BACKGROUND: Surgical antibiotic prophylaxis (SAP) is an essential part of perioperative care, yet adherence to recommended practices varies across surgical categories. Differences in antibiotic choice and duration between clean and clean-contaminated procedures highlight the need to understand local prescribing patterns. This study aimed to describe SAP practices in a tertiary care hospital and assess adherence to institutional SAP guidelines in clean and clean-contaminated surgeries.
METHODS: A prospective observational cross-sectional study was conducted over 6 months in a tertiary care hos…

METHODS: Design: Retrospective cohort study.
SETTING: Single, academic level one trauma center.
PATIENT SELECTION CRITERIA: Included were patients age 18-89 years with GA type III open lower extremity fractures (AO/OTA 32,33,41,42,43,4F3, 44) from 2013 to 2024 with at least 6 weeks follow-up.
OUTCOME MEASUREMENTS AND COMPARISONS: The primary outcome was FRI. Secon…

Surgical antibiotic prophylaxis is an important measure to prevent postoperative surgical site infections. Current guideline recommendations do not treat obesity specifically, although it can affect pharmacokinetics and pharmacodynamics. The objective of this review was to synthesize current evidence on the need for obesity-related dosing adjustments in surgical antibiotic prophylaxis. MEDLINE and Cochrane Library were searched for studies investigating antibiotic prophylaxis dosing in surgical patients with obesity. Outcomes of interest were pharmacokinetic parameters such as plasma and inter…

BACKGROUND: Surgical site infections raise the risk of serious health issues following oral and maxillofacial surgeries. There is full conse … s to curb resistance. Attention to antimicrobial stewardship and strict adherence to guidelines for antibiotic prophylaxis in oral and maxillofacial surgery are essential to reduce antibiotic misuse and improve the quality of study. More studies are required to provide recommendations for maxillofacial surgical interventions in immunocompromised patients.

Patients were classified as receiving appropriate or inappropriate intrapartum antibiotic prophylaxis based on their allergy risk stratification and whether they received standard (e.g. penicillin, cefazolin) or alternative (e.g. clindamycin, vancomycin) antibiotics. BACKGROUND: Periprosthetic joint infection (PJI) and superficial wound or skin infection are serious complications following hip arthroplasty for neck of femur (NOF) fractures. To reduce these risks, perioperative antibiotic prophylaxis is recommended in national guidance, and our trust's local guideline specifies three postoperative intravenous doses of flucloxacillin or teicoplanin in patients with a penicillin allergy. This audit aimed to assess compliance with this protocol and its association with postoperative wound and skin infection rates.
METHODOLOGY: A retrospective audit study was …

BACKGROUND: The majority of ambulatory pediatric penile, inguinal and scrotal cases are considered clean (class I) surgical wounds and guidelines do not recommend surgical antibiotic prophylaxis (SAP) use. Despite these recommendations, few data exist that have assessed utilization and outcomes of interest associated with SAP use for ambulatory pediatric urology procedures.
OBJECTIVE: To better understand national and individual hospital utilization of SAP for common, ambulatory pediatric urologic procedures at lowest risk for surgical site infection (SSI) and for which the benefits of SAP ar…

INTRODUCTION: The aim of this study was to find statistically valid criteria to preoperatively divide acute appendicitis into simple and complicated to enable surgeons to administer the most appropriate antibiotic prophylaxis/therapy before surgery.
MATERIALS AND METHODS: We retrospectively reviewed a cohort of patients who underwent appendectomy from January 2022 to December 2023. Patients included were 0-14 years of age. Exclusion criteria included patients who underwent interval appendectomy or concurrent procedures at the same time of appendectomy. We divided patients into two groups: sim…

Women received standard antibiotic prophylaxis and were randomized to either adjunctive azithromycin (500 mg) or placebo. The primary composite outcome was neonatal death, suspected or confirmed neonatal sepsis, and serious neonatal morbidities (NEC, PVL, IVH, BPD). Secondary outcomes included N …

OBJECTIVE: To evaluate the choice of antibiotic used for intrapartum Group B Streptococcus (GBS) prophylaxis in pregnant individuals with re … g neonates of mothers with a reported penicillin allergy. Administration of alternative intrapartum antibiotic prophylaxis with vancomycin or clindamycin is common in individuals with self-reported penicillin allergy, and maternal alternative antibiotic administration may impact neonatal care, particularly via increased lab draws.

BACKGROUND AND AIM: Primary sclerosing cholangitis (PSC) is characterized by multiple strictures of the biliary tree. Patients with PSC frequently require repeated endoscopic retrograde cholangiopancreatography (ERCP) procedures. These procedures are encumbered by an increased incidence of infectious adverse events such as cholangitis. Evidence regarding whether antibiotic prophylaxis (AP) should be administered is sparse; however, prophylaxis is recommended. We aimed to determine whether AP affects the rate of postprocedural infectious and overall adverse events.
METHODS: We conducted a retr…

This study estimated prophylactic antibiotic usage rates for the prevention of early-onset invasive neonatal group B Streptococcus infection among patients with penicillin allergy. Undertreatment (no antibiotics, underuse of cefazolin, overuse of clindamycin inconsistent with resistance patterns) and overtreatment (vancomycin use) were common. Academic hospitals were marginally more adherent to guidelines than nonacademic hospitals.

BACKGROUND: Although prophylactic antibiotics are considered the standard of care, data with regard to the comparative efficacy of specific antibiotics in the prevention of periprosthetic joint infection (PJI) have remained limited. This study evaluated whether perioperative antibiotic choice affects rates of PJI development in shoulder arthroplasty.
METHODS: From 2000 to 2019, all primary shoulder arthroplasty types (hemiarthroplasty, anatomic total shoulder arthroplasty, reverse shoulder arthroplasty) performed for elective and trauma indications with perioperative antibiotic data and a min

BACKGROUND: In areas where Lyme disease is endemic, bites from ticks are common, but no vaccine is currently available against Lyme disease for humans. Therefore, the feasibility of using antibiotic prophylaxis to prevent Lyme disease after a tick bite is worth further exploration. Previous meta-analyses lack sufficient power to demonstrate the efficacy of about antibiotic prophylaxis for the prevention of Lyme disease following a tick bite. In this study, we explored more precise evidence and attempted to identify and update optimum treatment strategies.

BACKGROUND: Surgical site infections are the third (14%-16%) most frequent cause of nosocomial infections among hospitalized patients. They still form a large health problem and result in increased antibiotic usage, increased associated costs, and prolonged hospitalization and contribute to increased patient morbidity and mortality. Therefore, studies on surgical site infections and surgical antibiotic prophylaxis contribute to identifying surgical site infection rate and risk factor associated with it as well as for identifying the gap in surgical antibiotic prophylaxis practice.

INTRODUCTION: Surgical site infections (SSIs) are a major health issue in surgical specialties in terms of health care costs and patients' clinical outcomes. At the level of the patient, prolonged hospital stays or readmissions for SSIs, can affect the patient's quality of life. At the level of the health care system, it exhausts the hospital's resources and increases the burden on the medical staff due to the need for continuous wound care, microbiological cultures, laboratory tests and medications. In this study, we assessed the effectiveness of two antibiotic prophylaxis regimens for the pr…
