Post-resuscitative management of the asphyxiated term and preterm infant.

Up until the recent past, the treatment for perinatal asphyxia included only supportive measures. Babies were resuscitated and then observed for signs of multi-organ system dysfunction. Apart from standard supportive management, a new arsenal of potential neuroprotective strategies have emerged over the past years, in order to decrease the severity of brain injury following asphyxia. Today, several neuroprotective therapies are being evaluated in human infants.

The use of hypothermia: a role in the treatment of neonatal asphyxia?

Perinatal asphyxia remains one of the most devastating neurologic processes. Although the understanding of the pathophysiology after perinatal asphyxia is extensive, there are few therapeutic interventions available to prevent or even mitigate the devastating process that unfolds after injury. The search for a safe and efficacious therapy has prompted scientists and clinicians to consider various promising therapies. One such therapy is therapeutic hypothermia. On the basis of adult, pediatric, and animal research, there is increasing evidence to suggest that therapeutic hypothermia may be an

Adverse effects of early phenobarbital administration in term newborns with perinatal asphyxia.

Taking advantage of the lack of consensus in our neonatal unit regarding the early (prior to the onset of signs of hypoxic-ischaemic encephalopathy) administration of phenobarbital to term infants with perinatal asphyxia, we evaluated the possible cerebral sparing effect of phenobarbital by doing a retrospective case-control analysis. Fifty-seven infants who received 10 mg/kg loading dose of phenobarbital within one hour following resuscitation and before the onset of signs of hypoxic-ischaemic encephalopathy were compared with 91 infants who did not to determine the effect of early phenobarbi

Outcome of neonatal stroke in full-term infants without significant birth asphyxia.

UNLABELLED: Seven children with neonatal stroke were identified and six were followed for 13 months to 5 years (mean 28 months). Gestational age ranged between 39 and 42 weeks and none had hypoxic ischaemic encephalopathy. Focal clonic seizures were the presenting feature in six, and one presented with apnoea. The age of the first seizure ranged between 8 and 48 h (mean 26). The total number of seizures ranged between 3 and 10 (mean 5), and all seizures resolved by day 3 in all cases. CT scan showed an ischaemic infarct in six cases, and one child had a haemorrhagic infarct in the right anteri

Selective head cooling in newborn infants after perinatal asphyxia: a safety study.

AIMS: To determine the practicality and safety of head cooling with mild or minimal systemic hypothermia in term neonates with moderate to severe hypoxic-ischemic encephalopathy. METHODS: Study group infants >/=37 weeks' gestation, who had an umbilical artery pH </=7. 09 or Apgars </=6 at 5 minutes, plus evidence of encephalopathy. Infants with major congenital abnormalities were excluded. TRAIL DESIGN: Infants were randomized to either no cooling (controls; rectal temperature = 37.0 +/- 0.2 degreesC, n = 10) or sequentially, either minimal systemic cooling (rectal temperature = 36.3 +/- 0.2

Diagnosis of severe birth asphyxia and early prediction of neonatal neurological outcome in term asphyxiated newborns.

Ten indicators available during the first two hours of life, such as clinical criteria of neonatal distress and postnatal arterial blood gases, were compared with the neonatal neurological course in sixty full term newborns with significant birth asphyxia in order to test their value for the diagnosis and the short-term prognosis of severe birth asphyxia. Birth asphyxia was defined as severe when it was followed by symptoms of moderate or severe post-asphyxial encephalopathy. We calculated a sensitivity lower than fifty percent for clinical criteria such as delay in establishing regular respir

Dyskinetic cerebral palsy and birth asphyxia.

Of 115 children with dyskinetic cerebral palsy (CP) in the Mersey region, 17 were born at term and at an appropriate weight for age, and have preserved cognitive abilities. 10 of these are likely to have sustained intrapartum asphyxial brain-damage as the cause of their CP. In such circumstances, a characteristic pattern is usually seen of severe fetal distress occurring late in labour, severe but short-lived birth asphyxia and only mild or moderate hypoxic-ischaemic encephalopathy. This clinical picture should be distinguished from the pattern of birth asphyxia which precedes the development

Birth trauma among liveborn infants in Mulago Hospital, Uganda.

In this retrospective review, data is presented of birth trauma seen among 4288 live born infants admitted to the special care unit of a large hospital over a three year period. Hypoxaemic brain injury resulting from birth asphyxia was by far the commonest form of birth trauma in this sample and it was the main cause of death, in many infants. All infants diagnosed as having sustained intracranial haemorrhage died while none of the other types of birth injuries were associated with any deaths. The need is emphasised to handle "high risk" deliveries with extra care while at the same time seekin

Case report--a neonate with nonimmune hydrops fetalis.

A post-dated intra-uterine growth retarded male Malay baby was born to a 30-year-old mother gravida II by Caesarean section. Her previous pregnancy ended in still-birth. The baby was severely asphyxiated at birth. He was intubated and immediately admitted to the neonatal intensive care unit. He had anasarca, anaemia, purpura and firm, massive hepatosplenomegaly. X-rays revealed ascites and bilateral metaphysiitis of the long bones. The haemoglobin level was 5.0 gm/dl and PCV 18.3%. Coombs' test was negative. Prothrombin time (PT) and partial thromboplastin time (PTT) were prolonged. The baby a

Dominant use of the left hand by athetotic cerebral palsied children.

Hand preference was studied in 57 children with athetotic cerebral palsy. A left-sided preference was seen in 61% of the subjects. In more severely affected children for whom the possible cause was asphyxia, the left-sided preference was especially common. The perinatal brain damage causing athetosis may affect a motor system controlling movement on the right side more severely.

Failure to prevent meconium aspiration syndrome.

To determine the impact of routine naso-oropharyngeal DeLee and tracheal suction on the prevention of meconium aspiration syndrome, we compared 755 infants with meconium-stained fluid, born during a 12-month period (1983), with a similar group of 742 infants born in a previous year (1975) when these suctioning techniques were not routinely used. Morbidity and mortality data from meconium aspiration syndrome in 103 infants, using combined DeLee and tracheal suction during a five-year period (1979-1983), were also analyzed retrospectively. In spite of a combined DeLee and tracheal approach towar

Low Apgar scores and birth asphyxia. Misconceptions that promote undeserved negligence suits.

The temptation to specify cause-and-effect relationships in cases of neurologic handicap has created a number of misconceptions that have stimulated irresponsible malpractice litigation focusing on childbirth. Paradoxically, the number of such lawsuits is on the increase, even though obstetric and neonatal care has improved dramatically since Virginia Apgar introduced her scoring system in 1953. Nowadays, no evidence of intrapartum hypoxia is found in the majority of newborns who require resuscitation. Consequently, the value of using the Apgar score as an index for assessing asphyxia should b

Hypoxic-ischaemic cerebral injury in the term newborn: correlation of CT findings with neurological outcome.

For asphyxiated term infants, prediction of neurological outcome based on clinical criteria alone is not always reliable. We have used changes in tissue density on CT scan (known to represent hypoxic-ischaemic injury) as an adjunct to clinical examination to assess the severity of cerebral injury. The study population comprised 56 term newborns with hypoxic-ischaemic encephalopathy. Scans were performed with a General Electric 8800 scanner and classified as Normal, Patchy, Diffuse or Global according to the extent of areas of decreased density (DD). These were correlated with neurological outc

Terminal pattern: characteristics and management.

Terminal prepartum patterns from 31 patients were associated with 39% perinatal mortality rate and major degrees of perinatal asphyxia among survivors. Conditions most commonly associated with these patterns were intrauterine growth retardation, preeclampsia, and prolonged pregnancy. NST patterns in which there was absence of accelerations, absent or reduced variability, with or without variable or shallow late decelerations, portended very omnious outcomes when accompanied by absent fetal movements and/or oligohydramnios. Perinatal mortality was 39% under these circumstances. Immediate cesare

Cardiac output in newborn infants with transient myocardial dysfunction.

Decreased cardiac output is a common presumption in left ventricular myocardial dysfunction in neonates, but because of a lack of reliable noninvasive techniques, data on cardiac output are missing. We measured cardiac output by pulsed Doppler echocardiography in 22 newborn infants with left ventricular myocardial dysfunction diagnosed by M-mode echocardiography. Eleven neonates had severe perinatal asphyxia, seven had tachypnea, two hypoglycemia, and one septic shock; one had no symptoms. Right ventricular function was abnormal in 13 of the 22 infants. Hypotension was found in eight; cardiac

Episodic airway obstruction in premature infants.

Seventy-six premature infants with clinical apnea, bradycardia, or cyanosis were studied with polygraph recordings of heart rate, nasal thermistor detection of airflow, and impedance pneumography. Pathologic apnea was defined by cessation of breathing for greater than 20 s or less than 20 s with bradycardia (heart rate, less than 100 beats per minute). Apnea was classified as central, obstructive, or mixed. Four hundred thirty-three apnea episodes were demonstrated: 238 (55.0%) were central, 53 (12.2%) showed obstructive apnea, and 142 (32.8%) were mixed. Fifty-two infants (68.5%) demonstrated
