Pulmonary Embolism Presenting as Backache After Appendectomy: An Atypical Presentation of a Common Diagnosis.

Pulmonary embolism (PE) is a disease with diverse clinical manifestations, which lacks specificity but may be life-threatening. We describe a 39-year-old male who presented with acute PE complicated by pulmonary infarction, confirmed by computed tomography pulmonary angiography 2 weeks following laparoscopic appendectomy. His initial symptom was right lower-back pain. This case underscores the diagnostic challenge posed by an atypical presentation of PE, in which isolated back pain was the sole initial manifestation. Furthermore, we delineate the pathophysiologic mechanisms that may account fo…

General Anesthetic Management for Laparoscopic Appendectomy in a Patient With Myotonia Congenita: A Case Report.

A 57-year-old man with myotonia congenita underwent elective laparoscopic appendectomy after perforated appendicitis treated with two drainages. Quadratus lumborum blocks were preoperatively placed bilaterally under ultrasound guidance. Anesthetic gases and succinylcholine were avoided, and total intravenous anesthesia was used for maintenance. Neuromuscular blockade was minimized and reversed with sugammadex. The operative course was uneventful. In the postanesthesia care unit, the patient reported a pain level of zero throughout his stay, required no opioids, and was discharged the same day …

Risk factors for surgical difficulty in interval appendectomy for perforated appendicitis with abscess in children.

PURPOSE: To predict surgical difficulty during interval appendectomy for perforated appendicitis with abscess. METHODS: The records of children diagnosed with appendiceal abscess who underwent interval appendectomy from 2012 to 2024 were reviewed. The clinical data associated with difficult surgeries (operative time > 2 h or addition of trocars) and uncomplicated surgeries were compared. RESULTS: Among the 61 children who underwent interval appendectomy, 42 had uncomplicated surgery, whereas 19 children had difficult surgery. Children who underwent difficult surgery were older (11.5 ± 3 vs. …

A Case of Postoperative Sepsis Triggered by Fecal Retention at the Suture Site After Appendectomy.

Postoperative sepsis after appendectomy can sometimes be led by surgical site infection, intra-abdominal abscess, or intestinal obstruction. However, there have been no reports that postoperative sepsis is certainly caused by only fecal retention in the intestine, including the appendectomy stump. A 60-year-old healthy woman visited a doctor with a chief complaint of right lower abdominal pain. Abdominal computed tomography (CT) showed a swollen appendix with fecal calculus, and then the patient was diagnosed with acute appendicitis. The patient underwent a laparoscopic appendectomy the next d…

Patients with Clostridium difficile infection and prior appendectomy may be prone to worse outcomes.

BACKGROUND: Clostridium difficile infection (CDI) occurs due to a dysbiosis in the colon. The appendix is considered a 'safe house' for gut microbiota and may help repopulate gut flora of patients with CDI. AIM: To study the impact of prior appendectomy on the severity and outcomes of CDI. METHODS: We retrospectively reviewed data of 1580 patients with CDI, admitted to our hospital between 2008 to 2018. Patients were grouped based on the presence or absence of the appendix. The primary aim was to (1) assess all-cause mortality and (2) the severity of CDI. Severity was defined as per the Infe…

An unusual cause of appendectomy in a child (a sharp pin trapped in the appendix): a case report.

Foreign bodies' (FBs) ingestion is a common problem among children and the psychologically deranged. Ingested FBs usually pass through the alimentary tract without any incident. They can rarely be placed in the appendix and may cause problems. Here we report a case of an appendectomy with no signs of inflammation indicated for a sharp metallic foreign body trapped in the appendix of a 13-year-old Syrian girl. The diagnosis was made through serially abdominal X-rays and abdominal ultrasound.

Complications of appendectomy and cholecystectomy in acute care surgery: A systematic review and meta-analysis.

INTRODUCTION: Acute care surgery (ACS) was initiated two decades ago to address timeliness and quality in emergency general surgery. We hypothesized that ACS has improved the management of acute appendicitis and biliary disease. METHODS: A comprehensive systematic review and meta-analysis of outcome studies for emergent appendectomy and cholecystectomy from 1966 to 2017, comparing studies prior to and following ACS implementation, were performed. RESULTS: Of 1,704 studies, 27 were selected for analysis (appendicitis, 16; biliary pathology, 7; both, 4). Following ACS introduction, the complic…

Lack of association between appendectomy and Parkinson's disease: a systematic review and meta-analysis.

BACKGROUND: Accumulation of aggregated α-synuclein from the enteric nervous system is believed to be involved in the pathogenesis of Parkinson's disease (PD). The appendix contains abundant α-synuclein and lacks a blood-tissue barrier, suggesting that appendectomy might reduce α-synuclein aggregation, and therefore the risk of PD. Studies on this intriguing possibility have not come to consistent conclusions. METHODS: PubMed, Embase (via Ovid), and the Cochrane Controlled Register of Trials were searched for studies published through February 20, 2019 on the potential relationship between app…

An Enhanced Recovery Protocol that Facilitates Same-day Discharge for Simple Laparoscopic Appendectomies.

UNLABELLED: Acute appendicitis is the most common gastrointestinal condition requiring urgent operation in the pediatric population with laparoscopic appendectomy (LA) being the current surgical technique. We describe the implementation of a standardized protocol to reduce postoperative nausea and vomiting (PONV) and facilitate same-day discharge after LA. METHODS: A multidisciplinary team developed this protocol to facilitate same-day discharge after observing high rates of overnight stay due to PONV among simple appendectomies performed in 2011-2012. The protocol was implemented in November…

[A low risk of ileus, incisional hernia and impact on female fertility after appendectomy].

Appendicitis is a common condition, which is often seen by the general practitioners as well as in the emergency departments. It is normally treated with laparoscopic appendectomy. This review gives an overview of the long-term surgical complications ileus and incisional hernia and of the impact of appendicitis and appendectomy on female fertility. The literature shows, that there is a low risk of ileus and incisional hernia after appendectomy, and that there is no change in female fertility after appendectomy, not even in cases where the appendix was perforated.

Laparoscopic Appendectomy: A Report on 1164 Operations at a Single-Institution, Safety-Net Hospital.

Laparoscopic appendectomy (LA) is the standard of care for the treatment of acute appendicitis. There is an ongoing debate regarding the optimal management of appendicitis, which led us to study outcomes after an appendectomy at a large safety-net hospital. We hypothesize that despite a high-risk population, LA remains a safe and effective treatment for acute appendicitis. A retrospective review was performed of all patients who underwent an appendectomy from 2011 to 2013. The primary end point was significant morbidity defined as a score of three or greater on the Clavien-Dindo scale of surgi…
