Post-transplant hypophosphatemia: Tertiary 'Hyper-Phosphatoninism'?

Hypophosphatemia is a common complication of kidney transplantation. Tertiary hyperparathyroidism has long been thought to be the etiology, but hypophosphatemia can occur despite low parathyroid hormone (PTH) levels and can persist after high PTH levels normalize. Furthermore, even in the setting of normal allograft function, hypophosphatemia, and hyperparathyroidism, calcitriol levels remain inappropriately low following transplantation, suggesting that mechanisms other than PTH contribute. Fibroblast growth factor-23 (FGF-23) induces phosphaturia, inhibits calcitriol synthesis, and accumulat…

[Management of renal osteopathy in patients with chronic dialysis].

During the past decade considerable progress has been made in the field of calcium metabolism associated with chronic renal failure. The main factor influencing the pathophysiology of calcium homeostasis is its dependence on parathyroid hormone and active vitamin D metabolites. Among several new diagnostic tools, and to establish the diagnosis of renal osteodystrophy, only the measurement of i-PTH activity has any clinical relevance. The main goal in treating renal osteodystrophy is the lowering of PTH activity by correcting hyperphosphatemia and administration of calcitriol given either orall…

Osteopetrosis. The pharmaco-physiologic basis of therapy.

Medical treatments of osteopetrosis have attempted to improve hematologic function, reduce the osteosclerotic condition, and/or improve immune function. Prednisone therapy has improved hematologic function in some patients, but has not resulted in a reduction in bone mass. Calcium deficient diets have limited further sclerosis in some patients. High-dose calcitriol and parathormone infusions have stimulated osteoclastic activity. In some patients, high-dose calcitriol has resulted in clinical improvement. Newer treatments, such as interferon gamma and macrophage colony stimulating factor, may …

A rare case of tuberculosis-induced hypercalcemia.

Laboratory investigations of hypercalcemia involve testing of various biochemical parameters such as parathyroid hormone (PTH), 25-(OH) Vitamin D (25-(OH) VitD), 1,25-(OH)2 Vitamin D3 (calcitriol) and PTH related peptide (PTHrp). We herein present an atypical case of severe hypercalcemia in a patient with rheumatoid arthritis who has been treated for years by various biological disease-modifying antirheumatic drugs (DMARDs) and suddenly presented with general state alteration, oedema and ulceration of her right ankle. We illustrate how tuberculosis (TB) can cause high calcitriol concentration …

Low-dose alfacalcidol controls secondary hyperparathyroidism in predialysis chronic kidney disease.

In the long term, secondary hyperparathyroidism (sHPT) causes severe osseous and non-osseous problems in patients with chronic kidney disease (CKD). Active vitamin D metabolites, i.e. calcitriol and alfacalcidol, have been utilized for sHPT treatment for more than 20 years. Yet, data on predialysis CKD patients are rather sparse. We monitored alfacalcidol therapy in 1,159 patients with CKD and sHPT in a 12-month multicenter observational study. Virtually all patients were in CKD stages 3-5 at study begin. The patients were recruited from 241 German nephrological centers. The mean alfacalcidol …

The roles of vitamin D in seborrhoeic keratosis: possible genetic and cellular signalling mechanisms.

OBJECTIVE: We review the relationship between vitamin D and seborrhoeic keratosis (SK). METHODS: Review literature from MEDLINE. RESULTS: Vitamin D3 supplementation can improve SK lesions. Genetics-based studies have identified the proteins that link vitamin D to SK pathology. Vitamin D also exerts its effect on SK through cell signalling mechanisms. CONCLUSION: Vitamin D plays a role in SK. Calcitriol is the best form of vitamin D for use in patients with SK because it is the active form of the vitamin D3 metabolite and its receptors are present in the skin. Further investigation of the ef…

Vitamin D and inflammation.

Calcitriol, or 1,25-dihydroxyvitamin D3 (1,25(OH)(2)D3) is a well-known endocrine regulator of calcium homeostasis. More recently, local calcitriol production by immune cells was shown to exert autocrine or paracrine immunomodulating effects. Immune cells that produce calcitriol also express the vitamin D receptor (VDR) and the enzymes needed to metabolize vitamin D3 (1α-, 25-, and 24-hydroxylases). Studies of animal models and cell cultures showed both direct and indirect immunomodulating effects involving the T cells, B cells, and antigen-presenting cells (dendritic cells and macrophages) an…

Effectiveness of 1,25-dihydroxyvitamin D supplementation on blood pressure reduction in a pseudohypoparathyroidism patient with high renin activity.

A 42-year-old man had biochemical and somatic abnormalities compatible with pseudohypoparathyroidism type I (PsHP) and also had high plasma renin activity (PRA). After 1,25-dihydroxyvitamin D (calcitriol) supplementation the systolic/diastolic blood pressure, assessed by 24-hour non-invasive ambulatory blood pressure monitoring, was reduced from 145/96 mm Hg to 128/85 mm Hg with normalization of the serum calcium level and its related hormones, as well as decreased PRA. Calcitriol supplementation successfully reduced the blood pressure in this patient with PsHP and a high PRA, suggesting that …

Fibroblast growth factor-23 is regulated by 1alpha,25-dihydroxyvitamin D.

UNLABELLED: Serum FGF-23 regulation was studied in patients with hypoparathyroidism or pseudohypoparathyroidism treated with calcitriol. Serum FGF-23 levels changed in parallel in response to changes in serum 1,25-D, suggesting that FGF-23 may be regulated by 1,25-D. In addition, the phosphaturic effect of FGF-23 may be diminished in the absence of PTH action on the kidney. INTRODUCTION: Fibroblast growth factor (FGF)-23 is a recently described hormone that has been shown to be involved in the regulation of phosphate and vitamin D metabolism. The physiologic role of FGF-23 in mineral metaboli…

Integrated transcriptomic and proteomic analyses for the characterization of parathyroid oxyphil cells in uremic patients.

Chief cells are the predominant cells in parathyroid glands of healthy adults; however, parathyroid oxyphil cells, whose function is unknown, increase dramatically in patients with secondary hyperparathyroidism (SHPT). Calcitriol and calcimimetics are the most powerful treatments for SHPT, while the mechanisms leading to calcitriol or calcimimetic resistance in oxyphil cell-predominant SHPT are unknown. Here we used transcriptomic and proteomic techniques to characterize oxyphil cells by comparing the differences between chief and oxyphil cell nodules of parathyroid glands in uremic patients. …

[Calcium homeostasis in pregnancy and lactation].

The daily required dose of calcium is greatly increased during pregnancy. It is caused by the transition of about 30 g to the fetus and by enhanced urinary excretion. The normal calcium homeostasis is maintained by parathormone (PTH), calcitriol and PTH-related peptide (PTHrP). The concentration of calcitriol is augmented as well as the absorptive surface of the gut. PTHrP is involved in the regulation of placental calcium transport and also in lactation and initiation of labor. Bone turnover is enhanced during pregnancy and sometimes may evoke osteoporosis, especially in bones which form the …
