Comparison of hospital based and home based exercise on quality of life, and neck and shoulder function in patients with spinal accessary nerve injury after head and neck cancer surgery.

OBJECTIVES: The purpose of this study was to compare the effects of hospital-based and home-based exercise programs on quality of life (QOL) and neck and shoulder function of patients who underwent head and neck cancer (HNC) surgery. METHODS: This clinical trial included 40 patients with neck and shoulder dysfunction after HNC. The exercise program included range of motion (ROM) exercises, massage, stretching, and strengthening exercises. Twenty patients who were assigned to the hospital-based exercise group performed physical therapy for 40 min three times a week for four weeks, and the rema

Ca2+/calmodulin-dependent protein kinase II and Dimethyl Sulfoxide affect the sealing frequencies of transected hippocampal neurons.

Traumatic injury often results in axonal severance, initiating obligatory Wallerian degeneration of distal segments, whereas proximal segments often survive. Calcium ion (Ca2+ ) influx at severed proximal axonal ends activates pathways that can induce apoptosis. However, this same Ca2+ -influx also activates multiple parallel pathways that seal the plasmalemma by inducing accumulation and fusion of vesicles at the lesion site that reduce Ca2+ -influx and enhance survival. We examined whether various inhibitors of Ca2+ /calmodulin-dependent protein kinases (CaMKs), and/or dimethyl sulfoxide (DM

Accessory Mental Foramina in Dry Mandibles: An Observational Study Along with Systematic Review and Meta-Analysis.

Background/Objectives: The mental foramen (MF) constitutes a passage for mental nerves and vessels, and it is a crucial anatomical landmark in the body of the mandible. The accessory mental foramen (AMF) is a small, addable foramen proximate to the MF, and it is mainly located posteriorly. The AMF is a rare anatomical variation in human mandibles that must be taken into consideration throughout dental and surgical operations. We aimed to assess the incidence and perform morphological and morphometric analyses of AMFs in the human dry mandibles of the Greek population, in addition to a relevant

Use of Polidocanol Endovenous Microfoam to Improve Hemodynamics and Symptomology in Patients with Challenging Clinical Presentations: A Case Series.

BACKGROUND: Chronic venous insufficiency (CVI) is a widely prevalent condition. Saphenous venous reflux is the most common underlying pathology that leads to CVI. Endovenous thermal ablations (ETA) are the current gold standard. However, some patients present with some unique challenges making ETA less ideal. Nonthermal nontumescent therapies could be considered as alternative therapy in these patients. METHODS: We performed a retrospective review of treatment effects with United States Food and Drug Administration-approved polidocanol endovenous microfoam (PEM; Varithena® 1%) on venous sympt

Candidates for limited lateral neck dissection among patients with metastatic papillary thyroid carcinoma.

BACKGROUND: Papillary thyroid carcinoma (PTC) is associated with an excellent prognosis but frequently spreads to regional lymph nodes. The extent of neck dissection, particularly routine level II or V lymphadenectomy, is still controversial as it may lead to spinal accessory nerve injury and associated postoperative morbidities. We assessed the diagnostic value of preoperative ultrasonography (US) plus computed tomography (CT) for detecting metastatic lymph nodes and for identifying predictors of level II or V metastasis in patients with PTC. METHODS: The results of US and CT were compared w

Preoperative detection and predictors of level V lymph node metastasis in patients with papillary thyroid carcinoma.

BACKGROUND: Papillary thyroid carcinoma (PTC) frequently metastasizes to regional lymph nodes. Metastasis to the posterior neck, level V, is uncommon, and level V lymphadenectomy may lead to spinal accessory nerve injury and associated postoperative morbidities. The aim of this study was to assess the diagnostic efficiency of preoperative ultrasonography and to identify predictors of level V metastasis in patients with PTC. METHODS: This study involved patients with previously untreated PTC that had metastasized to the lateral neck, and who underwent total thyroidectomy with central and later

Assessment of the canalis sinuosus and its branches in Taiwanese individuals using cone beam computed tomography.

BACKGROUND: The popularity of dental implant surgeries, particularly in the maxillary anterior region, is growing, and understanding variations in relevant anatomical structures such as the canalis sinuosus (CS) and its accessory canals (ACs) is critical to avoid complications, including nerve injury and tissue necrosis. This study determined the prevalence, diameter, location, and anatomical variations of the CS and AC in a Taiwanese population by using cone beam computed tomography (CBCT). METHODS: CBCT data were collected from 240 participants (aged 20-80 years, with an equal sex distribut

[The cervical dermatoglyphic character of 229 Chinese patients and its application in incision design for the cervical lymph nodes dissection].

OBJECTIVE: To summarize the cervical dermatoglyphics distribution in 229 participants, and to design dermatoglyphic incision for cervical lymph nodes dissection, including II , III, IV or/and V regions, in accordance with the requirement for cosmetology. METHOD: To keep the patient's head right forward when the number and locations of the dermatoglyphy were collected by macroscopic observation. Upper Cricoid-Region is defined as the region above the cricoids, and Cricoids-Collarbone-Region is referred to the region below the cricoids. The relationship among the distribution of cervical dermat

Arthroscopic debridement of loose bodies from a popliteal cyst.

To our knowledge, arthroscopic removal of loose bodies from a popliteal cyst has never been reported in the medical literature. This article describes our technique for removing loose bodies from a popliteal cyst and verifies that the procedure can be performed safely and effectively.A 52-year-old man had progressively worsening right knee pain and swelling of 3 years' duration that had been treated conservatively for 6 months. On physical examination, there was boggy swelling, a palpable popliteal cyst, crepitus with range of motion, and diffuse discomfort and tenderness about the knee. Magne

Restoration of normal BMP signaling levels and osteogenic differentiation in FOP mesenchymal progenitor cells by mutant allele-specific targeting.

Fibrodysplasia ossificans progressiva (FOP) is a rare autosomal dominant disorder of progressive heterotopic ossification for which there is presently no cure. FOP is caused by a recurrent heterozygous activating mutation (c.617G>A; R206H) of Activin receptor type IA/Activin-like kinase-2 (ACVR1/ALK2), a bone morphogenetic protein (BMP) type I receptor that occurs in all classically affected individuals. The FOP mutation dysregulates BMP signaling and initiates the formation of a disabling second skeleton of heterotopic bone. We generated allele-specific siRNA (ASP-RNAi) duplexes capable of sp

Intraoperative Brief Electrical Stimulation of the Spinal Accessory Nerve (BEST SPIN) for prevention of shoulder dysfunction after oncologic neck dissection: a double-blinded, randomized controlled trial.

BACKGROUND: Shoulder dysfunction is common after neck dissection for head and neck cancer (HNC). Brief electrical stimulation (BES) is a novel technique that has been shown to enhance neuronal regeneration after nerve injury by modulating the brain-derived neurotrophic growth factor (BDNF) pathways. The objective of this study was to evaluate the effect of BES on postoperative shoulder function following oncologic neck dissection. METHODS: Adult participants with a new diagnosis of HNC undergoing Level IIb +/- V neck dissection were recruited. Those in the treatment group received intraoperat

Measurement of the trapezius muscle volume: A new assessment strategy of shoulder dysfunction after neck dissection for the treatment of head and neck cancers.

BACKGROUND: The purpose of this study was to determine the actual degree of shoulder muscle change and its relation to symptoms after neck dissection for head and neck cancers. METHODS: Forty-two patients who underwent unilateral neck dissection were selected. Data obtained from each subject were trapezius muscle volume ratio and a Shoulder Disability Questionnaire (SDQ) score. Patients who had undergone neck dissection with spinal accessory nerve (SAN) preservation were compared with those who had received radical neck dissection. The preservation group was further separated into subgroups b
