Neck and brain transitory vascular compression causing neurological complications. Results of surgical treatment on 1,300 patients.

In this brief article we describe the role of compression of the vertebral subclavian arteries, internal mammary, internal carotid arteries, … tosis in the lenticular nucleus. This compression is caused by the anterior scalene muscles and the cervical ribs at the level of the vertebrae C6-C7; by the sternocleidomastoid at the level of the cervical atlas; and coiling and kinking of the vertebral, basilar and the internal carotid arteries. The decreased blood supply to the cerebellum and basal ganglia …

[Phalanx-like osseous structure posterior to the cervical vertebrae: a hitherto unreported location for a rare anomaly].

Several types of abnormalities of the vertebral column have been reported. Pelvic rib or pelvic digit is a rare congenital anomaly of the spine. A 58-year-old male patient presented with a complaint of a firm mass in the neck. He had no history of trauma and had been aware of the mass for years. Physical examination showed a painless and immobile mass in the neck, measuring 4 x 1 cm. About 10 to 15 degrees of restriction was noted in neck movements, with slight pain. A plain radiograph of the cervical region showed two bony formations resembling a coccygeal segment or a phalanx, posterior to t

[Thoracic vertebral nerve].

When studying phylogenetic transformations of the sympathetic trunk, a theoretical possibility has been substantiated that in Mammalia a caudal continuation of the vertebral nerve in the thoracic area can exist. By means of some embryological and anatomical methods, in man and in certain animals (cat, dog), together with the cervical, the thoracic vertebral nerve has been revealed. The latter has an appearance of a plexus situating on the rudimentary thoracic vertebral artery wall and is formed at the expense of the multisegmentary distribution of the gray connective branches fibers. Adolescent athletes can get back in the game after surgery for thoracic outlet syndrome.

OBJECTIVE: We compared the functional outcomes among adolescent athletes with venous thoracic outlet syndrome (VTOS) and neurogenic TOS (NTO … scalenectomy was performed for 100% of the VTOS patients and 94.4% of the NTOS patients undergoing cervical rib resection (2.8%) or scalenectomy alone (2.8%). Additionally, 11.1% of the NTOS patients had undergone combined first rib resection and cervical rib resection. For the VTOS patients, postoperative venography showed patent subclavian veins in 27.8%. I …

[Neurological Signs and Symptoms of True Neurogenic Thoracic Outlet Syndrome].

Thoracic outlet syndrome (TOS) is a well-known disorder, but many aspects of its pathology, including its definition, has been disputed. True neurogenic TOS (TN-TOS) is a rare but well-defined clinical condition. TN-TOS results from the compression of the C8/T1 roots (dominant for the T1 root) or the proximal lower trunk of the brachial plexus by a fibrous band. The band extends from the first rib to either the tip of an elongated C7 transverse process or a rudimentary cervical rib. The most common presenting symptoms of TN-TOS are insidious-onset atrophy and weakness of the intrinsic hand mus…

Extranodal presentation of Hodgkin's lymphoma of the sternum: A case report and review of the literature.

Hodgkin's lymphoma (HL) is typically a systemic disease with involvement of the cervical, supraclavicular and mediastinal lymph nodes. An extra-nodal presentation of HL is unusual and seldom encountered in the clinic. The most common sites of presentation for extra-nodal extension are the spleen, liver, lungs, bones and marrow. The bones that are frequently involved are the vertebrae, pelvis, ribs and femur. Involvement of the sternum has occasionally been reported. The current study presents an unusual case on the extra-nodal presentation of HL of the sternum arising in a 25-year-old woman, a

Thoracic outlet syndromes.

TOS may arise in a variety of circumstances including trauma, but it may also be seen as a result of postural abnormalities that may be induced by different factors. Cervical ribs and other congenital anomalies are not necessary prerequisites for either the causation or diagnosis of TOS, although they are more common in the TOS population. More important than diagnostic tests are the history and physical examination of the patient. Double crush syndrome can complicate the diagnosis and treatment; however, proper selection of candidates for surgery can produce excellent and good results in a hi…

Hoxa-5 acts in segmented somites to regulate cervical vertebral morphology.

The vertebrate axial skeleton (vertebral column and ribs) is derived from embryonic structures called somites. Mechanisms of somite formation and patterning are largely conserved along the length of the body axis, but segments acquire different morphologies in part through the action of Hox transcription factors. Although Hox genes' roles in axial skeletal patterning have been extensively characterized, it is still not well understood how they interact with somite patterning pathways to regulate different vertebral morphologies. Here, we investigated the role of Hoxa-5 in after somite segmenta

Mosaic tetrasomy 9p in a girl with multiple congenital anomalies: cytogenetic and molecular-cytogenetic studies.

We report on a 16-year-old girl with tetrasomy 9p mosaicism. Clinical investigations disclosed a malformation syndrome with craniofacial abnormalities, dysplasia of the right clavicle, short neck with cervical ribs, patella dislocation, Dandy-Walker malformation, mental retardation and blindness. Karyotype analysis of blood lymphocytes indicated an additional marker in the size of a C-group chromosome with a large heterochromatic block in 88% of the investigated metaphases. The origin and structure of this additional marker could not be determined by chromosome banding. Application of fluoresc…

["True neurologic thoracic outlet syndrome" -- anatomical features and electrophysiological long-term follow-up of lateral thenar atrophy].

Atrophies of the intrinsic muscles of the hand are considered to be a typical symptom of the "true neurologic" form of thoracic outlet syndrome (TOS). The classical form of this entity was described as early as 1970, consisting of a cervical rib or a prolonged transverse process of C7, complete with a fibrous band to the first thoracic rib, resulting in atrophy of the intrinsic muscles of the hand. All our TOS patients presenting with such atrophy displayed anatomical findings consistent with this definition. Based on this observation, the TOS classification currently in clinical use, which di…

Upper thoracic spinal cord injury without vertebral bony lesion: a report of two cases.

STUDY DESIGN: Case report. OBJECTIVES: To describe a rarely reported type of upper thoracic spinal cord injury without vertebral bony lesion in two cases with multiple trauma. SUMMARY OF BACKGROUND DATA: Because it is supported by the stiffness of the rib cage, the upper thoracic spine has greater stability than the cervical and lumbar regions, and thus its fracture or fracture dislocation is less frequent. Nevertheless, when fracture or fracture dislocation of upper thoracic spine occurs, spinal cord involvement and severe concomitant injuries are frequently associated. METHODS: Two cases

Solitary, spontaneous cervical and large bone fractures in aluminum osteodystrophy.

Aluminum-induced bone disease in uremic patients receiving dialysis was first described a little more than 10 years ago. The epidemic form of the disease was seen in centers where there was a high aluminum content in the water dialysate. Although this problem has been corrected, sporadic forms of the disease continue to be noted in dialyzed and nondialyzed patients. Multiple fractures are a radiological feature of aluminum-related bone disease. Fractures of the ribs and hips and vertebral crush fractures are the usual manifestations. We present four patients whose nontraumatic fractures involv

Homologies of the longissimus, iliocostalis, and hypaxial muscles in the anterior presacral region of extant diapsida.

Homologies of muscles of the m. longissimus and m. iliocostalis groups in the dorsal and cervical regions, as well as those of the subvertebral muscles and mm. intercostales externi that continue from the dorsal into the cervical regions, in extant Diapsida are proposed based on detailed dissections and published accounts of lepidosaurs, crocodylians, and birds. The morphology of tendons and innervation patterns suggest that the avian "m. iliocostalis" in the dorsal region include the homologs of both m. longissimus and m. iliocostalis in non-avian diapsids. The conserved nature of the morphol

[Anatomy and clinical application of transcervicothoracic polyphyletic blood supply skin flap].

Transcervicothoracic skin flap was used to repair the faciocervical scars left after burns in 56 cases. Anatomy and clinical application proved that the transverse cervical artery branches out a constant skin artery before it enters the trapezius muscle and this artery, after crossing the clavicle, is divided into two branches, one extending outwards, and the other downwards. The internal thoracic artery branched out an intercostal perforator on the spot 1 cm to the parasternum. The perforator extended along the intercostal plane and belonged to the direct skin artery. The rate for perforation

Spinal cord injury without radiological abnormality in adult thoracic spinal trauma.

INTRODUCTION: Spinal cord injury without radiological abnormality (SCIWORA) is a rare entity and usually involves the cervical spine. Thoracic spine involvement is very rare due to the stability provided by the rib cage. The mechanisms of injury and pathophysiology are still debatable. CASE PRESENTATION: We present a case of an adult male who had road traffic accident and presented with paraplegia. The initial radiological investigations carried out in the emergency department were reported to be normal, however, subsequent magnetic resonance imaging revealed spinal cord contusion without ver

Diving injuries: a preventable catastrophe.

During a 5-year period from January 1987 through January 1992, 58 patients were admitted to the Allegheny General Hospital trauma center for non-scuba, non-suicidal diving injuries. There were 46 men and 12 women (mean age, 23 years). Forty-five patients were injured in swimming pools. Twenty-two patients had blood alcohol levels > 100 mg/dL. Cervical spine injury was the most common pathologic entity encountered in this group of patients. Closed head injury, pelvic fracture, thoracic vertebral fracture, and rib fractures were other injuries identified. Some patients had multiple organ failure

Two cases of nevoid basal cell carcinoma syndrome.

Two cases of nevoid basal cell carcinoma syndrome were reported with a review of pertinent literature. The first case was a 59-year-old man, whose autopsy was warranted. Signs and symptoms manifested in this case were basal cell carcinoma, generalized multiple nevi, multiple cysts in the jaws and long bones, pits in the palm and sole, frontal and occipital bossing, ossification of the falx cerebri, a bifid rib, renal fibroma and a patent foramen ovale. The family history revealed a hereditary predisposition as to the syndrome. The patient in the second case included basal cell carcinoma, multi

Anti-directional segmental motion during flexion and extension in the adolescent thoracolumbar spine: a normative study of 166 participants.

PURPOSE: Anti-directional segmental motion has been documented in the cervical spine but never quantified in the thoracolumbar region. We aimed to determine its prevalence and determinants in adolescents and test its independence from coronal deviation. METHODS: Idiag M360 data from 166 adolescents (103 males, 63 females; 14.4 ± 0.9 years) were retrospectively analysed. Segmental angles (Th1/2–L5/S1) in upright, flexion and extension were recorded. Anti-directional motion was defined as segmental movement opposite to the intended trunk direction. Bonferroni-corrected correlations, kyphosis ter
