[Negative myoclonus associated with high-dose trimethoprim-sulfamethoxazole for treating Nocardia brain abscess].

A 77-year-old man was treated for Nocardia abscessus brain abscess with high-dose trimethoprim-sulfamethoxazole (TMP-SMX) and ceftriaxone (CTRX). During the treatment, he developed consciousness disturbance and negative myoclonus (NM) in all four limbs. Initially, we suspected CTRX-induced neurotoxicity because the clinical phenotype, characterized by NM, strongly resembled cephalosporin encephalopathy. However, we considered CTRX unlikely to be the cause because the patient's renal function was preserved, and NM appeared during the period when CTRX was withheld. Consequently, we suspected TMP…

Hypertrophic olivary degeneration secondary to brain abscess: a case report and literature review.

INTRODUCTION: Hypertrophic olivary degeneration (HOD) is a rare transsynaptic neurodegenerative disorder arising from disruption of the Guil … ever, headache, and vomiting. Initial neuroimaging revealed right cerebellar hematoma with multiple brain abscesses. Antibacterial treatment achieved symptomatic improvement, but follow-up was lost. Seven months later, readmission occurred due to memory decline and personality changes. Magnetic resonance imaging (MRI) showed T2-weighted hyperintensity in the le …

Primary Brain Abscess Caused by Nocardia terpenica: a Case Report and Literature Review.

BACKGROUND: Nocardia terpenica is an exceptionally rare opportunistic pathogen, with infections scarcely documented in clinical practice. We report the first case of primary brain abscess caused by Nocardia terpenica and present a comprehensive review of the microbiological features and clinical presentations associated with this pathogen. METHODS: The patient underwent intracranial puncture and drainage due to a brain abscess. During the procedure, cerebrospinal fluid (CSF) was collected for culture. The isolated pathogen was subjected to Gram staining and modified acid-fast staining. Identi…

Otogenic Brain Abscess: Judicious Management in a Case of Chronic Suppurative Otitis Media.

Intracranial consequences from chronic otitis media can be dreadful. Meningitis is the most frequent complication followed by a cerebral abscess. In this pre-antibiotic era, otogenic brain abscess is rare, but it poses one of the life-threatening complications of otitis media. In recent years, brain abscess was noticed almost only in patients of chronic Otitis media with cholesteatoma. A case of a 36-year-old non-diabetic male patient with an otogenic cerebellar abscess, who presented with no cerebellar signs and unique intraoperative ossicular chain status was successfully managed by a combin…

Brain Abscess Associated with Polymicrobial Infection after Intraoral Laceration: A Pediatric Case Report.

Brain abscesses, infections within the brain parenchyma, can arise as complications of various conditions including infections, trauma, and surgery. However, brain abscesses due to polymicrobial organisms have rarely been reported in children. We herein report a case of a 9-year-old girl with unresolved congenital cyanotic heart disease (CCHD) presenting with right hemiplegia who was diagnosed with brain abscess caused by Streptococcus intermedius, Parvimonas micra, and Fusobacterium nucleatum after oropharyngeal injury. She was treated with intravenous antimicrobial therapy, drainage under cr…

Risk Factors for Brain Abscess: A Nationwide, Population-Based, Nested Case-Control Study.

BACKGROUND: Knowledge on risk factors for brain abscess is limited and relies on single-center cohort studies. METHODS: We accessed nationwide medical registries to conduct a population-based, nested case-control study of risk factors for brain abscess. We applied risk-set sampling for the selection of population controls (1:10), who were individually matched by age, sex, and area of residence. Conditional logistic regression was used to compute adjusted odds ratios (aOR) with 95% confidence intervals (CIs). Population-attributable fractions were calculated. RESULTS: We identified 1384 brain…

[Congenital Dermal Sinus with Dermoid Cyst Complicated by Brain Abscess: A Case Report].

Congenital dermal sinus(CDS)is a rare entity of spinal dysraphism, caused by the focal failure of disjunction leading to adhesion between the cutaneous and neural ectoderm. Some reports found that tumors, such as dermoid and epidermoid cysts, meningitis and subdural abscess are often complicated by CDS. A 1-year-old girl was referred to our department for CDS with a dermoid cyst complicated by brain and subdural abscesses. Diffusion weighted imaging and gadolinium-diethylenetriamine penta-aceticacid administration revealed ring-enhancing lesions, suggesting brain abscess, in the left temporal …

Streptococcus pluranimalium: Emerging Animal Streptococcal Species as Causative Agent of Human Brain Abscess.

BACKGROUND: Streptococcus pluranimalium is a new and emerging animal streptococcal species associated with primary infection in bovine and avian species. Data in the literature regarding its pathogenic significance in human beings are limited. We hereby report a case of brain abscess caused by S. pluranimalium in a healthy adult male. S. pluranimalium, a causative agent of brain abscess, was unanticipated, and to the best of our knowledge, this is one of the rare cases reported in the medical literature. CASE DESCRIPTION: A 44-year-old male presented with headache and occasional episodes of v…

Trueperella pyogenes isolated from a brain abscess of an adult roebuck (Capreolus capreolus).

The present study was designed to characterize phenotypically and genotypically a Trueperella pyogenes strain isolated from a brain abscess of an adult roebuck (Capreolus capreolus). The species identity could be confirmed by phenotypical investigations, by MALDI-TOF MS analysis, and by sequencing the 16S ribosomal RNA (rRNA) gene, the 16S-23S rRNA intergenic spacer region (ISR); by sequencing the target genes rpoB, gap, and tuf; and by detection of T. pyogenes chaperonin-encoding gene cpn60 with a previously developed loop-mediated isothermal amplification (LAMP) assay. The T. pyogenes strain…

Brain abscess caused by aerobic Gram-negative bacilli: clinical features and therapeutic outcomes.

Thirty-three patients (24 males and nine females) with brain abscesses resulting from infection by aerobic Gram-negative bacilli were identified at Kaohsiung Chang Gung Memorial Hospital over a period of 14 years. Of these, 23 cases developed spontaneously, with the remaining ten postneurosurgery. The organisms most frequently involved were Klebsiella (K.) pneumoniae, Pseudomonas aeruginosa, Escherichia coli and Proteus species and included some rare pathogens, such as Salmonella and Enterobacter species, K. oxytoca, Vibrio and Morganella morganii. Apart from one exception, the locations of th…

EEG in brain abscess: its value in localization compared to other diagnostic tests.

The EEG records of 13 cases of brain abscess were reviewed for their value in localization. Of the 11 cases of supratentorial abscesses localization was achieved in 10, and correct lateralization in the remaining 1 case. Two other cases were cerebellar abscesses; 1 of them showed a false cerebral localization; the other showed no focal abnormalities. The most consistent EEG localizing sign was focal arrhythmic delta waves. These were as slow as 0.5 c/sec in 6 of the 13 cases. Epileptiform discharges in the pre-operative EEG were seen in 4 of the 13 cases. Asymmetry of beta activity correctly l…

[Gunshot craniocerebral wound complicated by anaerobic brain abscess. Case report and literature review].

UNLABELLED: Brain abscess is a focal infectious inflammatory lesion characterized by collection of pus in brain tissue surrounded by a vascularized capsule. The most common cause is spread of microorganisms from distant foci of infection (e.g., odontogenic infection or sinusitis). The risk of intracranial infectious complications and mortality increases significantly in penetrating gunshot wounds to the skull and brain. MATERIAL AND METHODS: Available literature contains a few studies on infectious complications following combat traumatic brain injury. This article presents treatment of anaer…

Brain Abscess Following CPX-351 Treatment in a Patient With Acute Myeloid Leukemia.

We describe a rare case of a brain abscess occurring during CPX-351 induction therapy for acute myeloid leukemia (AML). The patient experienced recurrent febrile neutropenia during the myelosuppression period and developed left-sided spatial neglect during hematologic recovery. Brain magnetic resonance imaging revealed a solitary abscess in the right parietal lobe. Burr hole drainage yielded Bacillus species, and subsequent treatment with vancomycin and linezolid, followed by endoscopic abscess resection, led to gradual neurological recovery. Complete remission of AML was achieved, and consoli…

Temporal lobe brain abscess: Etiology, microbiology, surgical management, and outcomes - A systematic review.

BACKGROUND: Temporal lobe brain abscess is an uncommon but clinically significant intracranial infection most frequently associated with otogenic disease. Although brain abscess has been widely studied, location-specific analyses focusing on the temporal lobe remain limited. This systematic review aimed to evaluate the etiology, microbiology, surgical management, and outcomes of temporal lobe brain abscess reported in the literature. METHODS: A systematic literature search was performed in PubMed, Scopus, Web of Science, and Google Scholar to identify studies reporting temporal lobe brain abs…

Brain Abscess Arising From Pansinusitis in an Immunocompetent Young Adult: A Diagnostic Challenge.

Brain abscesses are rare but potentially life-threatening infections that require prompt recognition and treatment. Although they typically occur in patients with identifiable risk factors, they can occasionally present in otherwise healthy individuals. Sinusitis is a recognized source of intracranial infection, but its progression to a brain abscess in immunocompetent patients is uncommon and may be clinically subtle. We report the case of a 21-year-old previously healthy male who presented to the emergency department (ED) with a non-specific bifrontal headache. He was initially diagnosed wit…

Survival outcomes and prognostic factors of fungal brain abscess: A comparative cohort study.

BACKGROUND: Brain abscess (BA) is a life-threatening intracranial infection. While bacterial BA has been extensively studied, the prognostic impact of fungal BA remains uncertain. This study aimed to compare survival outcomes between fungal and bacterial BA and to identify independent prognostic factors. METHODS: We used a retrospective cohort study of patients with BA in a tertiary care center in southern Thailand. The classification of patients as fungal or bacterial BA was made by microbiological or pathological confirmation. Kaplan-Meier analysis and Cox proportional hazards were used to …

Nocardia farcinica brain abscess with torque teno virus co-infection: A case report.

BACKGROUND: Brain abscesses caused by Nocardia are rare and difficult to diagnose. Nocardia farcinica is among the most common species; however, the conventional diagnosis of N. farcinica infection consists of cerebrospinal fluid (CSF) and blood culture and Gram staining. These procedures prolong the time to diagnosis and initiating treatment. CASE PRESENTATION: A 69-year-old woman with diabetes mellitus presented with headaches and dizziness persisting for 2 weeks, which was initially diagnosed as a brain abscess. Due to the unusual presentation and rapid progression of symptoms, she underwe…
