A multimodal approach to acne mechanica associated to medical face masks using clinical examination, fluorescent photography, and in vivo reflectance confocal microscopy.

With yet unclear pathogenesis, acne mechanica associated to medical face masks(MFM) gained increased attention during the COVID-19 pandemic. Between November 2021-January 2022 we conducted an exploratory, cross-sectional study evaluating the differences between the glabella and cheek-chin junction using clinical examination, fluorescent photography (FP), videodermatoscopy and in vivo reflectance confocal microscopy (RCM). Medical doctors from our country and 6th year medical students were invited. Participants self-identified as consistent MFM users during work hours. We analysed 19,432 images…

The Impact of Aesthetic Osseous Genioplasty on Upper Airway Volumes - A Single Center Retrospective Cohort Study.

BACKGROUND: The effect of aesthetic osseous genioplasty on upper airway volume remains unclear, particularly the relative contributions of horizontal and vertical chin changes. METHODS: We retrospectively analyzed volumetric airway changes in 70 patients who underwent osseous genioplasty, using pre- and postoperative craniofacial CT scans (140 total). Image segmentation was performed to approximate volumes of the nasopharynx, oropharynx, hypopharynx, and total pharynx. Horizontal chin projection change was defined as the difference in anterior-posterior (AP) position of the osteotomized chin …

Effect of common airway manoeuvres on upper airway dimensions and clinical signs in anaesthetized, spontaneously breathing children.

Chin lift, jaw thrust and these manoeuvres combined with continuous positive airway pressure (CPAP) can be used to improve the patency of the upper airway during general anaesthesia. We used video endoscopy and measurement of stridor to compare the efficacy of these manoeuvres in 24 children (3-10 yr) with adenotonsillar hyperplasia. A bronchofibrescope was passed via the nose while the children were breathing spontaneously, to identify (i) the shortest transverse distance between the tonsils during inspiration and during expiration and (ii) the distance from the tip of the epiglottis to the p…

The use of patient-specific implants in genioplasty and its clinical accuracy: a preliminary study.

The purpose of this study was to assess the accuracy and clinical validation of patient-specific implants (PSI) in genioplasty. Fifteen patients with chin deformities were enrolled. Virtual planning was performed with the computer-aided surgical simulation method. The three-dimensional-printed titanium cutting guide and patient-specific plate were designed to guide the osteotomy and allow repositioning and fixation of the chin. The outcome was evaluated by comparing the plan with actual outcomes. All operations were successfully completed with PSIs. There was no difficulty in using patient-spe…

Modification of square face in men.

BACKGROUND: Women with elliptical faces are considered to be aesthetically appealing in Asia. Some men prefer this shape for themselves and choose to have their square-shaped face altered to an elliptical shape. To accomplish this, we perform an ostectomy that includes resection of the mandibular angle, splitting of the lateral cortex, and reduction in the width of the chin. METHODS: Nineteen men with a square face (aged 22-30 years) underwent ostectomy, including resection of the mandibular angle, splitting of the lateral cortex around the mandibular angle, and reduction of the width of the …

The prevalence of facial asymmetry in the dentofacial deformities population at the University of North Carolina.

In a retrospective survey of 1,460 patients evaluated in the Dentofacial Clinic at the University of North Carolina, 495 (34%) were found to have clinically apparent facial asymmetry. When present, asymmetry affected the upper face in only 5% (n = 23), the midface (primarily the nose) in 36% (n = 178), and the chin in 74% (n = 365). The occlusal plane was canted, indicating vertical asymmetry, in 41% (n = 201). Patients with Class II problems, whether or not due to mandibular deficiency, had a 28% prevalence of asymmetry; those with other types of problems (e.g., Class III, long face, Class I)…

Captain America's Shield Genioplasty.

The chin represents one of the most important determinants of the facial aesthetics. Like many aesthetic parameters, the "ideal" chin has changed in history regarding projection and prominence. From the retrusive profiles of the Renaissance, stronger and more defined mandibular contour are nowadays desired both by masculine and feminine population. This change in the ideal references plays an important role in diagnosis and treatment planning. Various techniques for chin augmentation have been described, using both alloplastic materials and osteotomies. An interesting osteotomy variant, so-calle…

Generalised electrographic seizures presenting as perioral myoclonia.

A 41-year-old man, during a neurological consultation, reported "chin twitching" over a period of a week, which was diagnosed as intermittent perioral myoclonia. With only one tonic-clonic seizure seven years before, he had mentioned several episodes of chin twitching over the years. In the clinic, there were intermittent chin movements without apparent confusion, as he was able to provide a complete history and was fully oriented with intact memory. His video-EEG showed paroxysms of polyspike and slow-wave activity, with the longest burst-free interval being 20 seconds. Discharges were maxima…

Refining orthognathic outcomes: 3D accuracy with patient-specific implants for facial asymmetry correction.

This study evaluated the three-dimensional accuracy of maxillary and chin repositioning using patient-specific implants (PSIs) in non-syndromic patients with facial asymmetry. A retrospective cohort study was conducted at the University Hospital of Geneva between 2018 and 2023. Three-dimensional pre-operative planning and post-operative CT or CBCT scans of 21 patients with facial asymmetry were analyzed. Using 3D Slicer, images were imported and segmented, and the anterior surfaces of the maxilla and chin were extracted via Meshmixer. CloudCompare software calculated the distance between post-…

Ethnic and Gender Considerations in the Use of Facial Injectables: Male Patients.

The contemporary male look includes sharp contours of the face with strong nose, significant malar-midface structure, and a powerful chin with well-defined jawline. Neurotoxins in the upper facial lines should preserve a lower position of the brows and flatter arch in men. Excessive lifting of the eyebrows and full correction of forehead and periorbital lines are not desirable. The use of dermal fillers is fully accepted into the nasolabial. Male patients with sagginess and hollowness related or not related to acquired lipodystrophies are candidates for cheek correction with volumizers. Female…

Cholinergic interneurons mediate cocaine extinction in male mice through plasticity across medium spiny neuron subtypes.

Cholinergic interneurons (ChINs) in the nucleus accumbens (NAc) have been implicated in the extinction of drug associations, as well as related plasticity in medium spiny neurons (MSNs). However, since most previous work relied on artificial manipulations, whether endogenous acetylcholine signaling relates to drug associations is unclear. Moreover, despite great interest in the opposing effects of dopamine on MSN subtypes, whether ChIN-mediated effects vary by MSN subtype is also unclear. Here, we find that high endogenous acetylcholine event frequency correlates with greater extinction of coc…

PET/CT illustration of metastatic breast cancer to the left mandibular foramen.

Mental nerve neuropathy (numb chin/lip syndrome) is a sensory neuropathy presenting with numbness in the distribution of the inferior alveolar nerve/mental nerve (chin and lower lip). This is typically unilateral and can be secondary to dental disease or malignancy. When caused by malignancy, these symptoms can be either an initial presentation of an unsuspected tumor or progressive metastatic disease, both of which would indicate poor prognosis. We describe a 48-year-old female patient with a history of breast cancer who presented with left chin numbness and manifested a metastatic lesion inv…

Quantitative analysis of facial asymmetry based on three-dimensional photography: a valuable indicator for asymmetrical temporomandibular joint affection in juvenile idiopathic arthritis patients?

BACKGROUND: Juvenile idiopathic arthritis (JIA) can cause osseous deformity in the temporomandibular joint (TMJ) and may impair mandibular growth. This study aimed to evaluate whether facial asymmetry determined clinically or by morphometric analysis of three-dimensional (3D) photographs in JIA patients is associated with an asymmetric affection of theTMJ. METHODS: Of 76 consecutive JIA patients with a mean age of 11.7 years (range: 6.3-17.9), facial asymmetry was evaluated clinically (chin asymmetry, gonion asymmetry), and stereophotogrammetrically with 3D photographs. The facial surfaces we…
