Combined trauma and toxic inhalation in war and disaster medicine: alveolar-capillary barrier failure and respiratory countermeasures.

Severe trauma induces systemic inflammatory responses that predispose the lung to secondary injury. Acute respiratory distress syndrome (ARDS) remains a major cause of morbidity and mortality following severe trauma, particularly in military and disaster settings where inhalation exposure to toxic combustion products frequently accompanies physical injury. Combustion-derived particles, irritant gases, and complex aerosols generated by explosions or fires may amplify trauma-induced pulmonary inflammation and accelerate alveolar-capillary barrier failure. This review highlights the interactions

Vascular endothelial growth factor mRNA in pericytes is upregulated by phorbol myristate acetate.

Increased microvascular permeability, which occurs in conditions such as the adult respiratory distress syndrome and diabetes mellitus, is related to physicochemical alterations in the microvascular barrier. We postulate that, in part, capillary pericytes affect microvascular permeability via production of a vasoactive cytokine, viz, vascular endothelial growth factor (VEGF), also known as vascular permeability factor. The goal of the present study was to evaluate the effects of phorbol myristate acetate (PMA), a substance known to produce nonhydrostatic pulmonary edema in intact animals, on V

Diagnostic biomarkers and miRNAs in prognosis of acute respiratory distress syndrome.

Acute respiratory distress syndrome (ARDS) is a disease of the lung and/or extrapulmonary system characterized by acute, progressive breathing difficulty and refractory hypoxemia. After years of revision, the 2012 International Expert Conference developed a new diagnostic standard for ARDS, known as the Berlin definition, which provides good guidance on how to define and judge the disease in clinical practice. Despite the establishment of diagnostic standards and treatment improvements, ARDS mortality rate still remains high. The primary reason is that the pathophysiology has not been fully el

Vascular permeability factor/vascular endothelial growth factor in nasal polyps.

Although the aetiology of polyps is still, for the most part, unknown, the disease is known to involve tissue oedema. Vascular permeability/vascular endothelial growth factor (VPF/VEGF) is a major inducer of angiogenesis and capillary permeability. This study investigated VPF/VEGF expression in biopsies of nasal polyps from 39 patients and in healthy nasal mucosa from 10 patients by immunohistochemical staining. Staining for VPF/VEGF in the mucosal surface and in the glandular epithelium of nasal polyps was weaker than in normal controls. In two patients, strong staining for VPF/VEGF was found…

Endothelin-1 induces endothelial barrier failure in the cat hindlimb.

Our purpose was to see whether endothelin- (ET) 1 could produce a change in the endothelial membrane barrier to protein in skeletal muscle. Previous studies in other tissues have suggested that ET-1 affects this barrier, but the measurement methods used could not exclude vascular protein extravasation due to microvascular pressure changes or the effects of changes in perfused capillary surface area. We measured the protein sieving coefficient, a specific measure of the permeability of the membrane to protein, in the isolated, perfused cat hindlimb preparation. The integral-mass balance method

Hantavirus pulmonary syndrome: radiographic findings in 16 patients.

PURPOSE: To characterize chest radiographic features of Hantavirus pulmonary syndrome. MATERIALS AND METHODS: Initial and follow-up chest radiographs from 16 patients with confirmed Hantavirus pulmonary syndrome were reviewed for radiographic findings of either cardiogenic pulmonary edema or pulmonary edema due to increased permeability of the alveolar capillary membranes. RESULTS: Findings indicative of interstitial edema were present more frequently (14 [88%] of 16 patients) than is typically seen in adult respiratory distress syndrome (5%). Alveolar flooding subsequently developed in 11 (

Perifoveal vascular leakage and macular oedema after intracapsular cataract extraction.

Perifoveal capillary leakage of fluorescein was demonstrated in 60 per cent of 50 eyes when angiography was performed two weeks after cataract extraction. Repeat angiography six weeks postoperatively in 17 eyes demonstrated persistence of already established leakage in 11 of 12 eyes and no new leakage in five eyes previously negative. Cystoid macular oedema with visual acuity of less than 20/40 six weeks postoperatively occurred in five eyes (10 per cent). Eyes of patients with vascular disease and those patients of 60 years or older were found to have altered vascular permeability significant

Prostaglandin inhibitors in the treatment of nephrotic syndrome.

The management of nephrotic syndrome resistant to corticosteroid and cytotoxic therapy is unclear. In such patients, prostaglandin inhibitors can reduce proteinuria. Mechanisms may include a reduction in transcapillary hydraulic pressure and a decrease in capillary wall permeability. The antiproteinuric effect of these agents is enhanced by volume depletion induced by sodium restriction and thiazide diuretics. Complications may include aggravation of edema, hemodynamic renal failure, hyperkalemia, and drug nephrotoxicity. Although a reduction in protein excretion may improve the clinical statu

Pharmacology of scorpion (Lychas laevifrons Pock) venom with special reference to its neuromuscular activity.

L. laevifrons venom caused irreversible blockade of electrically induced twitch responses on phrenic nerve diaphragm and chick biventer cervicis preparation. The venom lowered cat blood pressure, caused a brief cardiac arrest and increased cutaneous capillary permeability. It contracted several smooth muscle preparations. The quick contraction produced on guinea pig ileum was partly antagonized by mepyramine and completely by methysergide. The residual slow contraction was antagonized by SC 19220, a prostaglandin blocker. Haemolysis was not produced by the venom on human RBC. LD50 of crude ven…

[Unilateral pulmonary oedema after re-expansion (author's transl)].

The authors report three cases of unilateral pulmonary oedema following the re-expansion of a spontaneous pneumothorax. The importance of the duration of the pneumothorax and the use of too negative pressure while re-expanding the lung were both underlined. The different physiopathological hypotheses responsible for the appearance of pulmonary oedema were discussed, notably altered mechanical properties of the lung and alveolar-capillary permeability. The numerous precautions to take to avoid the appearance of oedema, as well as the therapeutic measures to adopt for severe pulmonary oedema wer…

Choriocapillary changes and hyperfluorescence on indocyanine green angiography.

BACKGROUND: The purpose of this study is to examine the relationship between pathological changes of the choriocapillaris (CC) and the finding of bright fluorescence on indocyanine green (ICG) angiography. METHODS: An animal model was used, with chorioretinal lesions produced by injecting sodium iodate 3 h, 24 h, 7 days or 14 days previously. The ICG angiographic findings were compared with histology. RESULTS: Three hours after injection, many spots of bright fluorescence were scattered at the posterior pole. Histologically, variable changes in CC endothelial cells were observed. In some reg

[Effective therapy of acute cardiovascular and respiratory failure developed after removal of cervical spinal cord tumor in a child].

Development of hemodynamic and respiratory disturbances in cervical spinal cord injury is a known phenomenon. Its degree depends on severity of injury. Hemodynamic impairments include bradycardia and other arrhythmias, arterial hypotension due to decreased vascular tone and cardiac output and they may result in shock and asystolia. Pathogenesis of respiratory insufficiency is explained by decrease of vital and residual pulmonary volume, dysregulation of respiratory function, development of athelectases, changes in permeability of pulmonary capillaries, pneumonia. This leads to respiratory fail

Blood-brain barrier breakdown is an early biomarker of human cognitive dysfunction.

Vascular contributions to cognitive impairment are increasingly recognized1-5 as shown by neuropathological6,7, neuroimaging4,8-11, and cerebrospinal fluid biomarker4,12 studies. Moreover, small vessel disease of the brain has been estimated to contribute to approximately 50% of all dementias worldwide, including those caused by Alzheimer's disease (AD)3,4,13. Vascular changes in AD have been typically attributed to the vasoactive and/or vasculotoxic effects of amyloid-β (Aβ)3,11,14, and more recently tau15. Animal studies suggest that Aβ and tau lead to blood vessel abnormalities and blood-br

Modeling Impact of Crack Density on the Performance of the Microporous Layer in PEMFCs.

The crack density (CD) of the microporous layer (MPL) has a positive impact on the water management and electrochemical performance for PEMFCs. As there is still a lack of strategies for designing optimal CD under varying relative humidity (RH) for enhancing MPL performance, a numerical model is proposed with integration of the capillary pressure weakening coefficient, and the effects of cracks on water-gas transport, the capillary pressure drop, and water flooding are quantitatively considered. Furthermore, experiments are set up to adopt a kind of thickener (methylcellulose) to adjust the sl

Localized pulmonary vascular changes in a mouse model of subarachnoid hemorrhage created by combining filament perforation and blood injection.

INTRODUCTION: Subarachnoid hemorrhage (SAH) results in neurogenic pulmonary edema (NPE), a condition with a high mortality rate arising from increased hydrostatic pressure and vascular permeability. Two possible mechanisms of NPE are increased hydrostatic pressure and increased vascular permeability, and it is possible that increased permeability of capillaries in the lungs may contribute to the exacerbation of NPE. Recent research has highlighted the importance of the glycocalyx, a gel-like layer that lines blood vessels, in regulating vascular permeability in various diseases. However, its r

Perivascular Macrophages Limit Permeability.

OBJECTIVE: Perivascular cells, including pericytes, macrophages, smooth muscle cells, and other specialized cell types, like podocytes, participate in various aspects of vascular function. However, aside from the well-established roles of smooth muscle cells and pericytes, the contributions of other vascular-associated cells are poorly understood. Our goal was to ascertain the function of perivascular macrophages in adult tissues under nonpathological conditions. APPROACH AND RESULTS: We combined confocal microscopy, in vivo cell depletion, and in vitro assays to investigate the contribution

Total CA activity in isolated perfused guinea pig lung by 18O-exchange method.

The rate of exchange of 18O between alveolar CO2 and lung water was measured in isolated perfused guinea pig lungs to quantify carbonic anhydrase (CA) activity. The average lung CA activity, with a reaction velocity constant of 5.32 +/- 2.2 s-1, is sufficient to accelerate CO2 reactions in lung water by two orders of magnitude over the uncatalyzed rate at 22 degrees C and a PCO2 of 40 Torr. Three sulfonamide inhibitors of CA with different human erythrocyte membrane permeabilities were used to determine the availability of the enzyme to the perfusate. Ethoxzolamide, the most permeable at 0.1 m
