Lyme disease in North America is caused by infection with the spirochetal bacterium Borrelia burgdorferi and transmitted by Ixodes scapularis and Ixodes pacificus ticks. These ticks also have the potential to transmit a rapidly expanding list of other pathogenic bacteria, viruses, and parasites, including Anaplasma phagocytophilum, Babesia microti, deer tick (Powassan) virus, Borrelia miyamotoi, and the Ehrlichia muris-like organism. Coinfections with B burgdorferi and these other agents are often difficult to diagnose and may go untreated, and thus contribute significantly to patient morbidit…

BACKGROUND: To reduce the risk of transfusion-transmitted babesiosis (TTB), the Food and Drug Administration (FDA) released guidance in 2019 requiring Babesia screening of all donations collected in 14 states on the East Coast and Upper Midwest (Connecticut, Delaware, Massachusetts, Maryland, Maine, New Hampshire, New Jersey, New York, Pennsylvania, Rhode Island, Virginia, and Vermont Minnesota and Wisconsin), plus Washington, DC. The American Red Cross (ARC) implemented Babesia blood donation screening in those areas in May 2020. Screening uses a transcription-mediated amplification (TMA) nuc

In Europe, Ixodes ricinus ticks transmit pathogens such as Borrelia burgdorferi sensu lato and tick-borne encephalitis virus (TBEV). In addition, there is evidence for transmission to humans from I. ricinus of Anaplasma phagocytophilum, Babesia divergens, Babesia microti, Babesia venatorum, Borrelia miyamotoi, Neoehrlichia mikurensis, Rickettsia helvetica and Rickettsia monacensis. However, whether infection with these potential tick-borne pathogens results in human disease has not been fully demonstrated for all of these tick-borne microorganisms. To evaluate the available evidence for a caus…

BACKGROUND: A rhesus macaque developed chronic anemia, lymphocytic leukocytopenia, fever, and anorexia while immunodeficient following inoculation with a simian-human immunodeficiency virus.
METHODS: A complete blood count, peripheral blood smear, polymerase chain reaction and gene sequence were performed.
RESULTS: Blood smears demonstrated persistent intraerythrocytic piroplasms with rare Maltese cross forms. Babesia microti-like protozoa were confirmed by polymerase chain reaction and sequencing of the 18S ribosomal RNA gene.
CONCLUSION: With continued use of non-human primates as models …

Fifty years ago, the index case of human babesiosis due to Babesia microti was diagnosed in a summer resident of Nantucket Island. Human babesiosis, once called "Nantucket fever" due to its seeming restriction to Nantucket and the terminal moraine islands of southern New England, has emerged across the northeastern United States to commonly infect people wherever Lyme disease is endemic. We review the history of babesiosis on Nantucket, analyze its epidemiology and ecology there, provide summaries of the first case histories, and comment on its future public health burden.

Mice which had been pre-treated with killed Corynebacterium parvum given intravenously or intraperitoneally, but not subcutaneously, were completely resistant to infection with Babesia microti or B. rodhaini, and were protected from death caused by Plasmodium vinckei or P. chabaudi infection. There is evidence that the parasites died within circulating erythrocytes. This occurred much too soon for a specific antibody response to be evoked, and no antibody could be detected by the indirect fluorescent-antibody technique. Thus it is suggested that a non-specific soluble mediator may play an impo…

Human babesiosis is an emerging infectious disease caused by a bloodborne single-celled parasite belonging to the genus Babesia. Cases of human babesiosis are commonly reported in the United States, Western Europe and Asia. In the United States, the two major causative agents are Babesia microti and Babesia duncani. Transmitted to humans through tick bites, the parasite infects host red blood cells (RBCs). It induces flu-like symptoms and has evolved mechanisms to manipulate the immune system, enabling its persistence. One key mechanism is the secretion of extracellular vesicles (EVs) which ca…

Nerolidol is a sesquiterpene present in the essential oils of many plants, approved by the U.S. FDA as a food flavoring agent. Nerolidol interferes with the isoprenoid biosynthetic pathway in the apicoplast of P. falciparum. In the present study, the in vitro growth of four Babesia species was significantly (P<0.05) inhibited in the presence of nerolidol (IC(50)s values=21+/-1, 29.6+/-3, 26.9+/-2, and 23.1+/-1microM for B. bovis, B. bigemina, B. ovata, and B. caballi, respectively). Parasites from treated cultures failed to grow in the subsequent viability test at a concentration of 50microM.

Babesiosis is a tick-borne illness caused by the intraerythrocytic parasite Babesia microti. Adult respiratory distress syndrome (ARDS) is a complication of B. microti infection and generally presents later in the course of the disease. We present a case of babesiosis presenting with ARDS. A 59-year-old male with history of hypertension and atrial fibrillation presented with one day of progressive shortness of breath. The patient returned from a trip to Massachusetts one day prior. On arrival to the emergency department (ED) the patient was noted to be febrile with tachycardia, tachypnea, and …

Babesiosis, a tick-borne disease caused by protozoa of the genus Babesia, is endemic in North America, but rarely reported in Europe. We report a case involving a splenectomised 72-year-old man from New Hampshire, USA, who was admitted at Careggi University Hospital (Florence, Italy), complaining of a febrile syndrome appeared during a trip to central Italy. After the laboratory flagged the presence of intra-erythrocytic ring-shaped elements, malaria was initially suspected but subsequently excluded by molecular testing. Based on epidemiological and clinical history, Babesia microti infection …

Babesiosis, an infection caused by the protozoan Babesia microti and transmitted by the Ixodes scapularis tick, is commonly described in the literature with an approximate incidence of 1000 cases per year (Herwaldt et al., 2012). Infections in North America occur most frequently during the spring and summer months in the Northeastern and Midwestern United States. Babesia can cause a wide spectrum of clinical manifestations ranging from a self-limited febrile illness or mild anemia to severe illness causing acute respiratory distress syndrome (ARDS), disseminated intravascular coagulopathy (DIC…

Lyme disease, human granulocytic ehrlichiosis (HGE), and babesiosis are tickborne infections that are indigenous to Wisconsin. To assess their importance as a cause of nonspecific fever, we recruited patients with febrile illness at 10 clinics in northwestern Wisconsin from May through August of both 1997 and 1998. Eligible patients had a temperature >38.0 degrees C but no rash or other localizing source. Acute and convalescent serological tests were performed for Borrelia burgdorferi, Babesia microti, and Ehrlichia equi; polymerase chain reaction was performed to detect granulocytic Ehrlichia…

Most cases of babesiosis reported in the United States have been tickborne and caused by Babesia microti, the etiologic agent of all previously described transfusion-transmitted cases. A 76-year-old man with the first recognized case of transfusion-transmitted infection with the recently identified WA1-type Babesia parasite is described. The subject received multiple blood transfusions in 1994. Indirect immunofluorescent antibody testing of serum from 57 blood donors implicated a 34-year-old man (WA1 titer, 1:65,536) whose donation had been used for packed red cells. Isolates of the organisms …

Fewer than 200 confirmed cases of babesiosis have been reported in the last decade. Most cases in the United States have occurred on Cape Cod, Nantucket Island, and Long Island. Babesia microti, a malaria-like protozoan that parasitizes erythrocytes, is responsible for this illness in the United States. Infection is often subclinical but may be fulminant, especially in infants, the elderly, and in asplenic patients. Symptoms are nonspecific, usually consisting of fever and myalgias. Common laboratory abnormalities include evidence of hemolysis and thrombocytopenia. We report a case of babesios…
