PURPOSE: The global increase in the prevalence of substance use disorders (SUD) has resulted in higher mortality rates. While heroin use has decreased or stabilized, methamphetamine use is increasing, and the combined use of opioids and methamphetamine is becoming more common. FAAH is an enzyme that hydrolyzes endocannabinoid substances and plays a role in neurobehavioral processes. The aim of this study was to investigate the effect of the FAAH rs324420 polymorphism on SUD.
METHOD: The study included 562 individuals who used methamphetamine (MUD, n = 148), opioids (OUD, n = 114), or both met

Cocaine and amphetamine use is strongly associated with Type B aortic dissection (TBAD). This disproportionately affects younger patients, who are more likely to present with complicated disease and experience greater postoperative morbidity. Stimulant-positive patients are often underrepresented or excluded from prospective trials evaluating optimal medical therapy, thoracic endovascular aortic repair, and perioperative management. Ethical concerns, medicolegal risk, stigma, and barriers to follow-up have further constrained trial participation, resulting in reliance on retrospective data and

BACKGROUND: Contingency management (CM) is the most effective available treatment for stimulant use disorder (StUD). The Department of Veterans Affairs (VA) rolled out the largest national CM implementation effort to date, but ongoing efforts are needed to increase CM receipt. To inform these efforts, it is important to understand the reach of CM implementation across patient characteristics. This exploratory analysis examined whether patient characteristics are associated with CM receipt among VA patients with StUD.
METHODS: We extracted electronic health record data for patients with diagno

Background: Dopaminergic neurochemical markers are associated with working memory performance in healthy participants and patients with schizophrenia. Individuals with stimulant use disorder have below-control levels of dopaminergic markers, such as striatal D2-type receptor (D2/3 R) availability (i.e. D2 + D3 subtypes), and they underperform healthy controls on tests of working memory. Objectives: For consideration in the design of treatments for stimulant use disorder, we tested whether working memory and D2/3 R availability are related in this population. Methods: Eighty-four adults with stim

BACKGROUND: Initiation of buprenorphine for treatment of opioid use disorder (OUD) in acute care settings improves access and outcomes, however patients who use methamphetamine are less likely to link to ongoing treatment. We describe the intervention and design from a pilot randomized controlled trial of an intervention to increase linkage to and retention in outpatient buprenorphine services for patients with OUD and methamphetamine use who initiate buprenorphine in the hospital.
METHODS: The study is a two-arm pilot randomized controlled trial (N = 40) comparing the mHealth Incentivized Ad

BACKGROUND: Methamphetamine (MA) remains one of the most commonly used amphetamine-type stimulants, accounting for the second most widely-used substance after marijuana. Due to increased use of MA, a wide variety of research has focused on the patterns of MA use initiation among adolescents. Nevertheless, there are few data available for people who use MA. The present study set out to assess the sequential patterns of substance use initiation in patients with MA use disorders in Iran.
MATERIALS AND METHODS: This cross-sectional study described substance initiation patterns for 302 patients wh

BACKGROUND: The ongoing drug crisis in the United States continues to be headlined with numbers of deaths related to opioid overdose. Less known to the public and health care providers is the rise in methamphetamine use, often in conjunction with opioids or adulterated with fentanyl. An old practice with a new twist is the use of methamphetamine in conjunction with an opioid such as heroin.
PURPOSE: Although there are no Food and Drug Administration-approved medications to treat individuals with stimulant use disorders, a review of available studies suggests a few promising medications that m

We studied predictors of HIV-related sexual risk-taking among individuals who initiated the use of heroin or methamphetamine during the past 5 years. Many studies have linked drug use to HIV risk and most research has been based on samples of users with long-established drug careers. We conducted face-to-face interviews with 153 adult new drug users in Atlanta, GA. Drug use was not a statistically significant predictor of sexual risk, but gender, age, race, homelessness status, childhood neglect, level of paranoia, and level of (dys)functionality in handling disagreements were associated with

Cognitive impairment has been reported in some chronic users of psychostimulants, raising the possibility that long-term drug exposure might damage brain neuronal systems, including the cholinergic system, which are responsible for normal cognition. We measured the activity of choline acetyltransferase (ChAT), the marker enzyme for cholinergic neurones, in autopsied brain of chronic users of cocaine, methamphetamine, and, for comparison, heroin. As compared with the controls, mean ChAT levels were normal in all cortical and subcortical brain areas examined. However, the two of 12 methamphetami

OBJECTIVE: This manuscript explores the relationship between methamphetamine as the primary drug of choice, symptoms of posttraumatic stress disorder (PTSD), and adverse consequences of alcohol/drug use in a sample of American Indian (AI) people who self-identified as having a current substance use problem.
METHOD: Using a Community-Based Participatory Research framework, we conducted a cross-sectional survey with 198 AI tribal members experiencing addiction. In the analyses, substance use-related adverse consequences in the lifetime and past 30 days served as outcome variables.
RESULTS: Usi

BACKGROUND: Polydrug use may challenge effective treatment for substance use disorders. We evaluate whether secondary substance use modifies the association between treatment and primary drug use among primary heroin, cocaine and methamphetamine (MA) users.
METHODS: Data were obtained from prospective cohort studies on people who use illicit drugs (PWUD) in California, USA. Using repeated monthly data on self-reported secondary substance use (heroin, cocaine, MA, alcohol or marijuana; ≥1day in a month), primary drug use (≥1day in a month), and treatment participation, collected via timeline f

OBJECTIVE: The purpose of this study was to determine the prevalence of substance abuse in an inner city population at delivery admission by universal urine toxicology screening.
STUDY DESIGN: This was a retrospective analysis of universal urine toxicology screening at admission for delivery on the LSU obstetric service at University Hospital in New Orleans.
RESULTS: Four hundred sixty-two women delivered during the first 4 months of 2005. Four hundred and sixteen (90%) had a urine screen performed and 79 (19%) screened positive for 1 or more substances. Rates of a positive test by substance

Neuroimaging studies in stimulant use (eg, cocaine, methamphetamine) disorders show that diminished dopamine release by dopamine-elevating drugs is a potential marker of relapse and suggest that increasing dopamine at the D2/3 receptors may be therapeutically beneficial. In contrast, recent investigations indicate heightened D3 receptor levels in stimulant users prompting the view that D3 antagonism may help prevent relapse. Here we tested whether a 'blunted' response to amphetamine in methamphetamine (MA) users extends to D3-rich brain areas. Fourteen MA users and 15 healthy controls complete
