Cardiac digital angiography: applications in ventriculography and coronary arteriography.

Clearly, computer-based digital subtraction offers several advantages over conventional approaches to contrast angiography. Using the newer method, in many situations contrast loads can be beneficially reduced, radiation exposure can be markedly diminished, and right ventricle evaluations can be simplified. In addition, storage of data on magnetic tape is less space consuming, and less costly, than storage on film. Over the 5- to 10-year life expectancy of a digitally equipped laboratory, these advantages probably justify the 30-50% increase in the initial monetary outlay for equipment in exce

Technical considerations for lower limb multidetector computed tomographic angiography.

Multidetector computed tomography (MDCT) enables imaging of the entire arterial tree non-invasively. Optimal technical considerations for performing MDCT angiography (MDCTA) are essential for accurate diagnosis and atherosclerotic disease stratification. This review article focuses on the various technical aspects necessary for peripheral computed tomographic angiography (CTA) acquisition. Common clinical indications for peripheral MDCTA and the latest scan protocols are described. The essential issue of radiation dose reduction is discussed, along with methods of optimal contrast bolus detect

Surgical Anatomy of the Anterior Communicating Artery and Its Implications in Aneurysm Management: A Systematic Review.

The anterior communicating artery (AComA) is a common location for intracranial aneurysms and is known for its complex anatomic variability. Understanding its morphology is essential for accurate diagnosis and safe neurosurgical or endovascular procedures. This systematic review aims to compile current research on the surgical anatomy of the AComA and its relationship with pathologic conditions, mainly aneurysms. A total of 20 studies, including anatomic, imaging, surgical, and case reports, were analyzed. Key anatomic variants, such as hypoplasia or aplasia of the A1 segment, fenestration ano

Radiological approach to brain death.

Brain death is a complex clinical diagnosis with significant medical and legal implications that sometimes requires the use of ancillary tests to confirm the absence of electrical activity or cerebral blood flow (CBF). Among the ancillary tests used to assess CBF are: transcranial Doppler ultrasound, digital subtraction cerebral angiography, CT angiography (CTA), MR angiography, and scintigraphy. This article reviews these techniques, with particular emphasis on CTA, its multiphase protocol, the imaging findings that support the diagnosis of brain death, recommendations for radiology report wr

CT angiography with three-dimensional techniques for the early diagnosis of intracranial aneurysms. Comparison with intra-arterial DSA and the surgical findings.

INTRODUCTION: Cerebral CT angiography (CTA) is an established method applied to both the detection and treatment planning of intracranial aneurysms. The aim of our study was to compare CTA and digital subtraction angiography (DSA) findings with the surgical results mainly in patients with acute SAH and to evaluate the clinical usefulness of CTA. MATERIALS AND METHODS: During the last 2 years, 82 consecutive patients were admitted under clinical symptoms and signs suggestive of harboring an intracranial aneurysm. CT angiography performed immediately afterwards the plain CT, while DSA was perfo

Teaching NeuroImage: Clinical and Imaging Features of Cobb Syndrome.

A 17-year-old boy with typical neurodevelopment presented with 3 months of lower-limb weakness without a history of trauma. Physical examination, plain radiography and a T2-weighted MRI revealed a port-wine stain over the lumbar region, significant scoliosis below the cervical segment, and a multifocal flow void sign (steal phenomenon) from epidural space to dorso-lumbar spine, respectively (Figure 1). CT angiography identified an enhancing dorsal vascular mass with involvement of the spinal canal including destruction of the vertebral body determined to be extensive arteriovenous shunts on di

Case report: thrombectomy for left internal carotid artery occlusion with ipsilateral dual accessory middle cerebral arteries.

Accessory middle cerebral arteries (AMCAs) are found in 0.3-4.0% of cases, and ipsilateral dual AMCAs are rare. A seventy-three-year-old man presented with right hemiplegia and total aphasia. Magnetic resonance angiography showed left carotid artery occlusion. We infused him with tissue plasminogen activator. Digital subtraction angiogram showed revascularization of the left internal carotid artery. However, the left proximal anterior cerebral artery was occluded. We performed mechanical thrombectomy and achieved partial reperfusion. CT angiography on the tenth day showed ipsilateral dual AMCA

Portal vein embolization in the treatment of portal vein bleeding after percutaneous transhepatic biliary drainage: A case report and literature review.

Percutaneous transhepatic biliary drainage (PTBD) is an effective treatment for benign and malignant obstructive jaundice. Major bleeding complications occur in approximately 2-3% of patients after PTBD, which can result in death. A case involving a 63-year-old male with malignant obstructive jaundice, who experienced severe bleeding after PTBD, is reported. Emergency digital subtraction angiography, celiac trunk artery and superior mesenteric artery angiography were performed; however, no signs of arterial bleeding were found. To identify etiology, portal venography was performed under ultras

Carotid Cavernous Sinus Fistula with Contralateral Feed Not Diagnosed by Virtual Assessment or by Non-Invasive Vascular Imaging.

A 67-year-old woman had delayed initial diagnosis of her right low flow carotid cavernous fistula (CCF) during the coronavirus disease (COVID-19) pandemic due to difficulty detecting ocular signs via online virtual examinations. Her right eye conjunctival erythema and proptosis with medial rectus enlargement on computed tomography scan was initially misdiagnosed as euthyroid thyroid-associated orbitopathy without lid retraction. She developed vision loss, and increasing episcleral venous congestion and CCF was suspected. Computed tomographic angiography did not show an obvious fistula. Digital

New hybrid reformations of peripheral CT angiography: do we still need axial images?

PURPOSE: To quantify the detectability of peripheral artery stenosis on hybrid CT angiography (CTA) reformations. METHODS: Hybrid reformations were developed by combining multipath curved planar reformations (mpCPR) and maximum intensity projections (MIP). Fifty peripheral CTAs were evaluated twice: either with MIP, mpCPR and axial images or with hybrid reformations only. Digital subtraction angiography served as gold standard. RESULTS: Using hybrid reformations, two independent readers detected 88.0% and 81.3% of significant stenosis, respectively. However, CTA including axial images detect

Clipping of ipsilateral posterior communicating and superior cerebellar artery aneurysms.

The case is a 55-year-old female who presented with dizziness as the chief complaint. She has a family history of two relatives with subarachnoid hemorrhage. Digital subtraction angiography revealed the presence of a left-sided posterior communicating artery aneurysm and an ipsilateral superior cerebellar artery (SCA) aneurysm. Due to the smaller nature of the SCA, a decision was made to proceed with surgical clipping of both lesions through a pterional approach. A narrated video with illustrations depicts the intraoperative management of these lesions with postoperative angiography results. T

Bleeding scrotal vascular lesions: interventional management with transcatheter embolization.

Vascular lesions of the scrotum are uncommon; the most common among them are varicocele lesions. The other vascular lesions that may involve the scrotum are hemangioma, lymphangioma, and arteriovenous malformations, which are exceedingly rare. The imaging modalities useful in the diagnosis and management of scrotal vascular lesions are grayscale sonography, color Doppler sonography, magnetic resonance imaging, magnetic resonance angiography, and digital subtraction angiography. We present two cases of scrotal vascular lesions involving the extratesticular scrotal soft tissues. Patients present

Hypoplasia of the internal carotid artery: a noninvasive diagnosis.

We present the characteristic imaging findings of hypoplasia of the internal carotid artery (ICA) in two cases, one accompanied by an intracranial aneurysm. Finding of a diffuse luminal narrowing of the ICA on MR angiography or digital subtraction angiography (DSA) could wrongly evoke severe acquired diseases such as dissection or atherosclerosis. Absence of associated wall thickening and flow disturbances on color Doppler sonography (CDS) should suggest carotid hypoplasia. Confirmation of the diagnosis is obtained by CT of the skull showing a small carotid canal. Non-invasive procedures are s

Vertebral artery injury and cerebellar stroke while swimming: case report.

A twenty-five year old woman suffered the acute onset of dysequilibrium followed by headache, nausea, vomiting, vertigo, and slurred speech while swimming. Brain imaging revealed a right cerebellar infarct. Intravenous digital subtraction angiography showed a hypoplastic right vertebral artery and focal narrowing of the dominant left vertebral at the level of the C1-C2 junction. The patient was treated with aspirin and dipyridamole and immobilized for two weeks. She achieved almost complete recovery. Repeat angiography showed resolution of the left vertebral artery defect. Other cases of poste

[Intra-arterial digital angiography of the liver and portal system].

Twenty-seven patients were evaluated by intra-arterial digital subtraction angiography: 20 for portal hypertension--6 also after a porto-systemic shunt--, 4 for evaluation of a hepatic mass and 3 for pancreatic or biliary tumours. Intra-arterial DSA has always shown vascular abnormalities. The advantage of intra-arterial DSA are reduction of the required amount of contrast medium and, consequent, reduction of the frequency of contrast medium reactions. Its disadvantages are limited field size of image intensifier and reduced spatial resolution as compared with conventional angiography. However

Effective treatment with sirolimus in a 6-year-old boy with tracheal vascular malformation initially misdiagnosed as acute laryngitis: a case report.

BACKGROUND: Tracheal vascular malformations are rare congenital lesions that often mimic pediatric acute laryngitis, causing misdiagnosis and delayed care-with greater challenges when caregivers decline angiography and evidence for such scenarios is scarce. Though sirolimus works for vascular malformations, its use in misdiagnosed pediatric tracheal cases is underreported, making this case novel for addressing angiography refusal and showing sirolimus-induced rapid symptom resolution to guide complex pediatric airway care. CASE PRESENTATION: A 6-year-old Han Chinese boy had 6 days of cough, w

The Role of Angiographic Imaging in the Treatment of Spinal Vascular Malformations.

Spinal vascular malformations (SVMs) are rare and heterogeneous lesions that may lead to progressive neurological decline or hemorrhage, posing significant challenges for management due to their complex angioarchitecture and proximity to critical neural structures. This review examines the role of angiographic imaging modalities used intraoperatively and postoperatively in guiding treatment, confirming therapeutic success, and informing follow-up strategies. We summarize evidence on two-dimensional digital subtraction angiography (2D DSA), indocyanine green videoangiography (ICG-VAG), and emer
