A newborn with massive intra-abdominal haemorrhage.

We describe an unusual case of an intra-abdominal teratoma with massive bleeding presenting as sudden cardiovascular collapse immediately after birth. The infant required massive volume and blood product transfusion, alongside emergent tumour resection, in order to obtain haemostasis and haemodynamic stabilisation. Haemorrhage of a sacrococcygeal teratoma is a known entity, but we are not aware of a previous report of massive intra-abdominal haemorrhage due to a teratoma, immediately after birth. This case emphasises the need to consider intra-abdominal pathology, including a teratoma, in the

The impact of postpartum contraception on reducing preterm birth: findings from California.

OBJECTIVE: Family planning is recommended as a strategy to prevent adverse birth outcomes. The potential contribution of postpartum contraceptive coverage to reducing rates of preterm birth is unknown. In this study, we examine the impact of contraceptive coverage and use within 18 months of a birth on preventing preterm birth in a Californian cohort. STUDY DESIGN: We identified records for second or higher-order births among women from California's 2011 Birth Statistical Master File and their prior births from earlier Birth Statistical Master Files. To identify women who received contracepti

Key dimensions of women's and their partners' experiences of childbirth: A systematic review of reviews of qualitative studies.

BACKGROUND: The World Health Organization 2018 intrapartum guideline for a positive birth experience emphasized the importance of maternal emotional and psychological well-being during pregnancy and the need for safe childbirth. Today, in many countries birth is safe, yet many women report negative and traumatic birth experiences, with adverse effects on their and their families' well-being. Many reviews have attempted to understand the complexity of women's and their partners' birth experience; however, it remains unclear what the key dimensions of the birth experience are. OBJECTIVE: To syn

[Indications and prognosis of cesarean section at Befelatanana maternity unit--Central University Hospital of Antananarivo (apropos of 529 cases, during the year 1998)].

High maternal mortality rate (12 death per 1,000 births, in 1997) has suggested this study in order to improve cesarean's management, then to reduce maternal death. Therefore, during the year 1998, 529 women among 8,366 had cesareans, which are 6.8%. The epidemiological profile reveal evacuation (82.4% of the cases); their average age is 29 years old and the average parity were 3. Principal indications are: dystocia (26.8%), fetal distress (38.1%), dynamic dystocia, previous uterine incision (11.2%). Maternal prognosis is mediocre because of a high mortality rate (2% of cesarean) and an import

Modelling period fertility: Schooling and intervals following a birth in Eastern Africa.

We describe a regression-based approach to the modelling of age-, order-, and duration-specific period fertility, using retrospective survey data. The approach produces results that are free of selection biases and can be used to study differential fertility. It is applied to Demographic and Health Survey data for Ethiopia, Kenya, Tanzania, and Zimbabwe to investigate differential trends in fertility by education. Parity progression fell and the intervals following each birth lengthened between the 1970s and 2000s in all four countries. Fertility fell most among women with secondary education.

Mass Incarceration, Maternal Vulnerability, and Birth Outcomes Across U.S. Counties.

OBJECTIVES: To examine the associations among mass incarceration, maternal vulnerability, and disparities in birth outcomes across U.S. counties, utilizing an ecological model and reproductive justice perspective was used. This study tests whether mass incarceration is associated with infant mortality and low birthweight across U.S. counties, and whether maternal vulnerability explains the relationship between mass incarceration and birth disparities. METHODS: Data were derived from a variety of public sources and were merged using federal FIPS codes. Outcomes from the CDC Vitality Statistics

Maternal age in the epidemiology of common autosomal trisomies.

The birth prevalence rate of each common autosomal trisomy generally increases with advancing maternal age and there is a substantial fetal loss rate between late first trimester and term. The literature is reviewed in order to provide the best estimates of these rates, taking account where possible of biases due to prenatal diagnosis and selective termination of pregnancy. There is an almost exponential increase in Down syndrome birth prevalence between ages 15 and 45 but at older ages the curve flattens. There is no evidence of the claimed relatively high birth prevalence at extremely low ag

Epidemiology of preterm birth and its clinical subtypes.

Preterm birth (<37 weeks) complicates 12.5% of all deliveries in the USA, and remains the leading cause of perinatal mortality and morbidity, accounting for as many as 75% of perinatal deaths. Despite the recent temporal increase in preterm birth, efforts to understand the problem of prematurity have met with little success. This may be attributable to the under-appreciation of the etiologic heterogeneity of preterm birth as well as the heterogeneity in its underlying clinical presentations--spontaneous onset of labor, preterm premature rupture of membranes, and medically indicated preterm bir

Orthopaedic considerations in obstetric brachial plexus palsy.

Obstetric brachial plexus palsies are seen in orthopaedic practice today despite improved obstetric care. The incidence is believed to be 0.35 to 0.87 cases per 1,000 live births. The pathomechanics of these injuries are not fully understood, although numerous risk factors have been identified. Large birth weight, breech presentation, shoulder dystocia, and forceps-assisted deliveries have all been implicated. No one standard treatment modality exists. Therefore, the treating orthopaedist must be familiar with this entity and with the various accepted treatment approaches in order to formulate

A partial ranking method for identifying repeated inclusion of individuals in anonymized HIV infection reports.

Diagnoses of HIV infection are reported to the Public Health Laboratory Service (PHLS) AIDS Centre under a voluntary surveillance scheme. Names are not held in the data set, but the date of birth of the individual concerned is usually available. This paper describes a statistical method for identifying whether there are likely to be individuals repeatedly represented in the resulting data set, which is considered by birth year. A partial ordering method is used that is especially useful for years where the number of birth years in the sample is too small for chi2 tests to be used. At the 5% le

Improving Timeliness of Hepatitis B Vaccine Birth Dose Administration.

BACKGROUND AND OBJECTIVES: When given within 24 hours of birth, the hepatitis B vaccine is up to 90% effective in preventing perinatal infection. The American Academy of Pediatrics now recommends administration within 24 hours for infants with a birth weight >2 kg, but a national benchmark for compliance with this time frame has not been established. We aimed to increase the monthly average of eligible newborns receiving the vaccine on time from 40% to 80% over a 9-month period. METHODS: A series of plan-do-study-act cycles were conducted to improve timeliness of hepatitis B vaccine birth dos
