Neuraxial Anesthesia for External Cephalic Version and the Risk of Emergent Cesarean Delivery: A Retrospective Cohort Study.

OBJECTIVE: External cephalic version (ECV) is performed to reduce cesarean delivery rates by rotating a breech fetus to cephalic presentation. Neuraxial anesthesia has been proposed to improve success by reducing maternal pain and increasing uterine relaxation. However, its impact on emergency cesarean delivery following ECV remains unclear, as prior studies have not isolated emergent cases from overall cesarean rates. Therefore, we aimed to evaluate whether neuraxial anesthesia (NA) increases emergency cesarean delivery risk and impacts ECV success and delivery mode. METHODS: This retrospect

External cephalic version outcomes with tocolysis and sedation: A 10-year retrospective cohort study.

OBJECTIVE: To evaluate the effectiveness and safety of external cephalic version (ECV) performed with tocolysis and sedation or spinal anesthesia, and to identify predictors of ECV success, complications, and delivery outcomes after successful ECV. METHODS: This 10-year cohort study included 990 pregnant women with term non-cephalic presentation at a tertiary hospital in Spain. Data were collected retrospectively (2014-2020) and prospectively (2020-2024). All ECVs followed a standardized protocol using ritodrine plus either propofol sedation or spinal anesthesia. Multivariable logistic regres

Risk Factors for Developmental Dysplasia of the Hip Before 3 Months of Age: A Meta-Analysis.

IMPORTANCE: Two meta-analyses published in 2012 found breech presentation, family history of developmental dysplasia of the hip (DDH), female sex, and primiparity to increase the risk of DDH. However, the DDH definition, reference tests, and the age of the examined children varied considerably, complicating the translation of those findings to current screening guidelines. OBJECTIVE: To evaluate the association of previously proposed risk factors with the risk of sonography-verified DDH. DATA SOURCES: A literature search strategy used MeSH (Medical Subject Heading) terms and text words assoc…

External Cephalic Version: A Retrospective Chart Review at a Canadian Tertiary Care Centre.

OBJECTIVES: The primary objective is to identify our local external cephalic version (ECV) success rate, variables associated with increased likelihood of success, and complication rates. The secondary objective is to allow obstetrical care providers to accurately counsel patients undergoing a trial of ECV. METHODS: We analysed patient charts between January 2018 and December 2022 who underwent ECV. Variables included maternal age, parity, gestational age at the time of ECV attempt, breech type, anesthetic, uterine relaxant, placental location, neonatal birthweight, and provider seniority. Ou

Neuraxial analgesia to increase the success rate of external cephalic version: a systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: External cephalic version is a medical procedure in which the fetus is externally manipulated to assume the cephalic presentation. The use of neuraxial analgesia for facilitating the version has been evaluated in several randomized clinical trials, but its potential effects are still controversial. OBJECTIVE: The objective of the study was to evaluate the effectiveness of neuraxial analgesia as an intervention to increase the success rate of external cephalic version. DATA SOURCES: Searches were performed in electronic databases with the use of a combination of text words related

[Training achieves an internal version and a total breech extraction at birth of second twin].

OBJECTIVES: To evaluate French residents in obstetrics and gynaecology's training to internal version and breech extraction during vaginal delivery of the second twin. METHODS: A national descriptive survey conducted among 1064 residents between July and October 2014. Respondents were invited by email to specify the type of theoretical and practical training they had received, their university hospital obstetrical practices and the number of vaginal internal version and breech extraction of the second twin they had seen and performed. RESULTS: Response rate was 38.7% (n=412). Regarding the t

Intrapartum management of twin gestations.

A trial of labor and vaginal delivery are usually indicated in vertex-vertex twins. For vertex-nonvertex twins, vaginal birth is preferred, with the second twin being delivered by breech extraction, unless it is significantly larger than the first. Cesarean delivery is indicated if the first twin is nonvertex and for all cases of monoamniotic or potentially viable conjoined twins. There is a limited role for trial of labor after cesarean delivery in twin gestations. In my opinion, combined vaginal-cesarean birth is the riskiest method for mother and infants and should be avoided if possible.

Reproductive capacity and outcome of pregnancy after metroplasty following the technique of Bret-Palmer partially modified in the pathological symmetric malformations of müllerian ducts.

During the period 1977-90, metroplasty for infertility and unexplained sterility was performed respectively in 19 and 6 cases of symmetric uterine malformations. The Bret-Palmer technique was used in 6 cases for partial septate uterus, while the same modified technique was performed in 11 cases of complete bicornuate uterus and in 8 cases of partial bicornuate uterus. No operative and postoperative complications were noted. With the modified Bret-Palmer operation there was the reconstruction of a single, large and regular uterine cavity. Of the 17 cases with adequate follow-up pregnancy occurr

Indications and rate of first cesarean delivery in central region's maternity and children hospital.

To investigate the indications of first (non-repeated) cesarean deliveries, to categorize those indications into absolute and relative accor … s.  Results: The leading indications were fetal distress (27.5%), non-progression of labor (22.5%), breech presentation (18%), and failed initiation of labor (4.5%). Of the 200 cases, 26.5% had absolute indications, 50% had relative indications, and 23.5% had indications that were neither absolute nor relative. Women with absolute indications had lower mean gestation …

[Particular maternal or fetal clinical conditions influencing the choice of the mode of delivery in case of previous cesarean].

OBJECTIVE: To determine how particular maternal (age>35 years, multiparity, diabetes and obesity) or fetal (breech presentation, twin pregnancy, macrosomia, prolonged pregnancy, preterm delivery) clinical conditions, potentially associated with increased risk of failed trial of labor, uterine rupture, and perinatal asphyxia, should indicate a planned cesarean delivery or could still allow a planned vaginal birth after cesarean. METHODS: The PubMed database, the Cochrane Library and the recommendations from the French and foreign obstetrical societies or colleges have been consulted. RESULTS:…

Effect of mode of delivery on outcome of very-low-birthweight infants.

The hospital survival rate was 47% in 111 inborn infants who weighed 501-1000 g and 90% in 211 who weighed 1001-1500 g. The survival rate af … he former had a significantly higher disability rate. Within the group of 72 singleton infants with breech presentation, the survival rate after caesarean birth was significantly higher than after vaginal delivery in the 1001-1500 g group but not in the 501-1000 g group. No significant difference in late outcome was found between caesarean and vaginal births in this …
