INTRODUCTION: Cutaneous tuberculosis (CT) is rare in industrialized countries. Given the clinicopathological polymorphism and the difficulty of isolating the pathogen, diagnosis can be difficult. The condition may be associated with other known locations of the disease or in rare cases, it may be a tell-tale sign, as in our case, in which leg ulcers revealed paucisymptomatic disseminated tuberculosis.
OBSERVATION: A 67-year-old man was referred for rapidly extensive ulcers of the right leg contiguous to debilitating arthritis of the knee of unknown aetiology for 18 months. Earlier investigati

INTRODUCTION: Approximately 50% of cases of bacterial arthritis occur in childhood. Currently Kingella kingae is considered as an emerging pathogen.
CLINICAL CASE: A ten month old male patient presented joint immobility in right knee. Arthrocentesis was performed.Kingella kingae was identified. He received treatment with ampiciline for 14 days.
DISCUSSION: This pathogen colonizes the posterior pharynx.Isolation in solid culture media may be difficult.
CONCLUSION: It is essential to consider this pathogen when septic arthritis is suspected, and to routinely use blood culture bottles for proc

The acromioclavicular joint is an important component of the shoulder girdle experiencing significant loading during normal activities of daily living. The joint is frequently subjected to trauma and as a synovial articulation can become involved in rheumatoid arthritis and the seronegative arthropathies.

INTRODUCTION: Pyogenic infection of sacroiliac joint (SIJ) is very rare in children. It is the result of haematogenous spread primarily affecting the joint. The process is usually monoarticular. By rule, sequestra are rarely formed, there are no greater bone destructions, but there is usually early bone sclerosing.
CASE OUTLINE: Due to the pain in the lumbosacral area spreading down the right leg and positive Lazarevic's sign, a 13-year-old boy was referred to a neuropaediatrician. He was suffering from lumbosciatica. In the first three sick days, laboratory tests were done as well as X-ray e

A 69-year-old man with liver cirrhosis presented to a surgeon, complaining of pain and swelling involving the right knee joint. A cloudy yellow synovial fluid was aspirated. The examination revealed polymorphonuclear leukocytes and Listeria monocytogenes was grown in the culture. The patient was treated with oral cotrimoxazole. Further follow-up was uneventful. The literature on listerial joint infections is briefly reviewed.

The Authors describe the main features of the most common forms of primary hypogammaglobulinaemia (PH) focusing on the articular involvement. Patients with Bruton's agammaglobulinemia (BA) and common variable immune deficiency (CVID) are predisposed to develop septic arthritis (including arthritis due to atypical microorganisms such as mycoplasma), arthralgia and symmetrical (usually non-erosive) polyarthritis. In BA and CVID complicated by recurrent infections, amyloidosis, which may be itself a cause of arthropathy, can occur. In addition, patients with CVID and selective IgA deficiency show

Based on their own observation the authors describe pneumococcal coxitis in a young man. With regard to the atypical clinical picture of the disease the authors draw attention to the necessity to consider in the differential diagnosis of pain in the inguinal and coxal area also infectious arthritis. This concerns in the first place physicians in the first line of contact.

Two cases of septic sacroiliac (SI) joint arthritis are presented to illustrate the difficulty of diagnosing and treating this uncommon osteoarticular infection. The patients presented are a 68-year-old woman with septic sacroiliitis caused by Streptococcus agalactiae and a 20-year-old man with Salmonella infantis infection involving the SI joint. The recent literature is reviewed and compared with previously published series. Of the 177 cases we reviewed, 47 (27%) occurred in pediatric patients. The mean age was 20 years. Only six patients (3%) were older than 60 years of age. Magnetic resona

Acute infective sacro-iliitis is a rare condition. Though gonococcal arthritis is not uncommon, this organism does not appear to have been isolated from the sacro-iliac joint. The first proven case of gonococcal sacro-iliitis is reported here. Difficulties in correct diagnosis of infective sacro-iliitis are highlighted. Management of gonococcal sacro-iliitis is described. With the increase in number of gonococcal infections, a case can be made for routine culture of joint material N. Gonorrhoeae in cases of septic arthritis in patients at high risk.

Fungi are not a common cause of musculoskeletal infection; however, reports of fungal bone and joint infections have lately surged and are expected to increase more. There is evidence of an increasing incidence of pathogenic and opportunistic fungal infections and emergence of new species of disease-causing fungi, particularly in immunosuppressed patients. Distribution is worldwide, signs of infection can be mild, and chronic evolution as well as delayed diagnosis are common.

The most salient features of nongonococcal bacterial arthritis are reviewed. Factors such as life expectancy, prosthetic joints, arthroscopies, the spread of the AIDS epidemic, and of methicillin-resistant Staphylococcus aureus as modifiers of the course of these arthritides are discussed.
