Unexpected Twists: Intussusception as a Long Term Sequalae of Bariatric Surgery.

Bowel intussusception is rare in adults and accounts for less than 5 % of all cases, occurs in only 1 % of patients with bowel obstruction and in 0.64 % of patients following Roux-en-y bypass surgery with a female predominance. Our case underscores the unusual size and configuration of intussusception, wherein the distal segment telescopes into the proximal segment, defying the conventional presentation. Notably, the rarity of observing intussusception following Roux-en-Y gastric bypass surgery is also highlighted. It is important to be clinically vigilant and keep long term sequalae of bariat

Recovery After Bariatric Surgery: The effects of Mobilization-A Study Protocol.

BACKGROUND: Patients recovering from bariatric surgery report the highest average pain during hospital admission, compared to patients recovering from other abdominal surgeries. Studies have shown many benefits from early mobilization after surgery, however, the effect of early mobilization after bariatric surgery on pain and physical function is unknown. This study aims to investigate the effects of an early initiated mobilization at the post anesthesia care unit and continued for the first 4 postoperative weeks. METHODS/DESIGN: This randomized controlled trial with a nested interview study

Managing post-bariatric hypoglycemia: a systematic review of pharmacological therapies.

BACKGROUND: Post-bariatric hypoglycemia (PBH) is a challenging and increasingly recognized complication following bariatric surgery, particularly Roux-en-Y gastric bypass (RYGB). Multiple pharmacologic interventions have been explored for managing PBH, but evidence is scattered. This study aimed to systematically synthesize the evidence from comparative studies, single-intervention trials, case reports, and registered clinical trials to evaluate the efficacy of drug therapies for PBH. METHODS: This systematic review and mixed treatment comparison meta-analysis was conducted in accordance with

Bariatric surgery and the risk for osteoporotic fractures - a population-based study.

INTRODUCTION: Bariatric surgery (BS) is an effective procedure for sustained weight reduction. However, it may increase fracture risk. We aim to investigate the risk posed by BS for proximal femur and vertebral osteoporotic fractures. MATERIALS AND METHODS: A retrospective cohort study of 17,971 patients, aged 40 (SD12.7), who underwent BS was conducted. The incidence of femoral and vertebral fractures, in the six-year prior- and-post the BS was assessed. The effects of the BMI change, the type of BS performed and the treatment with osteoporosis medications or nutritional supplements was eval

Bariatric surgery and distal radius fractures, a population-based study.

INTRODUCTION: Obesity is a growing global concern. Bariatric surgery (BS) is the only intervention which leads to significant and long-lasting weight reduction. However, it has been associated with an increased risk of fracture. We aimed to investigate the association between BS and distal radius fractures (DRF). METHODS: A retrospective cohort study of patients with obesity who underwent BS was conducted. The incidence of DRF and the non-osteoporotic scaphoid fracture in the six-years pre-and post-BS was compared. Sub-analysis for surgical type, weight loss, nutritional supplements, and trea

The Relationship Between Obesity, Bariatric Surgery, and Infertility: A Systematic Review.

BACKGROUND: Obesity is a complicated, chronic condition that has a major impact on reproductive health, leading to infertility, anovulation, and poor pregnancy outcomes. It alters the hypothalamic-pituitary-ovarian (HPO) axis, promotes insulin resistance, and causes persistent low-grade inflammation, all of which result in hormonal abnormalities that compromise normal ovarian function. Because standard weight loss procedures frequently fail to provide significant and long-term reproductive benefits, bariatric surgery is becoming increasingly popular as a therapeutic option for obese women tryi

Bariatric surgery is associated with lower incidence of carpal tunnel syndrome.

BACKGROUND: Obesity is related with increased risk for carpal tunnel syndrome (CTS). The effect of bariatric surgery (BS) on the incidence of nerve entrapments is undetermined. We aimed to evaluate the incidence of CTS following BS in a large cohort study. METHODS: A retrospective cohort study of patients with obesity who underwent BS was conducted. The incidence of CTS in the 5-years pre- and post BS was compared. Patients who developed CTS following BS were compared to those who did not. RESULTS: Six-thousand, one-hundred and twenty-four patients met the inclusion criteria. The average age

The feasibility and outcomes of metabolic and bariatric surgery prior to neoplastic therapy.

BACKGROUND: Metabolic and bariatric surgery (MBS) is a potent intervention for addressing obesity-related medical conditions and achieving sustainable weight loss. Beyond its conventional role, MBS has demonstrated potential to serve as a transitional step for patients requiring various interventions. However, the implications of MBS in the context of neoplasia remain understudied. OBJECTIVES: To explore the feasibility of MBS as a possible attempt to reduce surgical and treatment risks in patients with benign tumors or low-grade cancers. SETTING: Multicenter review from twelve tertiary refe

Approach to duodenal pathology in bariatric surgery: Transit bipartition as an option.

Transit bipartition (TB) emerges as a bariatric surgery alternative to Duodenal Switch (DS), emphasizing complete intestinal access and reduced malabsorption. By directly stimulating the distal ileum, TB promotes weight loss while preserving endoscopic duodenal access. This technique enhances anorexigenic hormones, aiding in satiety and weight loss, with lower malnutrition risks than DS. TB shows favorable short-term outcomes in weight management and comorbidity reduction, serving as a simpler, less malabsorptive option than DS. Nonetheless, long-term studies are essential for comprehensive ef

Muscle Strength of Lower Limbs as a Postoperative Predictor in Bariatric Surgery.

OBJECTIVE: The aim of our study was to assess postoperative lower limbs muscle strength (MS) as a predictor of late surgical success (36 months). METHODS: Body composition analyses and isokinetic dynamometry evaluation were performed before (T0: n=123), six months (T1: n=123) and 36 months (T2: n=79) after Roux-en-y gastric bypass (RYGB). Surgical success (SS) was defined as ≥ 50% excess weight loss (EWL) 36 months after surgery or ≤ 50% surgical failure (SF). RESULTS: There was no difference between relative MS extension (Ext) and flexion (Flex) in T1 and T2. There was also, no difference b

Iron homeostasis in obesity and metabolic and bariatric surgery: a narrative review.

Iron deficiency has been recognized as a potentially modifiable nutritional complication of metabolic and bariatric surgery (MBS) since prior to the turn of the century. Despite this, it remains the most common and clinically significant nutritional complication of this surgery with the potential to negate quality of life and the health benefits of surgical weight loss. This narrative review summarizes the current literature regarding iron deficiency as it relates to patients with severe obesity and those who undergo MBS. Advances in the clinical knowledge of iron homeostasis in severe obesity

SAGES guidelines for the management of comorbidities relevant to metabolic and bariatric surgery.

BACKGROUND: Patients who are under consideration for or have undergone metabolic and bariatric surgery frequently have comorbid medical conditions that may make their perioperative care more complex. These recommendations address routine intraoperative cholangiography in patients with bypass-type anatomy, the management of reflux disease after sleeve gastrectomy, and the optimal bariatric procedure for patients with comorbid inflammatory bowel disease. METHODS: A systematic review was conducted including studies published from 1990 to 2022 to address these questions. These results were then p

Biliary Complications in Liver Transplant Recipients With a History of Bariatric Surgery.

OBJECTIVES: Bariatric surgery can greatly ameliorate obesity and its associated metabolic disorders. Alteration of foregut anatomy, as is seen after Roux-en-Y gastric bypass and biliopancreatic diversion with duodenal switch, renders traditional access to the biliary tree difficult, if not impossible. This may complicate management of anastomotic biliary complications after liver transplant. MATERIALS AND METHODS: In this single-center study, we retrospectively reviewed all adult patients with a history of any bariatric surgery who underwent liver transplant during the period January 2017 to

Impact of Weight Loss on Olfaction in Patients Undergoing Bariatric Surgery.

PURPOSE: Olfaction plays a vital role in regulating nutrition, avoiding potentially dangerous situations, and modulating behavior along with interpersonal relationships. Many common disorders can have a negative impact on olfaction; one of these is obesity. Bariatric surgery induces weight loss, and current evidence suggests that it can cause improvement in olfaction. However, more information is needed regarding this topic. MATERIALS AND METHODS: One hundred eighty-five patients who underwent bariatric surgery and 184 age- and gender-matched control group who did not undergo any surgery have

The Role of Informal Social Support for Patients Undergoing Bariatric Surgery.

BACKGROUND: Bariatric surgery is underutilized as a treatment for metabolic disease and its associated comorbidities. While social support is known to play a crucial role in outcomes following bariatric surgery, little is known about the role of social support prior to surgery, which may impact preparedness for and willingness to undergo surgery. The study's objective was to examine the role of informal social support prior to bariatric surgery, the types of support received, and patient attitudes toward different demonstrations of support. METHODS: We conducted semi-structured interviews wit

Association between bariatric surgery preoperative chemoprophylaxis and postoperative bleeding.

BACKGROUND: Use of chemoprophylaxis for prevention of venous thromboembolism (VTE) after bariatric surgery is a generally accepted principle; however, the optimal strategy in terms of medication type and pre- and postoperative dosing is uncertain. In our healthcare system, four hospitals performed bariatric surgery and utilized differing protocols for VTE prophylaxis. The analysis sought to evaluate the association of differing prophylaxis strategies on bleeding and VTE occurrence. METHODS: Adult patients who underwent laparoscopic sleeve gastrectomy (SG) or Roux-en-Y gastric bypass (RYGB) fr
