Removal of spinopelvic S2 alar-iliac osseointegrative screws: a case report and surgical technique guide.

BACKGROUND: Traditional S2 alar-iliac (S2AI) screws have an increased risk of screw loosening compared to iliac screws (IS), leading to advancements in technology with osseointegrative S2AI screws to increase bone integration and stability. Though the overall increased stability is desired, this can also pose an obstacle if removal of the implants are required. CASE DESCRIPTION: A 66-year-old female s/p T10-S2 posterior spinal fusion (PSF) nine months prior presented with sepsis, necessitating the removal of the implants including osseointegrative iFuse Granite S2AI implants (SI-BONE, Inc.; S

Comparison of expansive pedicle screw and polymethylmethacrylate-augmented pedicle screw in osteoporotic sheep lumbar vertebrae: biomechanical and interfacial evaluations.

BACKGROUND: It was reported that expansive pedicle screw (EPS) and polymethylmethacrylate-augmented pedicle screw (PMMA-PS) could be used to increase screw stability in osteoporosis. However, there are no studies comparing the two kinds of screws in vivo. Thus, we aimed to compare biomechanical and interfacial performances of EPS and PMMA-PS in osteoporotic sheep spine. METHODOLOGY/PRINCIPAL FINDINGS: After successful induction of osteoporotic sheep, lumbar vertebrae in each sheep were randomly divided into three groups. The conventional pedicle screw (CPS) was inserted directly into vertebra

Safety of terminally gamma-ray-sterilized screws coated with fibroblast growth factor 2-calcium phosphate composite layers in non-human primates.

Screws coated with fibroblast growth factor 2 (FGF-2)-calcium phosphate (CP) composite layers exhibit enhanced soft tissue and bone formation and angiogenesis because of the biological activity of FGF-2. Furthermore, the mitogenic activity of the FGF-2 within the composite layers remains unchanged after gamma-ray sterilization, which may improve the storage stability prior to clinical use. However, the in vivo safeties of these screws as spinal implants remain unknown. Here, a randomized controlled trial, involving non-human primates, investigated the safety of using FGF-2-CP composite layer-c

Implant stability is affected by local bone microstructural quality.

It is known that low bone quality, caused for instance by osteoporosis, not only increases the risk of fractures, but also decreases the performance of fracture implants; yet the specific mechanisms behind this phenomenon are still largely unknown. We hypothesized that especially peri-implant bone microstructure affects implant stability in trabecular bone, to a greater degree than more distant bone. To test this hypothesis we performed a computational study on implant stability in trabecular bone. Twelve humeral heads were measured using micro-computed tomography. Screws were inserted digital

Comparison of the in vitro holding strengths of conical and cylindrical pedicle screws in a fully inserted setting and backed out 180 degrees.

Previous investigations have suggested that conical and cylindrical pedicle screws have comparable holding strengths. So far, the remaining performance in screws turned back or loose as a result of other reasons has not been determined. Twenty-four cadaveric spines from 6- to 8-week-old calves were examined. After bone mineral density was determined, four pedicle screws (two conical and two cylindrical screws) were inserted. The screws were fully inserted and half of them turned back 180 degrees. Twenty-four axial pullout and 24 cyclic loading tests with subsequent pullout tests were conducted

A Retrospective Comparison of Headless Versus Standard Interlocking Screw Fixation of Tibia Intramedullary Nails.

INTRODUCTION: Symptomatic interlocking screws are common after intramedullary nail fixation of tibia fractures. Low-profile headless interlocking screws recently became available and could potentially reduce the rate of symptomatic screws. The purpose of this study was to compare the rate of symptomatic screws and screw removals between these screw types. METHODS: A retrospective review identified patients with tibia shaft fractures treated with tibia intramedullary nails between August 1, 2018 to September 1, 2023 by 11 surgeons. Low-profile headless interlocking screws became available on J

Patient-Specific Finite Element Models of Posterior Pedicle Screw Fixation: Effect of Screw's Size and Geometry.

Pedicle screw fixation is extensively performed to treat spine injuries or diseases and it is common for thoracolumbar fractures. Post-operative complications may arise from this surgery leading to back pain or revisions. Finite element (FE) models could be used to predict the outcomes of surgeries but should be verified when both simplified and realistic designs of screws are used. The aim of this study was to generate patient-specific Computed Tomography (CT)-based FE models of human vertebrae with two pedicle screws, verify the models, and use them to evaluate the effect of the screws' size

Primary stability of three different iliosacral screw fixation techniques in osteoporotic cadaver specimens-a biomechanical investigation.

BACKGROUND: The incidence of osteoporotic and insufficiency fractures of the pelvic ring is increasing. Closed reduction and percutaneous fixation with cannulated sacroiliac screws is well-established in the operative treatment of osteoporotic posterior pelvic ring fractures. However, osteoporotic bone quality might lead to the risk of screw loosening. For this reason, cement augmentation of the iliosacral screws is more frequently performed and recommended. PURPOSE: The aim of the present biomechanical study was to evaluate the primary stability of three methods of iliosacral screw fixation

Biomechanical evaluation of reconstruction plates in bone-grafted mandibular defects: a finite element analysis.

OBJECTIVE: To evaluate, through three-dimensional finite element analysis (FEA), the stress and strain distribution in reconstruction plates of different thicknesses used for fixation of bone-grafted segmental mandibular defects. METHOD: A dentate mandibular model was obtained from computed tomography data and digitally processed for FEA using ANSYS 7.0. Three reconstruction plate models (2.0, 2.4, and 3.0 mm thick) were applied to simulate defects in the symphyseal, body, and angle regions of the mandible, each associated with block bone grafts and fixed using 2.4-mm locking screws. A 300-N

Biomechanical Comparison of Sustained Dynamic Compression Screws vs Static Compression Screws in Subtalar Arthrodesis: A Cadaveric Study.

BACKGROUND: Subtalar arthrodesis is a reliable procedure to address subtalar arthritis, though nonunion remains a challenge. Adequate compression is critical to optimizing fusion, but bone resorption and settling impair the ability of standard static compression (SC) screws to maintain compression. Sustained dynamic compression (SDC) screw constructs that use the pseudoelastic property of nitinol may enable more sustained compression than static compression screws. METHODS: Ten fresh talo-calcaneal cadaveric specimens were instrumented with either 2 SC screws or 2 SDC nitinol screws. Using a

Biocompatibility and healing patterns in experimentally induced canine tibial fractures using Pedicle screw-Rod external fixation.

BACKGROUND: Biological osteosynthesis preserves blood supply and promotes rapid healing by aligning fracture fragments without direct surgical exposure. Pedicle screws are primarily designed for internal fixation in spinal procedures. A key objective of many orthopedic studies is to assess the biocompatibility of implants with bone and adjacent soft tissue. This study aims to evaluate the biocompatibility and effects of the Pedicle screw-Rod configuration as a novel external fixation method in canine tibial osteotomy. METHODS: With ethics approval, eight healthy, intact male dogs, aged 10-12

Adjustable-Loop Femoral Cortical Suspensory Fixation for Patellar Tendon Anterior Cruciate Ligament Reconstruction: A Time Zero Biomechanical Comparison With Interference Screw Fixation.

BACKGROUND: Adjustable-loop cortical buttons for femoral fixation of bone-tendon-bone grafts have potential advantages over interference screw fixation; however, these devices have not been benchmarked biomechanically against interference screws. Purpose/Hypothesis: The purpose was to compare the time zero biomechanical properties of commercially available, adjustable-loop cortical button and metallic interference screws for femoral fixation of bone-tendon-bone grafts. It was hypothesized that no significant differences would be found in biomechanical properties between fixation techniques. S

Removal of infected bone in-growth pelvic fixation implants and microcomputed tomography analysis of osseous integration: illustrative case.

BACKGROUND: A novel pelvic fixation screw fusion device (iFuse Bedrock Granite implant) was designed to promote bony fusion through self-harvesting fenestrations throughout the outer screw shank. Bone on-growth and in-growth using this design have been demonstrated in a sheep model, but data from human subjects have not been reported. A 66-year-old medically complex female with 2 prior spine fusions developed spondylodiscitis cephalad to a prior fusion, requiring the removal of instrumentation, which included screw fusion devices. OBSERVATIONS: Within this case report, the authors present a n

Application study of three-dimensional printed navigation template between traditional and novel cortical bone trajectory on osteoporosis lumbar spine.

To investigate the safety, accuracy and indications of traditional and novel cortical bone screws placement for osteoporosis lumbar spine, 4 lumbar vertebra specimens (2 males and 2 females) were used for this study. After the computed tomography scanning data of the above anatomical specimens were three-dimensional (3D) reconstructed, one side of each anatomical specimen was randomly chosen to place traditional cortical bone screws, and the other side received novel technical placement. The safety screw trajectory was designed, and a 3D navigation template complementary to the surface anatomi…

Biomechanical design of a new proximal humerus fracture plate using alternative materials.

Comminuted proximal humerus fractures are often repaired by metal plates, but potentially still experience bone refracture, bone "stress shielding," screw perforation, delayed healing, and so forth. This "proof of principle" investigation is the initial step towards the design of a new plate using alternative materials to address some of these problems. Finite element modeling was used to create design graphs for bone stress, plate stress, screw stress, and interfragmentary motion via three different fixations (no, 1, or 2 "kickstand" [KS] screws across the fracture) using a wide range of plat

Preventing Iatrogenic Fibula Fractures Using the Push-Pull Technique: A Biomechanical Comparison of Unicortical Versus Bicortical Post Screws.

BACKGROUND: Displaced diaphyseal fractures can be reduced using the push-pull technique, wherein a plate is affixed to the distal fragment of the fracture, a post screw is placed proximal to the plate, and a lamina spreader creates distraction. This study evaluated the load to failure and mechanism of failure of bicortical and unicortical post screws during reduction. MATERIALS AND METHODS: Four matched pairs of cadaver legs were subjected to a 2-cm oblique osteotomy simulating a displaced, oblique diaphyseal fracture. A 6-hole compression plate was affixed to the distal fragment with 2 unico

The loneliness of a long-distance runner. A ten-year survey of a patient diagnosed with Gorham-Stout syndrome at the occipitocervical junction.

This case study presents a 47-year-old male who was diagnosed with Gorham-Stout syndrome (GSS) 10 years ago in the occipitocervical junction. The pathology caused the resorption of the suboccipital bone, clivus, foramen magnum, and C1-C3 laminae. After his first fusion attempt in 2010 using occipital plate-cervical lateral mass screws, he needed many revision surgeries either for the progression of the pathology or for instrumentation failure and wound healing problems. Eventually, a new occipital plate and cervical pedicle screws were applied to obtain the exact solution. The involvement of t

Biomechanical comparison of posterior short-segment fixation with or without intermediate screws for thoracolumbar burst fractures under normal and osteoporotic conditions: a finite element analysis.

STUDY DESIGN: Finite element analysis. PURPOSE: To investigate the biomechanical response of posterior short-segment fixation with or without intermediate screws at the index vertebra in osteoporotic thoracolumbar burst fractures using finite element analysis. OVERVIEW OF LITERATURE: Spinal fixation in elderly patients with osteoporotic vertebral fractures is challenging because osteoporosis weakens the screw-bone interface, leading to screw loosening and loss of fracture reduction. Short segment fixation with intermediate screws has been proposed to reduce kyphosis recurrence and implant fa
