Sequential combined spinal-epidural anesthesia for cesarean section in a woman with a double-outlet right ventricle.

The number of women with complex cyanotic heart disease reaching childbearing age is continuously increasing. For anesthesiologists, management of this 'new' obstetric population is particularly challenging. We report the case of a parturient with a palliated double-outlet right ventricle, who underwent a cesarean section at 34 weeks with low-dose sequential combined spinal-epidural anesthesia with patient-controlled epidural postoperative analgesia. Anesthetic considerations and specific limitations of invasive monitoring are discussed, along with a review of recent literature on maternal and

[Famotidine does not modify the plasma levels of lidocaine for spinal anesthesia. A comparative study of famotidine and cimetidine].

H2 receptor antagonists are often used to reduce gastric acidity prior to general or regional anaesthesias. However cimetidine and ranitidine have been found to increase plasma levels of local anaesthetics such as lidocaine. This study aimed to compare famotidine--another H2 receptor antagonist--with cimetidine in this respect. 33 male patients 70 years old or more, scheduled for surgery under spinal anaesthesia and randomized into 3 groups, received either diazepam 0.15 mg.kg-1 and famotidine 20 mg (group A) or diazepam and cimetidine 200 mg (group B) or diazepam only (group C) on the previou

Distribution of lidocaine and disopyramide in human blood and tissue, a case report of death caused by spinal anesthesia.

An 11-year-old girl was anesthetized with hyper-baric solution of lidocaine as spinal anesthesia for an appendectomy in a surgical clinic. Respiratory arrest which occurred soon after the injection, was not discovered for a period of time. Since spontaneous respiration recovered within 2 h of intensive resuscitation, the patient was transferred to a community hospital for intensive care. Ten hours after the spinal anesthesia, she died of cardiac failure. The concentration of lidocaine in the brain was 5-10 times more than that in other tissues. The relationship between the possibility of malpr

[Experience with spinal anesthesia using pethidine in the surgery of vesicovaginal fistula in African women (30 cases)].

We report our experience of spinal anesthesia using pethidine at doses of 1 ml/5 kg applied to the surgery of the vesico-vaginal fistulas in the african woman. The sensitive block lasts 80.5 +/- 15.5 min. at its maximal block stage and its recovery lasts 25.6 +/- 12.6 min. The complete block of the motor nerves, obtained in 83.3% of cases, lasts 60.5 +/- 12.4 min. The hemodynamic tolerance has always proved to be excellent. The only noticeable after effect is pruritus (13.3% of cases). This kind of anesthesia of still unknown mode of action (local anesthetic action or/and action on the morphin

Acid-base status in diabetic mothers and their infants following general or spinal anesthesia for cesarean section.

Acid-base status was studied in 30 diabetic mothers and their infants and in 30 healthy mothers and their babies after general or spinal anesthesia for cesarean section. A normal acid-base state was found for the diabetic subjects following general or spinal anesthesia. However, the infants of diabetic mothers given spinal anesthesia had an average pH of 7.20 and a base excess value of -5.67 mEq/l in umbilical-artery blood at delivery. These values were significantly lower than those observed in the infants of the other groups, where the average pH was between 7.28 and 7.30 and the base excess

Effect of Intrathecal Bupivacaine Temperature Variation on Prophylaxis of Shivering in Urologic Patients Undergoing Spinal Anesthesia.

BACKGROUND: Postspinal shivering is a frequent complication of spinal anesthesia (SA) and is particularly relevant in urological procedures, where perioperative heat loss can be exacerbated by irrigation fluids. Modifying the temperature of the intrathecal local anesthetic may affect thermoregulatory responses and the properties of the blockade. METHODS: This prospective, randomized, double-blind, controlled trial was conducted in 96 patients (ASA I-II, aged 18-60 years) undergoing elective urological procedures under SA. Participants received intrathecal hyperbaric bupivacaine 0.5% (15 mg) c

Emergent cesarean delivery under spinal anesthesia for a patient with hemoglobin Beth Israel: a case report.

A 20-year-old primigravida with hemoglobin Beth Israel, a rare low oxygen-affinity hemoglobinopathy, presented for emergency cesarean delivery with cyanosis and low oxygen saturation. Spinal anesthesia with supplemental oxygen maintained a stable intraoperative and postoperative course, with good maternal and neonatal outcomes. Low oxygen-affinity hemoglobinopathies are often overlooked during evaluation of cyanosis. Understanding the pathophysiology of oxygen delivery in this condition is essential for targeted perioperative care, as obstetric patients may have an elevated risk of hypoxia. Ne

Impact of Combined Immersive Virtual Reality and Spinal Anesthesia on Sedative Consumption in Total Knee Arthroplasty.

BACKGROUND: Perioperative anxiety plays a key role in patients, particularly for postoperative recovery. Immersive virtual reality (VR), which has been developed in recent years for use in regional anesthesia, may help reduce this anxiety and limit intraoperative sedative consumption. The aim of this study was to objectively assess the impact of VR distraction on perioperative anxiety by evaluating sedative use during total knee arthroplasty (TKA) under spinal anesthesia (SA). METHODS: A retrospective, single-center cohort study was conducted in patients receiving SA with a VR headset (group

Effectiveness and safety of thoracic segmental spinal anesthesia for breast surgery: A systematic review and meta-analysis.

General anesthesia is the standard approach for thoracic and abdominal procedures; however, it has notable limitations, particularly in high-risk patients. Regional anesthesia techniques, such as thoracic segmental spinal anesthesia, have gained popularity due to their potential to reduce these associated risks. The aim of this study was to assess the effectiveness and safety of thoracic segmental spinal anesthesia in breast cancer surgery using systematic review and meta-analysis. This study adhered to the preferred reporting items for systematic reviews and meta-analyses (PRISMA) 2020 guidel

Systematic review and meta-analysis of spinal versus general anesthesia in decompressive surgeries of the lumbar spine.

INTRODUCTION: Decompressive lumbar spine surgery is a common procedure for disc herniation and spinal stenosis. Besides intervention under general anesthesia (GA), awake surgery (AS) in local or spinal anesthesia offers potential benefits regarding GA-related side effects and simplified periprocedural management. RESEARCH QUESTION: Within this systematic analysis, we sought to compare postsurgical outcomes of spinal decompression surgeries in GA and AS. METHODS: Following the PRISMA guidelines, we extracted all relevant studies from three databases and collected all data concerning surgery d

Emergency Cesarean Section in a Parturient with Ruptured Sinus of Valsalva under Spinal Anesthesia: A Case Report.

Ruptured Sinus of Valsalva (RSOV) is a rarely encountered cardiac anomaly that can potentially lead to adverse clinical outcomes. RSOV increases the risk of morbidity during pregnancy due to the physiological changes associated with gestation, that can exacerbate the underlying cardiac pathology. We present the case of a 29-year-old female with an uncorrected RSOV who required an emergency cesarean section for abruptio placenta. The patient underwent the procedure under spinal anesthesia, with careful titration of norepinephrine infusion and close monitoring of hemodynamic parameters using an

Comparison of Fentanyl With Midazolam As Adjuvants to Levobupivacaine in Spinal Anesthesia for Cesarean Sections: A Randomized Controlled Trial.

Background and objectives Spinal anesthesia stands as a cornerstone for patients undergoing lower segment cesarean section (LSCS), offering advantages like faster onset and high block density. Levobupivacaine, known for its high potency and long-acting nature, has a slower onset. The safety of intrathecal fentanyl or midazolam is evaluated as an adjuvant to levobupivacaine in parturients. This study aims to compare the duration of postoperative analgesia provided by fentanyl or midazolam added to 0.5% hyperbaric levobupivacaine in elective cesarean sections. Secondary objectives include evalua

Spinal anesthesia for open gastrostomy in an infant after stage I Norwood for hypoplastic left heart.

Infants with hypoplastic left heart are at increased risk of adverse events including mortality when they undergo procedures with general anesthesia in the inter-stage period after stage I Norwood. This is primarily caused by an imbalance between pulmonary and systemic blood flows augmented by decreased function of the single ventricle. These factors can be aggravated by general anesthesia, hence the increased risk. Many of these infants experience feeding dysfunction and require a gastrostomy to optimize nutrition. We report a case of open gastrostomy in an infant with Norwood physiology unde
