Incident Coronary Heart Disease After Preeclampsia: Role of Reduced Fetal Growth, Preterm Delivery, and Parity.

BACKGROUND: Preeclampsia is a severe pregnancy disorder often complicated by reduced fetal growth or preterm delivery and is associated with long-term maternal morbidity and mortality. We aimed to assess the association between preeclampsia phenotypes and risk of subsequent coronary heart disease and maternal cardiovascular mortality. METHODS AND RESULTS: Women aged 16 to 49 years who gave birth during 1980-2002 and registered in the Medical Birth Registry of Norway were followed prospectively (1-29 years) for an incident major coronary event and mortality through linkage with the Cardiovascu

Predicting the distribution of under-five deaths by cause in countries without adequate vital registration systems.

BACKGROUND: The absence of complete vital registration and atypical nature of the locations where epidemiological studies of cause of death in children are conducted make it difficult to know the true distribution of child deaths by cause in developing countries. A credible method is needed for generating valid estimates of this distribution for countries without adequate vital registration systems. METHODS: A systematic review was undertaken of all studies published since 1980 reporting under-5 mortality by cause. Causes of death were standardized across studies, and information was collecte

Prognostic value of von-Willebrand factor in patients with liver cirrhosis and its relation to other prognostic indicators.

BACKGROUND: Von-Willebrand factor (vWF) disposes certain prognostic value in patients with liver cirrhosis, but its relation to other prognostic indicators has not been fully investigated. AIM: To analyze the relation between vWF and other prognostic indicators in cirrhotic patients and to evaluate its prognostic value for mortality. METHODS: This analytic prospective study was carried out in a tertiary center and initially enrolled 71 patients with liver cirrhosis and portal hypertension. It analyzed the relation between vWF and the stage of the disease and several inflammatory and prognost

Left Lateral Sector In Situ Split Liver Transplantation Technique: Step-by-Step Video Demonstration.

The split liver technique enables transplanting 2 recipients with one single graft (typically an adult-child pair). It facilitates small recipients' access to liver transplantation and reduces mortality on the waiting list. However, splitting is technically demanding and may increase peri- and postoperative complications. To be able to obtain comparable outcomes to a full graft liver transplantation, careful donor-recipients selection, experienced surgeons, and logistic planning are paramount. The video shows an in situ split liver procedure from a 32-year-old brain stem death donor to generat

Childhood mortality in a cohort treated with mass azithromycin for trachoma.

BACKGROUND: Mass azithromycin distributions are used to clear ocular strains of chlamydia that cause trachoma, but treatments may also affect respiratory infections, diarrhea, and malaria. Here, we monitor a large cohort in which almost 90% of individuals received azithromycin. We assess whether receiving treatment is associated with reduced all-cause and infectious childhood mortality. METHODS: As part of a clinical trial for trachoma, a census was conducted in 24 communities in rural Ethiopia. All individuals ≥1 year of age were eligible for single-dose oral azithromycin, although antibioti

Trends in pediatric mortality from food choking in children with intellectual disability during 1991-2005.

Unintentional injury is one of the leading causes for mortality in childhood. Choking or the interruption of respiration by internal obstruction of the airway, usually by food or small toys, is such a case. The present study was conducted to examine choking-related deaths from 1991-2005 among children with intellectual disability younger than 18 years of age living in residential care centers in Israel. The data for this study were obtained from the Office of the Medical Director (OMD), Division for Mental Retardation (DMR), Ministry of Social Affairs and Social Services. Every case of death i

Global trends in adolescent health.

Increasingly, morbidity and mortality trends for young people in developing nations are paralleling those in the industrialized world. As infectious causes of mortality diminish, unintentional injuries, suicide, homicide, war, and maternal mortality represent the primary causes of death in the second decade of life for most nations where data are maintained. As developing nations increasingly place priority on the education of their youth, early marriage and precocious child rearing are discouraged, and other problems, such as out-of-wedlock childbirth and illicit abortions, emerge. Problems s

Bacterial and viral pneumonia: New insights from the Drakenstein Child Health Study.

Pneumonia remains the major cause of childhood morbidity and mortality globally, despite improved preventative strategies. Increasing evidence also links early life pneumonia with the development of chronic non-communicable respiratory diseases into childhood and adulthood. The Drakenstein Child Health Study is a unique, multidisciplinary, South African birth cohort, investigating the incidence, risk factors, aetiology and long term impact of early life pneumonia on child health. The study provides novel data showing a high incidence of pneumonia despite excellent immunisation coverage; that p

Child health implications of transitioning from charcoal to electric cooking in Uganda: a qualitative stakeholder study.

BACKGROUND: Household air pollution and cooking-related injuries from charcoal use remain major contributors to preventable child morbidity and mortality in low- and middle-income countries, including Uganda. Electric cooking (e-cooking) could reduce children's exposure to these risks, yet adoption remains limited and the policy conditions shaping this transition are poorly understood from a child health perspective. OBJECTIVE: To explore policy stakeholder perspectives on transitioning from charcoal to e-cooking in Uganda and to examine perceived implications for child health related to hous

Neutrophil-lymphocyte ratio is associated with worse outcomes in patients with cirrhosis: insights from the PRO-LIVER Registry.

BACKGROUND: Liver cirrhosis (LC) is a leading global cause of morbidity and mortality, with inflammation playing a key role in disease progression and clinical complications of LC. The Neutrophil/Lymphocyte Ratio (NLR), a readily available marker of systemic inflammation, has been linked to short-term adverse outcomes in LC, but data on long-term follow-up are limited. This study aimed to investigate the relationship between NLR and long-term all-cause mortality in an unselected cohort of LC patients. METHODS: Data were gathered from the Italian multicenter observational study "PRO-LIVER". Pa

Esophageal variceal bleeding secondary to portal hypertension: endoscopic sclerotherapy as the first-step treatment.

BACKGROUND: Variceal bleeding from the esophagus is an important cause of mortality and morbidity in children with portal hypertension (PHT). PATIENTS AND METHODS: A series of 69 PHT cases (41 intrahepatic, 28 extrahepatic) have been evaluated in our department since 1990. According to the Child-Pugh classification, 49 cases were in class A, 16 cases were in class B, and 4 cases were in class C at admission. In our protocol, endoscopic sclerotherapy is performed in all patients, and the diagnosis is achieved directly by diagnostic laparoscopy and fine-needle liver biopsy. The procedure is app

[Perioperative risk among patients with cirrhosis].

Ten percent of cirrhotic patients with severely compromised liver function will require a surgical procedure. This article reviews the surgical risk associated with liver damage and surgery. The most important prognostic factor is the degree of functional impairment of the liver. It is evaluated using the Child-Pugh score, which has limitations that have been partially overcome by the Model for End Stage Liver Disease (MELD) score. Cardiac surgery has the highest risk, while extra-thoracic and extra-abdominal procedures have the lowest risk. The mortality for abdominal surgery fluctuates betwe
