Management of dyspnea in advanced motor neuron diseases.

PURPOSE OF REVIEW: Patients with amyotrophic lateral sclerosis or motor neuron disease (ALS/MND) invariably develop respiratory muscle weakness and most die from pulmonary complications. Little evidence is available that identifies optimal management approaches for caring for the dying patient. This review discusses the state of the art on dyspnea in advanced ALS/MND and its treatment. RECENT FINDINGS: Multiple observational studies have demonstrated that noninvasive positive pressure ventilation is beneficial in ALS/MND. It is a relatively safe intervention in the late stages of disease with

Amyotrophic lateral sclerosis (ALS), the most common adult motor neuron disease, is an acquired disorder that results in loss of function in … . Spirituality and religion are important issues to address, as are end-of-life concerns, including advance directives, hospice, and the dying process. In contrast to the discouraging view that "there is nothing we can do," a broad approach to management, through collaboration with a multidisciplinary team, will permit the ALS physician to make a meaningful differen …

Advance care planning in people with intellectual disabilities, more than 'to resuscitate or not to resuscitate': A qualitative interview study with general practitioners and family caregivers.

BACKGROUND: Advance care planning for people (ACP) with intellectual disabilities requires timely and carefully communication between family caregivers and general practitioners. OBJECTIVES: Despite its recognised importance, little is known about how these stakeholders experience advance care planning and decision-making processes, particularly regarding goals of care and do not resuscitate (DNR) orders, highlighting a critical gap in ensuring person-centred palliative care for this population. METHODS: A qualitative design was used to investigate how Belgian general practitioners and famil

Development of an Advance Care Planning Pilot Module for New York City Community Members.

Advance care planning (ACP) facilitates discussions about future healthcare decisions to ensure alignment with individuals' values, goals, and medical preferences. Despite its benefits, ACP engagement and completion of advance directives (AD) remain limited due to low awareness, misconceptions, and structural barriers. The Tisch Cancer Institute Community Outreach and Engagement Department at Mount Sinai in New York City (NYC) identified this knowledge gap as a key community need. In response, a community-based educational module was created to enhance ACP and AD knowledge. Curriculum developm…

'The case for a pro-active approach to advance care planning in older person's mental health - insights from interdisciplinary mental healthcare professionals.

INTRODUCTION: Advance care planning (ACP) involves nuanced conversations to elicit an individual's beliefs, values, and preferences regarding future care. However, despite recognition of the positive impact, representative population surveys indicate low uptake of ACP, figures replicated internationally. Complex medical issues, including cognitive impairment, can interact with and exacerbate mental health challenges in later life, which can make decision-making more complex. ACP and Advance Healthcare Directives support people during periods of incapacitating mental illness. This paper reporti

Temporomandibular Joint Bioengineering Conference: Working Together Toward Improving Clinical Outcomes.

The sixth temporomandibular joint (TMJ) Bioengineering Conference (TMJBC) was held on June 14-15 2018, in Redondo Beach, California, 12 years after the first TMJBC. Speakers gave 30 presentations and came from the United States, Europe, Asia, and Australia. The goal of the conference has remained to foster a continuing forum for bioengineers, scientists, and surgeons and veterinarians to advance technology related to TMJ disorders. These collective multidisciplinary interactions over the past decade have made large strides in moving the field of TMJ research forward. Over the past 12 years, in

Disruptions to HIV services due to the COVID pandemic in the USA: a state-level stakeholder perspective.

BACKGROUND: The United States envisions a 90% reduction in HIV infections by 2030. However, the COVID-19 pandemic disrupted the HIV continuum and disproportionately affected access to social and health services for people at the highest vulnerability. This study shows how stakeholders in the State of Michigan handled disruptions and their key recommendations. As a case study, this study adds to the literature about preparedness for future pandemics. METHODS: We interviewed 33 statewide Michigan HIV/AIDS Council members-practitioners, researchers, and community representatives, guiding service

TRIAD IX: Can a Patient Testimonial Safely Help Ensure Prehospital Appropriate Critical Versus End-of-Life Care?

OBJECTIVE: The present study sought to assess the clarity of Physician Orders for Life-Sustaining Treatment (POLST) or Living Will (LW) documents alone or in combination with a video message/testimonial (VM). METHODS: Emergency medical services (EMS) personnel responded to survey questions about the meaning of stand-alone POLST and LW documents and those used in conjunction with emergent care scenarios. Personnel were randomized to receive documents only or documents with VM. Questions sought a code status for each scenario and a resuscitation decision. Code status responses were analyzed for

India's Mental Healthcare Act, 2017: Content, context, controversy.

India's new mental health legislation, the Mental Healthcare Act, 2017, was commenced on 29 May 2018 and seeks explicitly to comply with the United Nations Convention on the Rights of Persons with Disabilities. It grants a legally binding right to mental healthcare to over 1.3 billion people, one sixth of the planet's population.
