Administration of peripheral parenteral nutrition: a prospective study comparing rotation of venous access sites with ultrafine cannulas.

Peripheral veins have been used successfully for patients requiring short- to medium-term total parenteral nutrition. This study prospectively compares two methods of peripheral parenteral nutrition (PPN). Forty-six patients requiring parenteral nutrition (PN) were identified prospectively. Fifty courses of PPN were prescribed using a standardized PPN formula of 9.3 g nitrogen, 1400 kCal, 2500 ml (KABI II, Pharmacia). Patients were randomized to receive PPN via 23G, 15 cm flexane catheters (Nutriline) inserted into an antecubital vein which remained in-situ with a continuous infusion over 24 h

Influence of the compliance of the pump housing and cannulas of a paracorporeal pneumatic ventricular assist device on transient pressure characteristics.

The dependence of transient pressure characteristics of a ventricular assist device (VAD) on the compliance of its housing and cannulas was investigated in a mock circulation. The peak rate of change of pressure (dP/dtmax) values was greater in the cannulas than other compartments and was associated with valve closure-induced pressure oscillations. When cannula compliance was increased from 0.0057 to 0.0129 cm3/mm Hg, these values decreased by approximately 20%, and outflow cannula pressure oscillation frequency decreased from 17.5 Hz by 35%. This trend was also apparent in the inflow. A VAD h…

Respiratory Support in Bronchiolitis: A Narrative Review of the Evidence for and Against High-Flow Nasal Cannula (HFNC) Use.

Bronchiolitis is a leading cause of pediatric hospitalization, and the use of high-flow nasal cannula (HFNC) as respiratory support has increased substantially over the past decade despite ongoing uncertainty about its clinical benefits. This narrative review evaluates the evidence supporting and questioning HFNC use in infants and young children with bronchiolitis. Early observational studies suggested reductions in intubation rates and improved clinical parameters with HFNC; however, more recent randomized controlled trials show no consistent benefit in length of stay, duration of oxygen the…

High-flow nasal cannula for hypoxia in the post-anesthetic recovery unit: A systematic review and meta-analysis.

BACKGROUND: Earlier meta-analyses demonstrated conflicting results on whether high-flow nasal cannula (HFNC) reduces rate of intubation in the postoperative period when compared to conventional oxygen therapy (COT). Since then, newer studies have come out, so an updated comparison of these two modalities is warranted. AIM: To determine whether HFNC reduces the rate of reintubation compared to COT. METHODS: We conducted a random-effects meta-analysis, searching databases (MEDLINE, EMBASE, Web of Science) to identify randomized controlled trials comparing HFNC with COT on postoperative patient…

Use of a Novel Steerable Tip Suction Cannula in Large and Giant Pituitary Adenomas: A Cadaveric Feasibility Study.

ObjectiveThe endoscopic transsphenoidal approach is commonly used for sellar and suprasellar pathologies. However, reaching above the diaphragma sella, especially for posterosuperior and retrocavernous orientation, still poses some challenges. We designed and developed a steerable tip suction cannula (STSC) that has distinct leverage for endoscopic resection of such pathologies.MethodsThe entire suction cannula is made of stainless steel. The instrument consists of a handle, vacuum tube, suction tip bed, suction tip, finger knuckle, wire path, countersunk headpin, and isolated steel wire. The …

The Effectiveness of High-Flow Nasal Cannula in Respiratory Distress in the Pediatric Population of Bundelkhand Region: An Observational Study.

INTRODUCTION: Respiratory distress is the most frequently encountered cause of admission in the pediatric intensive care unit (PICU) requiring immediate medical attention. Standard oxygen therapy is provided via traditional nasal cannula, oxygen mask, or oxygen hood, whose failure warrants the need for invasive mechanical ventilation. High-flow nasal cannula (HFNC) is increasingly being used as a form of noninvasive respiratory support with fewer side effects and has been shown to reduce the requirement for intubation. OBJECTIVES: To evaluate the effectiveness of HFNC in various indications o…

High-flow nasal cannulas versus standard oxygen therapy for moderate bronchiolitis: a quasi-experimental analysis.

BACKGROUND: In the last decades none of the medical therapies investigated have shown clear efficacy in the treatment of bronchiolitis, and literature agrees on a general de-implementation of pharmacological therapies, recognizing an effective role only to nutritional support and oxygen therapy. High-flow nasal cannulas (HFNC) has become increasingly popular in the last decade, despite its lack of clear efficacy. Recent randomized controlled trials (RCT) comparing standard oxygen therapy (SOT) and HFNC did not demonstrate significant benefit of HFNC. To acquire more clinical data on HFNC effic…

Comparison of high flow nasal cannula therapy to nasal oxygen as a treatment for obstructive sleep apnea in infants.

INTRODUCTION: Obstructive sleep apnea (OSA) in infants is treated with low flow oxygen via nasal cannula (NC), CPAP (continous positive airway pressure), or surgery. Literature supports the use of high flow NC (HFNC) in children in the outpatient setting, however there is limited data on the use of HFNC in infants. OBJECTIVE: The purpose of this study was to compare HFNC and low-flow oxygen as treatments for OSA in infants. METHODS: A prospective pilot study was performed at two institutions. Infants with primarily OSA underwent a 3-4 h sleep study with HFNC titration at 6-14 lpm for OSA, fo…

The impact of blood sampling technique, including the use of peripheral intravenous cannula, on haemolysis rates: A cohort study.

AIMS: To explore the relationship between blood sampling techniques and haemolysis. BACKGROUND: Haemolysis rates of blood samples have been thought to be influenced by the method of collection. There is a lack of research evidence available to clearly show the comparative risk of haemolysis across different blood sampling methods, including venepuncture and use of peripheral intravenous cannulas. DESIGN: A prospective cohort study. Reporting followed the STROBE checklist. METHODS: A trained observer was used to record blood sampling techniques over a 10-week period between April and June 20…

SpO2/FiO2 as a predictor of high flow nasal cannula outcomes in children with acute hypoxemic respiratory failure.

The high-flow nasal cannula (HFNC) is a useful treatment modality for acute hypoxemic respiratory failure (AHRF) in children. We compared the ability of the oxygen saturation to fraction of inspired oxygen ratio (S/F) and arterial oxygen partial pressure to fraction of inspired oxygen ratio (P/F) to predict HFNC outcomes in children with AHRF. This study included children treated with HFNC due to AHRF from April 2013 to March 2019 at the Severance Children's Hospital. HFNC failure was defined as the need for mechanical ventilation. Trends of S/F and P/F during HFNC were analyzed. To predict HF…

Development and Preclinical Trials of a Novel Steerable Cannula for 360° Arthroscopic Capsular Release in Minimally Invasive Surgery.

Many of the surgical devices for minimally invasive surgery have struggled to navigate in tight spaces and to safely address to surgical sites. This research demonstrates a scalable steerable cannula for 360° arthroscopic capsular release (ACR) of the glenohumeral joint. The steerable cannula mainly consists of the pre-curved inner tube of nitinol, the straight outer tube of stainless steel, and the controller part. Using anisotropic patterns and heat treatment, the inner tube has a high degree of curvature. The mechanism controls the distal end with the two degrees of freedom. The inner tube …

Dermabond as a Novel Solution for Needle-Hole Repair in Arterial Cannula while on Extracorporeal Membrane Oxygenation Therapy.

The annual incidence of extracorporeal membrane oxygenation (ECMO) for adult patients is increasing. Indications may vary from acute lung injury, ECMO-assisted cardiopulmonary resuscitation, to cardiac failure with an inability to wean from cardiopulmonary bypass. Complications may happen to the ECMO circuit, including cannula leaks from holes, cracks, or other damage, during the support period. Here, we present a novel solution for repairing a bleeding structural leak in the side of an arterial cannula. Dermabond was used to seal a small leak in the cannula likely caused by an earlier acciden…

Position of draining venous cannula in extracorporeal membrane oxygenation for respiratory and respiratory/circulatory support in adult patients.

Extracorporeal membrane oxygenation (ECMO) is used in critically ill patients with severe pulmonary and/or cardiac failure. Blood is drained from the venous system and pumped through a membrane oxygenator where it is oxygenated. For pulmonary support, the blood is returned to the patient via a vein (veno-venous ECMO) and for pulmonary/circulatory support it is returned via an artery (veno-arterial ECMO).Veno-venous ECMO can be performed either with a single dual-lumen cannula or with two separate single-lumen cannulas. If the latter is chosen, flow direction can either be from the inferior cav…

Does the RAM Cannula Provide Continuous Positive Airway Pressure as Effectively as the Hudson Prongs in Preterm Neonates?

OBJECTIVE: To compare the level of continuous positive airway pressure (CPAP) delivered by the RAM cannula system (Neotech, Valencia, CA) with that delivered by a traditional CPAP nasal delivery interface (Hudson prongs; Hudson-RCI, Temecula, CA) in preterm infants with respiratory distress. METHODS: This was a crossover intervention study in a convenience sample of preterm infants with respiratory distress requiring treatment with CPAP. We measured the mean intraoral (pharyngeal) pressure, which approximates the applied CPAP level, using both the RAM cannula and Hudson prongs. The primary ou…
