Intermittent hyperglycemia due to autonomic nervous system dysfunction: a new feature in patients with congenital central hypoventilation syndrome.

OBJECTIVE: To analyze glycemic profile in children with congenital central hypoventilation syndrome, which is characterized by autonomic nervous system dysfunction. STUDY DESIGN: We carried out a university hospital-based observational study. Participants included 14 patients assessed from 2007 to 2009 with a median age of 7.6 (25th-75th percentiles, 1.5-9.6) years at the time of the study. Glucose metabolism was assessed by calculating 24-hour plasma glucose (before and after meals) and fasting insulin concentrations and carrying out an oral glucose tolerance test (OGTT). The main outcome me…

Day-night pattern of autonomic nervous system modulation in patients with heart failure with and without sleep apnea.

INTRODUCTION: Among patients with congestive heart failure (CHF) both obstructive and central sleep apnea (SA) are associated with increased sympathetic activity. However, the day-night pattern of cardiac autonomic nervous system modulation in CHF patients with and without sleep apnea is unknown. MATERIAL AND METHODS: Twenty-five CHF patients underwent polysomnography with simultaneous beat-to-beat blood pressure (Portapres), respiration and electrocardiogram monitoring. Patients were divided according to the presence (SA, n=17) and absence of SA (NoSA, n=8). Power spectral analyses of heart …

Chronic fatigue syndrome: what role does the autonomic nervous system play in the pathophysiology of this complex illness?

Chronic fatigue syndrome (CFS) is a serious health concern affecting over 800000 Americans of all ages, races and socioeconomic groups and both genders. The etiology and pathophysiology of CFS are unknown, yet studies have suggested an involvement of the autonomic nervous system (ANS). A symposium was organized in December 2000 to explore the possibility of an association between ANS dysfunction and CFS, with special emphasis on the interactions between ANS dysfunction and other abnormalities noted in the immune and endocrine systems of individuals with CFS. This paper represents the consensus…

Long-term experience with a contractility (autonomic nervous system) driven pacemaker sensor in patients with chronic chagasic cardiomyopathy.

We analyzed performance of a ventricle-paced, ventricle-sensed, inhibited, rate-responsive (VVIR) pacemaker driven by autonomic nervous system activity sensor in chagasic patients with cardiac conduction system disturbances. Forty-seven chagasic patients were studied (28 male, 19 female; age, 24-68 years). Thirty-six patients had complete atrioventricular (AV) block, eight had second-degree AV block, and the remaining three patients had sinus node disease. Patients were divided into two groups according to their heart rate (HR) at rest: group 1 had an HR more than 65 bpm; group 2, HR less than…

Genetic segregation analysis of autonomic nervous system dysfunction in families of probands with idiopathic congenital central hypoventilation syndrome.

Idiopathic congenital central hypoventilation syndrome (CCHS) is a very rare syndrome with major respiratory complications. Hypothesizing that CCHS is the most severe manifestation of general autonomic nervous system dysfunction (ANSD), we applied a case-control family study design to investigate the genetics of ANSD. Fifty-two probands with CCHS were identified, as well as 52 age-, race-, and gender-matched controls. ANSD phenotypic features were characterized in the cases, controls, and their family members. Our earlier studies found that most ANSD symptoms were more likely in CCHS cases and…

Subclinical neurophysiological effects of lead: A review on peripheral, central, and autonomic nervous system effects in lead workers.

BACKGROUND: The peripheral nerve conduction velocity (NCV), distribution of nerve conduction velocities (DCV), somatosensory, visual, and brainstem auditory evoked potentials (SEP, VEP, and BAEP, respectively), event-related potential (P300), computerized static posturography with spectral analysis (postural balance), and electrocardiographic R-R interval variability (CV(RR)) with spectral analysis appear to be promising techniques for assessing subclinical effects of lead on the peripheral, central and autonomic nervous systems. This article presents an overview of research addressing subclin…

Contribution of the autonomic nervous system to blood pressure and heart rate variability changes in early experimental hyperthyroidism.

A great deal of uncertainty persists regarding the exact nature of the interaction between autonomic nervous system activity and thyroid hormones in the control of heart rate and blood pressure. We now report on thyrotoxicosis produced by daily intraperitoneal (i.p.) injection of L-thyroxine (0.5 mg/kg body wt. in 1 ml of 5 mM NaOH for 5 days). Control rats received i.p. daily injections of the thyroxine solvent. In order to estimate the degree of autonomic activation in hyperthyroidism, specific blockers were administered intravenously: atropine (0.5 mg/kg), prazosin (1 mg/kg), atenolol (1 mg…

Chronotropic cardiac effects of falipamil in conscious dogs: interactions with the autonomic nervous system and various ionic conductances.

The chronotropic cardiac effects of falipamil were studied in conscious dogs with chronic atrioventricular (AV) block. Falipamil (0.5-2 mg/kg) initially increased atrial rate dose dependently. After atropine and atropine-pindolol, falipamil (2 mg/kg) decreased atrial rate, but after pindolol, it did not modify atrial rate. After atropine-pindolol-phenoxybenzamine, atropine-pindolol-yohimbine, atropine-pindolol-verapamil, and atropine-pindolol-quinidine pretreatment, falipamil produced atrial bradycardia. Falipamil dose-relatedly decreased ventricular rate. Falipamil (2 mg/kg) decreased ventric

[Effects of insulin-induced hypoglycemia on catecholamine secretion and blood pressure in neurological disorders affecting autonomic nervous system].

The effects of insulin-induced hypoglycemia on catecholamine secretion were investigated in patients with various neurological disorders affecting the autonomic nervous system. In control subjects, insulin-induced hypoglycemia resulted in marked increases in plasma epinephrine and norepinephrine levels. Heart rates were increased within 15 minutes after the insulin injection which were associated with slight elevation and depression of systolic and diastolic blood pressure, respectively. In patients with upper level spinal cord lesions (C1-T6) of various etiology, Shy-Drager syndrome and famil…

[Electron microscopy of human nasal mucosa in disturbances of the autonomic nervous system. I. Sympathetic denervation (author's transl)].

The fine structure of the human nasal mucosa is described shortly after unilateral stellate gang-lion blockage as well as in a case of sympathetic denervation of four weeks duration. For the clinical picture, corresponding to vasomotor rhinitis, there is a characteristic fine structural correlation consisting of dilatation and increased permeability of the vessels as well as stimulation of the nasal glands. In the glands, both increased activity and degenerative changes suggesting "secretory exhaustion" are observed. The findings differ from those seen in acute and chornic allergic rhinitis, b

[Neuropathological background of spinocerebellar degeneration--with special reference to autonomic nervous system lesions in olivopontocerebellar atrophy, Shy-Drager syndrome and multiple system atrophy].

For elucidation of actual status of olivopontocerebellar atrophy (OPCA) in nosological relation to Shy-Drager syndrome (SDS) and multiple system atrophy (MSA), neuropathological examination with morphometric survey on the intermediolateral column of the spinal cord was performed. The materials were 13 cases registered in our laboratory. Clinically, they consisted of 9 sporadic OPCA including 2 cases with prominent autonomic failure, and 4 hereditary spinocerebellar degeneration. In the autonomic nervous system of CNS, degeneration of the intermediolateral column was found in the sporadic cases…

Electrodiagnostic assessment of the autonomic nervous system: A consensus statement endorsed by the American Autonomic Society, American Academy of Neurology, and the International Federation of Clinical Neurophysiology.

Evaluation of disorders of the autonomic nervous system is both an art and a science, calling upon the physician's most astute clinical skills as well as knowledge of autonomic neurology and physiology. Over the last three decades, the development of noninvasive clinical tests that assess the function of autonomic nerves, the validation and standardization of these tests, and the growth of a large body of literature characterizing test results in patients with autonomic disorders have equipped clinical practice further with a valuable set of objective tools to assist diagnosis and prognosis. T…

Modulation of neurofluid fluctuation frequency by baseline carbon dioxide in awake humans: the role of the autonomic nervous system.

INTRODUCTION: Cerebrospinal fluid (CSF) pulsations are linked to hemodynamics, with autonomic mechanisms, suggested to modulate slow-wave induced pulsations. METHOD: To explore autonomic regulation's role in neurofluid flow, independent of sleep and neural activity, we hypothesized that modulating basal CO2 (altering vascular tone, cardiac activity and respiration) would highlight this link. RESULTS: Using resting-state BOLD fMRI in neurofluid regions under different CO2 levels (capnic states), we found: 1) biomechanical modulation does not explain neurofluid dynamic variations across capnia

Modeling Latent Dynamics of the Autonomic Nervous System in Response to Trauma Recall and Non-Invasive Vagus Nerve Stimulation.

OBJECTIVE: To develop a person-specific dynamic modeling approach to quantify the time course of multiple peripheral markers of the autonomic nervous system (ANS) in response to acute stressors and potential stress-reducing interventions. METHODS: We curated data (N = 50 participants) from a double-blind, randomized, sham-controlled trial of non-invasive vagus nerve stimulation (nVNS) for posttraumatic stress disorder (PTSD). For each participant, a multi-input, multi-output, linear state space model (SSM) was trained on $\sim$3500 s of cardiovascular and respiratory marker time series data a…

Modulating autonomic nervous system activity with transcutaneous auricular vagus nerve stimulation in Parkinson's disease: a proof of concept study.

BACKGROUND: Cardiovascular dysautonomia is a debilitating non-motor symptom of Parkinson's disease (PD) that limits exercise capacity and neurorehabilitation outcomes. Transcutaneous auricular vagus nerve stimulation (taVNS) is an emerging non-invasive neuromodulatory therapy that modulates cardiovascular activity and could potentially serve as an adjunct to exercise, yet its physiological effects on cardiovascular function in PD remains unexplored. OBJECTIVE: This proof-of-concept, sham-controlled crossover pilot study (N = 8) investigated the acute effects of taVNS on cardiovascular autonom

Paroxysmal sympathetic hyperactivity risk modeling based on transients in time series describing the autonomic nervous system and cerebral hemodynamics.

PURPOSE: Overstimulation of the autonomic nervous system (ANS) in the acute phase after traumatic brain injury (TBI) may lead to paroxysmal sympathetic hyperactivity (PSH) syndrome. This study aimed to investigate the impact of the relationship between ANS activity and cerebral hemodynamics on the development of PSH syndrome. MATERIALS AND METHODS: This retrospective study included 41 TBI patients admitted to Wroclaw University Hospital (Poland). Among them, 14 were classified as at risk for PSH based on the probabilistic Paroxysmal Sympathetic Hyperactivity Assessment Measure (PSH-AM), with …

Atrioventricular nodal reentrant tachycardia onset, sustainability, and spontaneous termination in rabbit atrioventricular node model with autonomic nervous system control.

Atrioventricular nodal reentrant tachycardia (AVNRT) is one of the most common types of paroxysmal supraventricular tachycardia. The activity of the autonomic nervous system (ANS) is known to influence episodes of AVNRT, yet the precise mechanisms underlying this effect remain incompletely understood. In this study, we update our compact multifunctional model of the rabbit atrioventricular (AV) node with ANS control to simulate AVNRT. The refractoriness of the model cells is adjusted by a specific ANS coefficient, which impacts the effective refractory periods, conduction delays, and intrinsic…

Electrocardiographic and Autonomic Nervous System Changes after Changes in the Posture of Children and Adolescents with Duchenne Muscular Dystrophy.

BACKGROUND: Duchenne Muscular Dystrophy (DMD) is a rare inherited neuromuscular disease. At first, cardiac involvement may be asymptomatic. Therefore, assessing patients using non-invasive methods can help detect any changes. OBJECTIVES: Analyze the electrocardiogram (ECG) test and heart rate variability (HRV) of the DMD group and compare the information with that of the age-matched control group. METHODS: A prospective study with 27 male patients with DMD (11.9 years old), who underwent clinical evaluation, ECG, echocardiogram, and Holter monitoring. ECG (200% increase) was assessed by two
