Modified Masquelet's technique with nail and allograft: A case report.

The large bone defect treatment is a challenge issue in modern orthopaedic trauma surgery. One of the most used technique is the Masquelet's technique. In this case report we used a modified Masquelet technique to fill a six centimeters bone gap in the proximal femur. A 18-year-old boy with a septic pseudoarthrosis was treated with a two stages procedure: in the first step we used a hollow antibiotic spacer and an intramedullary nail. In the second step, we used both omologous (6 cm of bone allograft) and autologous bone graft with a new intramedullary nail. Immediate partial weight bearing wa

Surgical approach to fragility fractures: problems and perspectives.

The frequency of osteoporosis is constantly increasing all over the world. This pathology generates several problems, mostly due to fragility fractures, the worst consequence of impaired bone quality. Osteoporotic fractures often cause disability and loss of independence, partly because fracture fixation is not always easy and durable. So orthopedic surgeons need to learn and use new techniques to improve bone healing and surgical outcome, in order to grant fragility fracture patients a good quality of life. There are nails, screws and plates designed to maximize the bone-implant interface, su

Internal fixation of supracondylar femoral fractures: comparative biomechanical performance of the 95-degree blade plate and two retrograde nails.

OBJECTIVE: The biomechanical stability of supracondylar femoral fractures fixed with a condylar blade plate (plate), a Green Seligson Henry nail (GSHN), or a new retrograde unreamed supracondylar femoral nail (new nail) based on the AO unreamed femoral nail were compared. DESIGN: A standardized simulated comminuted supracondylar femoral fracture (segmental defect) in fresh frozen paired cadaveric femora was stabilized with one of the implants. The interfragmentary fracture site stiffness in three directions and axial strength of the fixator-bone construct were compared (pairwise). RESULTS: T

[Tibiotalocalcaneal arthrodesis using a retrograde nail locked in the sagittal plane].

PURPOSE OF THE STUDY: To evaluate our experience with the use of a retrograde nail locked in the sagittal plane for tibiotalocalcaneal arthrodesis indicated in severe post-traumatic arthritis of the ankle. MATERIAL: Twenty patients, 16 men and four women at an average age of 58.7 years (range, 23 to 72) were evaluated. All patients had severe post-traumatic changes in the talocrural and talocalcaneal joints. Five patients also had an equinus deformity. In two patients arthrodesis followed the treatment of purulent arthritis of the talocrural joint. A local fasciocutaneous flap was used for so

Early Failure of the Paragon28 Phantom Hindfoot Fusion Nail: A Case Report.

CASE: Tibio-talar-calaneal fusion nails can be used to treat acute ankle fractures in patients with diabetes, Charcot arthropathy, or other comorbidities. Recently, Paragon28 released the Phantom Hindfoot Fusion Nail (Phantom nail) with a unique design feature-a proximal coil. We present a case of an ankle fracture-dislocation in a diabetic patient treated with the Paragon28 nail who experienced early implant failure. CONCLUSION: It is important for surgeons to understand that the proximal coil in the Phantom nail should not be considered part of the working length of the nail and may introdu

A Comparative Study of Nail Fold Approach and Microscopic Nail Bed Approach in the Surgical Treatment of Subungual Exostosis.

Subungual exostosis is a benign tumor that occurs under the nail plate of the distal phalanx and causes nail deformity and pain. There are many reports of recurrence and postoperative nail deformity, and the surgical approach needs to be reassessed. Two typical surgical approaches are from the nail fold and the nail bed. Here, we compare 2 surgical approaches for subungual exostosis that were performed in our department. Twenty-three cases of subungual exostosis were treated between 2010 and 2019; 12 cases were treated by the nail fold approach and 11 by the microscopic nail bed approach. The

Position Statement: Atypical Femoral Fracture from the Korean Society for Bone and Mineral Research in 2023.

As the aging population increases, the number of patients with osteoporosis is gradually rising. Osteoporosis is a metabolic bone disease characterized by low bone mass and the microarchitectural deterioration of bone tissue, resulting in reduced bone strength and an increased risk of low-energy or fragility fractures. Thus, the use of anti-resorptive agents, such as bisphosphonates (BPs), to prevent osteoporotic fractures is growing annually. BPs are effective in reducing hip and other fractures. However, the longer a patient takes BPs, the higher the risk of an atypical femoral fracture (AFF

Percutaneous autologous bone marrow transplantation for nonunion of the femur.

Percutaneous bone marrow injections were performed on 7 nonunions of the femur. There were 6 hypervascular nonunions and one avascular nonunion. Two nonunions presented with active infections. One other patient had a history of infection which had subsided. One nonunion received the injection twice. After the site of nonunion was curetted and the bone surface was scored, 150 ml of bone marrow aspirated from the iliac bone was injected. Complete union occurred in 4 patients within 9 months; all of them were uninfected hypervascular nonunions following intramedullary nail fixation. One nonunion

Biomechanical analysis of protective plating configurations for interimplant femoral fracture prevention.

INTRODUCTION: Interimplant femoral fractures (IFFs), occurring between or adjacent to implants such as hip prostheses and intramedullary nails, pose complex treatment challenges, particularly in osteoporotic patients. Biomechanical research highlights the need to protect the interimplant region from high strains, however, optimal configurations for plate fixation remain unclear. This study analyzes strains in the interimplant region under various protective plate configurations. MATERIALS AND METHODS: Twelve synthetic proximal femora, mimicking osteoporotic bone, were instrumented with a prox

Plate-tensioned reconstruction nail for proximal femur diaphyseal nonunion.

A "plate-tensioned nail" implant construct is described as a single-stage salvage procedure for proximal femur diaphyseal nonunion. This construct combines the stability of a medullary nail with the compression afforded by a tension band, permitting immediate full weight bearing. The proximal hole of a 4.5-mm plate is modified to accept a 6.5-mm recon screw placed through the plate and nail into the femoral neck and head. Plate-based compression or an articulated tensioning device is used to compress across the nonunion site while affixing the plate to the lateral femoral diaphysis. Eight adul

Ungual squamous cell carcinoma mimicking a chronic paronychia: clinical, pathological and radiological correlation.

Squamous cell carcinoma is a malignant skin tumor that is rarely located in the nails. Its presentation, can sometimes be confused with an inflammatory process as onychomycosis or psoriatic onychopathy or other tumors as viral warts. We report the case of a 73-year-old woman with a locally invasive squamous cell carcinoma reaching the bone, simulating a chronic paronychia.

Maximizing length with precice nail: a novel technique.

Intramedullary magnetic lengthening nails represents a marked advance in the process of limb lengthening, reducing the complications associated with other methods, and improving patient's comfort. However, one important limitation is its lengthening ability, especially when it comes to short bones. Previous reports have shown techniques that may allow further lengthening but at a cost of having to carry out repeated procedures, requiring the use of other devices, and submitting the patient to long-winded surgical procedures. This report presents a novel technique to achieve lengthening beyond

New concepts in the surgical treatment of actual and impending pathological fractures in metastatic disease.

INTRODUCTION: Long bone metastases are a disease of high social importance. The goals of surgical treatment are to relieve pain, maintain or restore joint function, and prevent or treat pathological fractures. "Oligometastases" is a disease with a limited number (3-5) of metastatic lesions in the same body district, where an aggressive treatment can be carried out with "curative" intent. This study aimed to evaluate patients with bone metastases surgically treated to determine how surgical treatment can influence prognosis and quality of life, comparing solitary metastasis, oligometastases, an

Treatment outcome in 60 children with pathological fractures of the humerus caused by juvenile or aneurysmal bone cysts.

The treatment of pathological fractures of the humerus caused by juvenile or aneurysmal bone cysts (JBC/ABC) should be a single approach with a high success rate and low complication rate. This study evaluates how day by day treatment concepts fulfil these aims. Children below 15 years of age with a pathological fracture of the humerus caused by a JBC or ABC between 01.01.2001 and 31.12.2010, were investigated by chart review in four major paediatric trauma centres. Age, gender, fracture localisation, X-ray findings, treatment and outcome - assessed by the Capanna classification (I to IV), wer

A single-channel telemetric intramedullary nail for in vivo measurement of fracture healing.

OBJECTIVE: The objective of this study was to develop a single-channel telemetric intramedullary nail that measures anterior-posterior bending strains and determine whether these forces decrease sigmoidally when normalized to the ground reaction force during fracture healing. METHODS: A transverse midshaft femoral osteotomy (1 mm) was stabilized using a customized TriGen intramedullary nail incorporating a strain gauge in the anterior-posterior plane. Fourteen skeletally mature sheep (2-3 years old) were treated in two pilot studies (n = 3/pilot) and a pivotal study (n = 8). Three animals wer
