Long-acting analgesic or long-acting local anesthetic in controlling immediate postoperative pain after lower third molar surgery.

In a double-blind crossover study, the effect on postoperative pain of lidocaine plus diflunisal was compared with that of bupivacaine and placebo. Forty-eight patients with bilateral impactions of lower third molars had these removed on two occasions four weeks apart. One pain-control regimen was used on one occasion and the alternate on the second. The pain intensity was indicated on a visual analog scale at 2, 3, 4, 5, and 6 hours postoperatively. The pain values for the first postoperative hours were higher for the lidocaine diflunisal combination, whereas after 5 hours the opposite was tr

Local anesthetic lidocaine decelerates cytoplasmic streaming in Egeria densa via altered actin dynamics and reactive oxygen species generation.

Local anesthetics (LAs) block voltage-gated Na+ channels in mammals, but they also affect cytoplasmic streaming in plant cells. Although certain LAs inhibit streaming in algae such as Chara, the underlying mechanisms remain unclear. This study aimed to elucidate how LAs reduce cytoplasmic streaming velocity, focusing on the actomyosin system and oxidative stress responses, using lidocaine as a representative LA. Streaming velocity in Egeria densa leaves decreased reversibly after lidocaine treatment and recovered following a washout procedure. Confocal laser scanning microscopy of fluorescentl

Effect of Local Anesthetic-Eluting Foley Catheter (FreeFoley) on Urethral Pain and Discomfort After HoLEP: A Multicenter Prospective Controlled Study.

BACKGROUND AND OBJECTIVE: Benign prostatic hyperplasia (BPH) is common in middle-aged and older men and often requires surgical intervention. Postoperative Foley catheterization frequently causes urethral pain and catheter-related bladder discomfort (CRBD). Systemic analgesics may be insufficient and can be contraindicated in older patients. This study evaluated the efficacy and safety of a local anesthetic-eluting Foley catheter (FreeFoley) for reducing urethral pain and discomfort after holmium laser enucleation of the prostate (HoLEP). METHODS: In this prospective multicenter observational

Local anesthetic overdose during nurse-led epidural analgesia due to occlusion alarm misinterpretation: a case report and literature review.

RATIONALE: Epidural analgesia (EA) medication errors are rare but serious adverse events in anesthesia and pain management. Previous literature has predominantly reported errors involving drug types or routes of administration, whereas those caused by infusion devices are uncommon. This case highlights the potential hazard of dose overdose resulting from an abnormal "occlusion" alarm when using a syringe pump for EA bolus. PATIENT CONCERNS: A 36-year-old male with recurrent hyperlipidemic severe acute pancreatitis, third episode, was admitted to the intensive care unit (ICU). On admission, th

The impact of cannabis use on local anesthetic dosing during hair restoration surgery: a case report, proposed mechanisms, and clinical recommendations.

Cannabis use has increased significantly in the last decade. This article presents a case where a patient needed more local anesthetic (LA) than usual to induce effective anesthesia during hair transplant surgery. The reason cannabis users often need more LA is poorly understood. One possibility is that cannabis withdrawal effect makes patients more sensitive to pain and stress. Additionally, vasodilatory property of cannabis may speed up LA clearance from the application site. The interactions of two major cannabinoids, cannabidiol (CBD) and tetrahydrocannabinol (THC), with cannabinoid recept

A comparative evaluation of peppermint oil and lignocaine spray as topical anesthetic agents prior to local anesthesia in children: a randomized clinical trial.

BACKGROUND: In pediatric dentistry, fear and anxiety are common among children. Local anesthetics (LA) are widely used to control pain and reduce discomfort in children during dental treatment. Topical anesthetics play a vital role in reducing pain and the unpleasant sensation of a needle puncture in children. Peppermint oil has been extensively used for various diseases. However, its anesthetic properties remain unknown. Peppermint oil, used in mouthwashes, toothpastes, and other topical preparations has analgesic, anesthetic, and antiseptic properties. This study aimed to compare and evaluat

Synthesis of Ottonia anisum Extract Mediated ZnO NPs and Their Local Anesthetic, Analgesic and HCl‑induced Acute Lung Injury Activities.

In this study, we outlined the green synthesis of Zinc oxide nanoparticles (ZnO NPs) using the plant-mediated method. Employing the nitrate derivative of Zinc and the extract from the native medicinal plant, Ottonia anisum, the nanoparticles were effectively produced. After obtaining a yellow-colored paste, it was meticulously dried, gathered, and set aside for subsequent examination. The UV-visible spectrometry analysis indicated an absorption peak at 320 nm, which is indicative of ZnO NPs. Characterization techniques, such as XRD and HR-TEM, confirmed the existence of agglomerated ZnO NPs wi

Randomized clinical trial comparing pericapsular nerve group (PENG) block and periarticular local anesthetic infiltration for total hip arthroplasty.

BACKGROUND: This randomized trial compared pericapsular nerve group block and periarticular local anesthetic infiltration in patients undergoing primary total hip arthroplasty. We hypothesized that, compared with pericapsular nerve group block, periarticular local anesthetic infiltration would decrease the postoperative incidence of quadriceps weakness at 3 hours fivefold (ie, from 45% to 9%). METHODS: Sixty patients undergoing primary total hip arthroplasty under spinal anesthesia were randomly allocated to receive a pericapsular nerve group block (n=30) using 20 mL of adrenalized bupivacain

Evaluation of contrast-enhanced ultrasonography with Sonazoid® in visualization of local anesthetic distribution in rectus sheath block: a prospective, clinical study.

PURPOSE: Clear visualization of ultrasound (US) images is crucial for successful US-guided nerve block. However, accurate determination of local anesthetic (LA) distribution from US images remains difficult. Sonazoid®, which comprises perflubutane microbubbles, is used to diagnose hepatic and breast tumors. This study aimed to investigate the visibility of Sonazoid® in perioperative US-guided nerve block. METHODS: We performed rectus sheath block (RSB) in patients scheduled for laparoscopic abdominal surgery (n = 10). 10 mL of a mixture containing equal amounts of 0.75% ropivacaine and iohexo

Local Anesthetic Delivered with a Dual Action Ring and Injection Applicator Reduces the Acute Pain Response of Lambs during Tail Docking.

Docking the tail of lambs is a standard husbandry procedure and is achieved through several techniques including clamps, hot or cold knives and latex rings, the last of which is the most popular. All tail docking methods cause acute pain which can be reduced by application of local anesthetic, however precise anatomical injection for optimal efficacy requires considerable skill. This pen trial evaluated the ability of local anesthetic (LA) delivered with a dual function ring applicator/injector to alleviate acute tail docking pain. Thirty ewe lambs were assigned to one of three treatment group

Variation Between and Within Hospitals in Single Injection Caudal Local Anesthetic Dose: A Report From the Pediatric Regional Anesthesia Network.

BACKGROUND: Given that variation exists in health care utilization, expenditure, and medical practice, there is a paucity of data on variation within the practice of anesthesia. The Pediatric Regional Anesthesia Network (PRAN) data lend itself to explore whether different medical practice patterns exist and if there are nerve blocks with more local anesthetic dosing variation than others. The primary aim of this study was to quantify variation in single injection caudal block dosing, and the secondary aim was to explore possible causes for variation (eg, number of blocks performed versus geogr

Modeled Wide-Awake, Local-Anesthetic, No-Tourniquet Surgical Procedures Do Not Impair Driving Fitness: An Experimental On-Road Noninferiority Study.

BACKGROUND: The use of wide-awake, local-anesthetic, no-tourniquet (WALANT) surgical techniques is increasingly common, and patients commonly ask whether they may drive home following these procedures. The impact of a numb hand and bulky dressing on driving fitness is unknown, and there is no literature to guide surgeons when counseling these patients. Thus, the primary objective of the present study was to determine driving fitness following a modeled-WALANT procedure. METHODS: Twelve right-handed individuals (6 male and 6 female) with an average age of 50 years (range, 38 to 64 years) were

Management of distal adjacent segment disease due to central subsidence of PLIF using local anesthetic transforaminal foraminotomy and lumbar discectomy.

INTRODUCTION: Spinal fusion is one of the most common procedure as the treatment of lumbar pathology, this procedure not only stops the progression of spinal pathology, but also immobilizes the painful motion segment. Furthermore, it also stabilizes the spine after neural decompression. However, the increase of mechanical stress and segmental motion at adjacent segments after spinal fusion has been reported regarding to this biomechanical alteration, various pathologies might occur at the adjacent segments, including acceleration of degenerative changes (Chul et al., 2015). Intermittent bolus versus continuous infusion techniques for local anesthetic delivery in peripheral and truncal nerve analgesia: the current state of evidence.

Manually delivered intermittent bolus (MIB) and programmable intermittent bolus (PIB), alternatives to continuous infusion (CI), involve administering a set volume of solution at a set interval of time. The benefits of intermittent bolus techniques in truncal and peripheral nerve blockade (TPNB) are unclear, and studies have largely demonstrated conflicting results. Using MEDLINE, Embase, Google Scholar, and the Cochrane Library, we conducted an evidenced-based review of published randomized controlled trials comparing intermittent bolus and CI methods in TPNB. In total, 13 randomized controll
