Compartment pressures in children with normal and fractured lower extremities.

PURPOSE: Needle manometry is a tool to confirm suspected acute compartment syndrome (ACS). There is scarce evidence of normal pressure values of the lower extremities in children. The aim of this study is to assess the normal compartment pressures in non-injured lower extremities of children. METHODS: This prospective study included children up to the age of 16 years with lower extremity fractures that needed reduction. Between June 2009 and August 2015, 20 children were included. We used needle manometry to measure the pressures in the superficial (SPC), deep posterior (DPC) and in the anter

Risk of Cardiovascular Disease Due to General Anesthesia and Neuraxial Anesthesia in Lower-Limb Fracture Patients: A Retrospective Population-Based Cohort Study.

The purpose of this study was to analyze the relationship between elevated cardiovascular disease (CVD) risk and type of anesthesia by using the National Health Insurance Research Database (NHIRD) of Taiwan in a one-year follow-up period. We assessed whether general anesthesia (GA) or neuraxial anesthesia (NA) increased CVD occurrence in lower-limb fracture patients. Approximately 1 million patients were randomly sampled from the NHIRD registry. We identified and enrolled 3437 lower-limb fracture patients who had received anesthesia during operations conducted in the period from 2010 to 2012.

Biological Intercalary Reconstruction with Bone Grafts After Joint-Sparing Resection of the Lower Limb: Is this an Effective and Durable Solution for Joint Preservation?

Due to advances in neoadjuvant therapies and preoperative imaging modalities, joint-sparing resections have become appealing in bone tumor surgery. However, the intercalary reconstruction of metadiaphyseal bone defects of the femur and the tibia after juxta-articular tumor resection remains challenging. Both biological and prosthetic reconstructions have been used for joint-sparing resections, but little is known about the long-term outcome of these procedures. The authors reviewed a consecutive series of 64 patients treated with joint-sparing intercalary resection and reconstruction with bone

Deformity correction in Haiti with the Taylor Spatial Frame.

The Taylor Spatial Frame (TSF) is an external-fixator that corrects deformities in 6 axes, and can successfully manage disorders involving multiplanar deformities. In the developing-world, orthopaedic surgeons are often faced with deformities from neglected trauma and birth defects more severe than those typically seen in developed-countries. This article evaluates the applicability of TSF in the challenging medical environment of Haiti. At Haiti Adventist Hospital, the authors treated 80 cases using the TSF with a minimum follow-up of 1-year. Good results were observed in 99% of the cases (79

Experience with patient-specific guides, insteatientad of general surgical experience, improves the accuracy of 3D-guided corrective osteotomies.

PURPOSE: This study investigated whether the accuracy of 3D-guided osteotomies is influenced by surgeon experience. Two types of experience were assessed: (1) general surgical experience, defined as self-reported years in practice as an orthopaedic trauma surgeon, and (2) prior use of patient-specific guides (PSGs). Secondary aim was to evaluate whether accuracy varies by anatomical location. METHODS: 24 orthopaedic-trauma surgeons performed 75 corrective osteotomies on cadaveric long-bones (43 single-cut, 32 double-cut; 107 cuts in total). Each osteotomy was preoperatively planned on CT-deriv

Recovery of decreased bone mineral mass after lower-limb fractures in adolescents.

BACKGROUND: Loss of bone mineral mass, muscle atrophy, and functional limitations are predictable consequences of immobilization and subsequent weight-bearing restriction due to leg or ankle fractures. The aim of this study was to prospectively determine whether decreased bone mineral mass following lower-limb fractures recovers at follow-up durations of six and eighteen months in adolescents. METHODS: In the present study, we included fifty adolescents who underwent cast immobilization for a leg or ankle fracture. Dual x-ray absorptiometry scans of four different sites (total hip, femoral ne

Secrets of Success in the Management of Lower Extremity Nonunions.

The key to obtaining healing of nonunions in the lower extremity is to provide a balance of biology and stability. To achieve this goal, the surgeon must understand why the bone did not heal with the initial treatment and change the strategy to improve the outcome. Patients need to be optimized before any proposed surgery. Whether the surgeon uses a certain type of internal or external fixation depends on the location on bone, the type (atrophic versus hypertrophic) of nonunion, the local soft tissue, the element of infection, and the health of the host. The mechanical stability of the fixatio

Use of the osteocutaneous free scapular flap on the lower extremities.

Reports on the use of osteocutaneous free scapular flap transfers have focused primarily on mandible reconstruction. Yet the scapula is a nearly straight bone, and it is also useful in the reconstruction of long bones. We would like to report on 23 patients undergoing osteocutaneous free scapular flap transfers to the lower extremities in whom we obtained extremely satisfactory results. These 23 surgical patients included 10 with pseudoarthrosis with osteomyelitis, 3 with pseudoarthrosis due to trauma, and 5 with osteomyelitis of the lower extremities, as well as 5 with other miscellaneous pro

Intraosseous Varix of the Left Tibia Causing Severe Lower Extremity Pain: A Report of a Rare Case.

Varicose veins are venous anomalies of the peripheral venous system that commonly present with lower extremity pain and swelling. Intraosseous varices are a very rare subtype of varicose veins with an atypical presentation and thus pose a significant diagnostic challenge, leading to prolonged patient morbidity. Only a handful of cases have been reported of this rare entity. We report a case of a 73-year-old male with a history of hyperlipidemia who presented with a 4-week history of worsening left lower extremity pain. Initial workup, including labs, radiography, and ultrasound Doppler, ruled

Local antibiotic therapy strategies in orthopaedic trauma: Practical tips and tricks and review of the literature.

The use of local antibiotics for the prevention of infection in the setting of open fractures and as part of the treatment of osteomyelitis is well established. Antibiotics are most commonly incorporated into polymethylmethacrylate (PMMA) cement, which can then be formed into beads, moulded to fit a bone defect or used to coat a guide wire or IM nail. Newer delivery vehicles and techniques are being evaluated to improve upon these methods. Many factors influence how local antibiotics are applied. Treatment strategies are challenging to standardise due to the variability of clinical presentatio

A Rare Case of Emphysematous Osteomyelitis With Concurrent Septic Arthritis.

A rare and possibly fatal infection of the bone called emphysematous osteomyelitis (EO) is caused by the presence of intraosseous gas due to gas-forming organisms. Common gas-producing organisms are in the Enterobacteriaceae family or are anaerobes. This gas within bones is most frequently detected using computed tomography (CT) imaging, and prompt diagnosis is important due to the high mortality rate. We present a 76-year-old male who complained of altered mental status, right upper and lower extremity weakness, and lower back pain. The MRI of the lumbar spine showed moderate edema in L3 and

Preserving the Knee in the Setting of High Segmental Tibial Bone and Massive Soft-Tissue Loss Using Vascularized Distal Tibial Bone Docking and a Foot Fillet Flap: A Case Report.

CASE: We discuss our reconstructive approach to avoid an above-knee amputation in a 33-year-old man presenting after lower extremity crush injury. We used a vascularized tibial bone flap and a foot fillet flap to restore length and joint functionality to the residual limb. The patient ambulates with good prosthetic fit on durable heel pad skin and 100° active knee motion. CONCLUSION: This pairing of intramedullary nail with vascularized bone flap and fillet flap to address soft-tissue coverage and retain limb length is a useful tool in traumatic lower extremity injury management, providing an
