Comparison of electroglottographic and acoustic analysis of pitch perturbation.

Pitch perturbation is a measure of the cycle-to-cycle variation in vocal fold vibration. Perturbation can be assessed by means of electroglottographic or acoustic signals. The purpose of this study was to determine if these two analysis techniques are equivalent measures. The Laryngograph, an electroglottograph, and the Visi-Pitch, an acoustic analyzer, were used to measure pitch perturbation in 80 dysphonic subjects. Both instruments use Koike's formula to calculate relative average perturbation. While intra-subject variability appeared erratic, statistical analysis of intersubject data indic

[Evoked acoustic oto-emissions in cochlear deafness].

In order to give a contribution to the genesis of the EOAEs, we have recorded echos in 24 subjects with unilateral sudden deafness (I. group); 20 ears with Menière disease (II. group); 22 ears with progressive sensorineural hearing loss (III. group) and 10 normally hearing young subjects as control group. The results have shown that the EOAEs were present in 100% (I. group), 84.6% (II. group) and 86.3% (III. group) in response to the tone-burst; while the echos were present only in the 57.1% (I. group), 38.4% (II. group) and 45.4% (III. group) in response to the click, although the audiometric

Acoustic neuroma: postoperative vertigo and the mechanisms of compensation.

Ninety-five percent of patients with acoustic neuroma experience some degree of vertigo preoperatively, postoperatively or both. Postoperative vertigo is often a debilitating condition that results when the eighth cranial nerve is severed during surgery to resect a tumor growing around the vestibular portion of this nerve. Thus, surgery for acoustic neuroma affects the membranous labyrinth, which is the functional part of the vestibular apparatus and is primarily concerned with equilibrium. Compensation for impaired equilibrium may be by biofeedback in the vestibular pathways, visual control,

Acoustic patterns of an adolescent with multiple articulation errors.

This is a case study on an adolescent who demonstrated multiple articulation errors. In particular, an acoustic analysis was performed on the same features as those investigated by Kent and Forner (1980) in children with normal articulation and by Glasson (1984) in speech- and language-disordered children. The results of the analysis were compared to the data obtained by Kent and Forner and Glasson. Our subject demonstrated longer phrase durations and more variability than any of the groups with whom he was compared. In addition, he displayed an interaction between syntactic and articulatory p

Stereotactic radiosurgery in the treatment of patients with acoustic tumors.

Stereotactic radiosurgery is performed under local anesthesia, and most patients can be discharged from the hospital within 24 hours of treatment. All patients in our series returned to their preoperative level of employment or function within 5 to 7 days of treatment, and this functional level was maintained over the period of follow-up. "Tumor control" was achieved in 96% of patients during an average follow-up of 1.7 years. Tumor shrinkage occurred in 45% of patients who had at least 1.5 years of follow-up. Useful hearing preservation rates were 50% at 6 months and 30% 1 year after treatmen

Facial nerve outcome in acoustic neuroma surgery.

Patients consider facial paralysis the most concerning sequelae following acoustic tumor resection. Surgical and anesthetic refinements have lowered operative mortality to allow the surgeon to focus on preserving facial nerve function. Tumor size, microsurgical technique, and intraoperative monitoring are the most important factors that define the risk of postoperative facial paralysis. A protocol for uniform surgical reporting is proposed.

Acoustic trauma in extracranial magnetic brain stimulation.

The effects of the magnetic coil acoustic artifact (MCAA) associated with extracranial magnetic field stimulation (EMFS) of the brain were studied in normal hearing rabbits. Spectral and intensity analyses showed that the MCAA is a high intensity transient signal with peak energy between 2 and 5 kHz, and peak amplitudes in the first 100-200 mu sec. At EMFS levels of 50-100% of maximum output (2.0 Tesla), the corresponding MCAA levels were 131-142 dB sound pressure level (peak hold) at the outer ear and amplified by the external meatus to reach 145-157 dB sound pressure level (SPL) at the posit

Facial nerve monitoring in acoustic tumor surgery.

Anatomic and functional preservation of the facial nerve during acoustic tumor surgery remains a primary goal. Intraoperative electromyographic facial nerve monitoring with auditory feedback has enabled the surgeon to more readily achieve this goal. We compared a group of monitored translabyrinthine acoustic tumor removals (N = 89) to a similar unmonitored group (N = 155) in regard to facial nerve function. Function was assessed immediately postoperatively, at time of discharge, and at 1 year postoperatively using the House six-point scale. Results were grouped as satisfactory, intermediate, o

Acoustic neuroma (schwannoma) surgery 1978-1990.

A series of 151 patients with 154 acoustic schwannomas have been operated upon in Manchester Royal Infirmary by a joint Otological and Neurosurgical team, employing either the translabyrinthine or the suboccipital approach. The perioperative mortality rate was 3 per cent. Anatomical preservation of the facial nerve was achieved in 89 per cent of tumour removals and a good to normal functional result in 79 per cent of cases. Attempts at hearing preservation have been unsuccessful, largely because of the small number of patients in the series in whom useful hearing was present preoperatively. Co

[Malignant schwannoma of acoustic nerve: a case report].

During a routine physical examination in 1976, a 54-year-old man was noted to suffer from hearing difficulty and continuing tinnitus of his right ear. He had, however, no further consultations for the next five years, although the symptom persisted and gradually worsened. In May 1981, he experienced complete hearing loss in his right ear. A computed tomography disclosed no abnormalities, and other laboratory tests were unremarkable. In September 1981, the patient began to complain of paresthesia of the right angle of the mouth and tongue, right-sided facial paralysis, and walking difficulty. A

MR of acoustic neuromas--relationship to cranial nerves.

In this report, the relationship of acoustic neuromas to the adjacent cranial nerves is discussed. On T1-weighted images, the trigeminal nerve was detected in all 13 cases. Mild to marked compression of these nerves by the tumors was observed in eight cases. The extent of compression did not always correspond to the clinical symptoms. In four cases with a maximum tumor diameter of 2 cm or less, the 7th and 8th cranial nerves were identified. There was no facial palsy in these patients. Two patients with a tumor diameter of more than 2 cm also had no facial palsy. All patients, including those

Acoustic and physiological reaction times of stutterers and nonstutterers.

This study investigates reaction times in the fluent speech utterances of stutterers and nonstutterers under several experimental conditions. Twenty adult stutterers and twenty matched controls produced utterances of three lengths--one syllable words, polysyllabic words, and sentences--in two conditions of time pressure (high and low) and two conditions of preparation (delayed and immediate responding) in a reaction-time paradigm. Recordings were made of EMG signals from the orbicularis oris inferior and from the extrinsic laryngeal area, as well as electroglottographic and acoustic signals. M

The neuroethology of acoustic startle and escape in flying insects.

The acoustic startle/escape response is a phylogenetically widespread behavioral act, provoked by an intense, unexpected sound. At least six orders of insects have evolved tympanate ears that serve acoustic behavior that ranges from sexual communication to predator detection. Insects that fly at night are vulnerable to predation by insectivorous bats that detect and locate their prey by using biosonar signals. Of the six orders of insects that possess tympanate hearing organs, four contain species that fly at night and, in these, hearing is sensitive to a range of ultrasonic frequencies found

The acoustic stimulation test in the anencephalus: preliminary results.

It is not yet clear whether the acoustic stimulus influences the fetus by auditory or vibrational pathways. The anencephalic fetus is an interesting model for the study of the pathway for receipt of acoustic stimulation because of the near absence of cerebral hemispheres. After the traditional nonstress test (NST), the response to acoustic stimulation was assessed in eight fetal heart rate (FHR) recordings of six anencephalic fetuses of gestational age between 18 and 39 weeks. Although four preterm fetuses demonstrated nonreactive results in the NST, two term fetuses revealed reactive NSTs. Ho

[Acoustic oto-emissions: the ear emits sounds].

Otoacoustic emissions are sounds emitted by the inner ear. Their genesis requires the outer hair cells of the organ of Corti to be intact. Analysis of these sounds provides a fine evaluation of cochlear function. Evoked otoacoustic emissions (in response to a sound) are constant in subjects with normal hearing, whereas they always have pathological characteristics in patients with endocochlear conduction or perceptive deafness. When the acoustic nerve or the central nervous system are affected, otoacoustic emissions are normal for as long as the pathological process has no repercussions on the

Early detection of small bilateral acoustic tumors.

Bilateral acoustic tumors are one of the most difficult problems in acoustic tumor surgery. Total tumor removal with preservation of hearing has seldom been achieved in the past. Recently, hearing has been successfully preserved if the tumors are small when removed. We present the case of a child with a family history of von Recklinghausen's disease associated with bilateral acoustic tumors. A high index of suspicion of bilateral tumors and a thorough work-up resulted in the early diagnosis of small tumors. The tumors were removed with preservation of normal hearing.

Angioma of the cerebellopontine cistern simulating acoustic neurinomas.

We report a cryptic angioma associated with a cisternal hematoma confined to the cerebellopontine cistern occurring in a 29-year-old woman. The angioma was related to the acoustic nerve. The cisternal hematoma appeared to have increased in size gradually with repeated hemorrhages from the angioma. The location of the lesion, in conjunction with the gradual size increase of the cisternal hematoma, resulted in the lesion simulating acoustic neurinomas radiologically as well as neurologically.
