Molecular Analysis of Biliary Tract Neoplasms Associated with Familial Adenomatous Polyposis.

OBJECTIVES: Familial adenomatous polyposis (FAP) is an inherited disorder characterized by multiple colorectal polyposis and is frequently associated with a variety of extracolonic lesions. However, neoplastic lesions of the biliary tract system, excluding those of the ampulla of Vater, are extremely rare in FAP, and the impact of APC alterations on their tumorigenesis remains unclear. We aimed to clarify the relationship between germline variants and somatic variants in the APC gene in biliary tract neoplasms (BTNs) of FAP patients. METHODS: A total of 115 genetically confirmed FAP cases tre…

Immunotherapy in advanced biliary tract cancer: current status and future directions.

Biliary tract cancers (BTCs) are aggressive, molecularly heterogeneous malignancies, and most patients present with unresectable or metastatic disease at diagnosis. For more than a decade, gemcitabine-platinum chemotherapy served as the first-line standard of care, with only modest survival benefit. The phase 3 TOPAZ-1 and KEYNOTE-966 trials have now established chemoimmunotherapy with immune checkpoint inhibitors as the new standard front-line regimen, and the phase 2 IMbrave151 trial is evaluating whether adding antiangiogenic therapy provides further benefit. Nonetheless, only a minority of…

Early-life body size and risk of developing biliary tract cancers.

BACKGROUND: Childhood body mass index (BMI) trajectories, BMI, height, and birth weight were investigated in relation to biliary tract cancer (BTC) risk in this population-based cohort study. METHODS: The study included 172,113 males and 168,503 females born between 1930 and 1996 from the Copenhagen School Health Records Register. Heights and weights measured at ages 6-15 years identified five sex-specific BMI trajectories. BMI and height were analyzed as z scores; overweight was defined via US Centers for Disease Control and Prevention criteria. Sex-specific hazard ratios (HRs) were estimate…

Advancing biliary tract malignancy treatment: emerging frontiers in cell-based therapies.

Biliary tract malignancies, including intrahepatic cholangiocarcinoma, extrahepatic cholangiocarcinoma, and gallbladder cancer, represent a group of aggressive cancers with poor prognosis due to late-stage diagnosis, limited treatment options, and resistance to conventional therapies like chemotherapy and radiotherapy. These challenges emphasize the urgent need for innovative therapeutic approaches. In recent years, cell-based therapies have emerged as a promising avenue, offering potential solutions through immune modulation, genetic engineering, and targeted intervention in the tumor microen…

Genomic landscape of biliary tract cancer and corresponding targeted treatment strategies.

Biliary tract cancers (BTCs) are classified on the basis of their anatomical origin, and the feasibility of surgical resection depends on the tumor location and extent of progression. However, for unresectable BTCs, systemic therapy has been uniformly applied. Gemcitabine and cisplatin (GC) therapy and GC-based therapies were established as the first-line standard BTC treatment. However, no highly effective second-line therapy has been established, and the prognosis remains poor, highlighting the need for further therapeutic advancements. Meanwhile, the era of precision medicine has expanded t…

Surgical Management Of Feline Biliary Tract Disease: Decision-making and techniques.

PRACTICAL RELEVANCE: Diseases of the hepatobiliary system are not uncommon in first-opinion practice. The anatomy of the feline hepatobiliary tract differs from that of dogs and may predispose cats to inflammatory hepatobiliary disease. Case management can be challenging, and the prognosis is variable and dependent on the underlying disease process. CLINICAL CHALLENGES: Biliary tract disease in the cat has non-specific clinical signs that overlap with those of diseases of other body systems. Diagnostic tests of the hepatobiliary system are widely available, but interpretation of results can b…

Hereditary cancer variants and homologous recombination deficiency in biliary tract cancer.

BACKGROUND & AIMS: The heritability and actionability of variants in homologous recombination-related genes in biliary tract cancers (BTCs) are uncertain. Although associations between BTC and BRCA germline variants have been reported, homologous recombination deficiency has not been investigated in BTCs. METHODS: We sequenced germline variants in 27 cancer-predisposing genes in 1,292 BTC cases and 37,583 controls without a personal nor family history of cancer. We compared pathogenic germline variant frequencies between cases and controls and documented the demographic and clinical character…

Emerging treatment evolutions and integrated molecular characteristics of biliary tract cancers.

Biliary tract cancers (BTCs) consist of a group of highly heterogeneous malignancies that are characterized by genomic differences among tumors from different anatomic sites. The current treatment for BTC includes surgery, chemotherapy, target therapy, and immunotherapy. Although surgery remains the primary option for localized disease, representing the only potential curative treatment, a high risk of recurrence cannot be neglected. Chemotherapy has been considered the standard of care for both advanced and metastatic disease and in adjuvant settings. However, drug resistance is a major obsta…

3D printed soft surgical planning prototype for a biliary tract rhabdomyosarcoma.

Biliary tract rhabdomyosarcoma is a soft tissue malignant musculoskeletal tumor which is located in the biliary tract. Although this tumor represents less than 1% of the total amount of childhood cancers, when localized, a >70% overall 5-year survival rate, the resection is clinically challenging and complications might exist during the biliary obstruction. Although surgery remains a mainstay, complete tumor resection is generally difficult to achieve without mutilation and severe long-term sequelae. Therefore, manufacturing multi-material 3D surgical planning prototypes of the case provides a…

Phylogeny, Resistome, and Virulome of Escherichia coli Causing Biliary Tract Infections.

Escherichia coli is the most frequent Gram-negative bacilli involved in intra-abdominal infections. However, despite high mortality rates associated with biliary tract infections due to E. coli, there is no study focusing on this pathogen. In this study, we have characterized a group of 15 E. coli isolates obtained from 12 patients with biliary tract infections. Demographic and clinical data of the patients were recovered. Phylogeny, resistome, and virulome analysis through whole genome sequencing and biofilm formation were investigated. Among the 15 E. coli isolates, no predominant sequence t…

Second-line chemotherapy in biliary tract cancer: Outcome and prognostic factors.

BACKGROUND & AIMS: The prognosis of biliary tract cancer (BTC) is poor. Standard treatment for advanced BTC is a chemotherapy (CT) with gemcitabine and cisplatin. Phase III evidence for a second-line (2L) CT is lacking. We aimed to investigate the feasibility of a 2L CT, to estimate the outcome and to identify prognostic markers. METHODS: Patients of our institution with advanced BTC between 2000 and 2015 receiving CT were included. Data were analysed in univariate and multivariate analysis. RESULTS: Three-hundred and fifteen and 144 patients (45.7%) received first-line (1L) and 2L CT respec…

Neonatal cholestasis mimicking biliary atresia: Could it be urinary tract infection?

Cholestasis can occur in newborns due to infections. However, the manifestations of the underlying infections usually dominate the presentation. We present a 2-month-old infant who presented with jaundice and no fever or signs of systemic illness. Liver biopsy was suggestive of cholangitis. He was subsequently detected to have urinary tract infection with Klebsiella pneumoniae. The child was treated with appropriate antibiotics for 2 weeks following which the cholestasis resolved. Thus, neonatal cholestasis due to infections can also occur in the post-neonatal period without clinical manifesta
