing to screening failures. MATERIALS AND METHODS: The cohort consisted of 246 women. Information on their Pap tests and colposcopic/gynecologic examinations was obtained from the files of Calgary Laboratory Services and their colposcopic/cancer center treatment charts. Screening failure factors were defined, and frequencies were calculated. RESULTS: Screening failure factors were as follow

Screening test results associated with cancer diagnoses in 287 women with cervical squamous cell carcinoma.

CONTEXT: The limitations of newer cervical screening tests are still being studied. OBJECTIVE: To investigate those limitations, we conducted a retrospective review of 287 cases of cervical squamous cell carcinoma (SCC). DESIGN: A search through 5 years of records identified 287 women with cervical SCC diagnoses. Clinical history, histopathology, and liquid-based cytology (ThinPrep) and Hybrid Capture 2 (HC2) high-risk human papillomavirus (HPV) results were documented. Polymerase chain reaction HPV tests were performed on SCC tissues with prior negative HC2 results. RESULTS: Of 287 women,

Human papillomavirus in invasive cervical cancer and cervical intraepithelial neoplasia 2 and 3 in Venezuela: a cross-sectional study.

BACKGROUND: This study investigated the distribution of human papillomavirus (HPV) types in invasive cervical cancer (ICC), cervical intraepithelial neoplasia 2 (CIN2) and cervical intraepithelial neoplasia 3 (CIN3) in Venezuela. METHODS: Paraffin-embedded samples from 329 women from 29 medical centers of the 24 states of Venezuela were analyzed to determine the distribution of HPV types for ICC, CIN2, and CIN3, the prevalence of single and multiple infection, and the association of HPV types with severity of lesion, comparing CIN2 versus CIN3+ (CIN3 and ICC). The samples were analyzed with t

[Precancerous lesions of the cervix. Biomarkers in cytological diagnosis].

Despite the success of the German screening program for cervical cancer a new discussion has started with the aim to improve its quality. The main reason for this new discussion has been the finding of many international studies that the quality of screening programs could be improved by introducing human papillomavirus (HPV) testing. It is well known that the sensitivity of the HPV test is much better than that of a single Papanicolaou (PAP) smear. On the other hand, it is generally accepted that the specificity of the established HPV tests is significantly lower than that of cytology, i.e. t

Cervical dysplasia in pregnancy: a multi-institutional evaluation.

OBJECTIVE: This study was undertaken to identify the prognostic indicators associated with postpartum regression of cervical dysplasia diagnosed in pregnancy. STUDY DESIGN: A retrospective cohort study of pregnant women referred for colposcopy from 2004-2007 at four academic centers. RESULTS: One thousand seventy-nine patients were identified. Colposcopic impression by cervical cytology is detailed later in the text. Of patients who underwent biopsy, results correlated with or were less severe than colposcopic impression in 83% with CIN 1 and 56% with CIN 2/3. Fifty-seven percent had follow-

Comparison of ProEx C with p16INK4a and Ki-67 immunohistochemical staining of cell blocks prepared from residual liquid-based cervicovaginal material: a pilot study.

BACKGROUND: Although liquid-based cervicovaginal cytology has high sensitivity for detecting dysplastic/malignant lesions, many pitfalls exist. Cell blocks can be prepared from residual liquid-based cervicovaginal material and used for immunohistochemistry. The aim of this study was to evaluate a new marker, ProEx C, on cell blocks and its ability to distinguish dysplastic/malignant lesions from morphologically abnormal but benign cells. The results of this study were compared with previously reported results for p16 and Ki-67 on the same material. METHODS: ProEx C is a cocktail of monoclonal

A prospective descriptive study of women attending a colposcopy clinic.

This paper reports the findings of a prospective convenience sample cohort study investigating self-reported purpose of pap smears and worry interference associated with having abnormal pap smear results in women attending a public colposcopy service between 05 April and 19 December 2003. Intrusive thoughts about cancer have been postulated to be associated with dysfunction in women at increased risk of developing breast or ovarian cancer. A study designed questionnaire and the Multidimensional Health Locus of Control was completed by the women whilst waiting to have colposcopy. This study sho

Evidence for practice: oral contraception and risk of cervical cancer.

PURPOSE: The issue of continued oral contraceptive use among women with abnormal Pap smears may be controversial due to reported evidence of an increased risk of cervical cancer among long term oral contraceptive users. This article reviews the evidence concerning oral contraception and cervical cancer risk. The role of Human Papilloma virus (HPV) and proposed mechanisms for the development of cervical cancer are explained. Evidence from World Health Organization (WHO) studies and additional research evidence are discussed. Implications for clinical practice are included. DATA SOURCES: WHO co

Integrating mental health screening and abnormal cancer screening follow-up: an intervention to reach low-income women.

The results of implementing mental health screening within cancer screening and diagnostic programs serving low-income ethnic minority women are reported. Multi-phased screening for anxiety and depression was provided as part of structured health education and intensive case management services to improve abnormal mammogram or Pap test follow-up. Seven hundred fifty-three women were enrolled in the Screening Adherence Follow-up Program. Ten percent (n = 74) met criteria for depressive or anxiety disorder. Women with depressive or anxiety disorders were more likely to have cancer, significant p

Adherence to guidelines for antiretroviral therapy and for preventing opportunistic infections in HIV-infected adults and adolescents in Ryan White-funded facilities in the United States.

To determine adherence by health care providers to guidelines for antiretroviral therapy and for prevention of opportunistic infections (OIs) in adults with HIV infection in federally funded facilities in the United States, we reviewed records of HIV-infected adults (>13 years) in 11 Ryan White Title III facilities in four states for information on eight standard-of-care recommendations during November 1996 through September 1997. Eligibility required a visit to the facility within 6 months before record abstraction and a lowest CD4+ lymphocyte count <500 cells/microl. Reviews were completed f

Cervical and breast cancer screening rates in Sioux Indian women.

With increasing evidence of the effectiveness of mass screening in reducing mortality from both cervical and breast cancer, we began to study the rates of cervical cytology and mammography in 1994 for its impact on increased efforts to gain participation among its target population, the Sioux Indian women. Data were collected from 100 diabetic and 100 randomly selected patients aged 50 to 69 years. Patients with diabetes were selected to better evaluate the effects of patient acceptance of screening tests, since these patients are believed to be more motivated to have preventive care. However,

Performance of HPV Genotyping Combined with p16/Ki-67 in Detection of Cervical Precancer and Cancer Among HPV-Positive Chinese Women.

Women with positive high-risk human papillomavirus (hrHPV) need efficient triage testing to determine colposcopy referrals. Triage strategies of combining p16/Ki-67 with extended HPV genotyping were evaluated in this study. In total, 899 women attending cervical cancer screening program and 858 women referred to colposcopy from five hospitals were recruited. All the participants were tested by HPV assays and p16/Ki-67 dual staining. Colposcopy and biopsy were performed on women with any abnormal results. HPV genotypes were divided into four strata (HPV16/18, HPV31/33/58/52, HPV45/59/56/66, and

Age at last screening and remaining lifetime risk of cervical cancer in older, unvaccinated, HPV-negative women: a modelling study.

BACKGROUND: There is a paucity of empirical evidence to inform the age at which to stop cervical cancer screening. The recommended age to stop screening generally varies between age 50-70 years worldwide. However, cervical cancer incidence and mortality remain high in older women. We used a Markov model of cervical cancer screening to estimate the remaining lifetime risk of cervical cancer at different ages and with different exit screening tests, with the aim of informing recommendations of the age at which to stop cervical cancer screening in developed countries. METHODS: For this modelling
