Laparoscopic cholecystectomy in acute cholecystitis. A prospective comparative study in patients with acute vs. chronic cholecystitis.

BACKGROUND: The aim of this prospective study was to compare the outcome of laparoscopic cholecystectomy (LC) in patients with acute cholecystitis versus those with chronic cholecystitis and to determine the optimal timing for LC in patients with acute cholecystitis. METHODS: From January 1991 to July 1998, 796 patients (542 women and 254 men) underwent LC. In 132 patients (67 women and 65 men), acute cholecystitis was confirmed via histopathological examination. These patients were divided into two groups. Group 1 (n = 85) had an LC prior to 3 days after the onset of the symptoms of acute ch…

Diffuse lymphoplasmacytic acalculous cholecystitis: a distinctive form of chronic cholecystitis associated with primary sclerosing cholangitis.

Inflammation of the gallbladder is known to occur in patients with primary sclerosing cholangitis (PSC). However, the histological features of this form of cholecystitis have not been adequately defined. The aim of this study was to compare the inflammatory lesions of PSC-associated cholecystitis with those present in other cholecystopathies. The cases consisted of 11 gallbladders from patients with PSC who underwent liver transplantation. As controls, gallbladders from liver transplant patients with primary biliary cirrhosis (n = 4) and other chronic nonbiliary hepatopathies (n = 8), and 13 c…

Electrocardiographic Changes in Cholecystitis and Cholangitis.

INTRODUCTION: Electrocardiographic (ECG) changes associated with biliary diseases, including cholecystitis and cholangitis, have been sporadically reported; however, detailed observational data remain scarce. This study aimed to investigate the prevalence and characteristics of ECG abnormalities in patients with cholecystitis and cholangitis, and to identify associated clinical factors. MATERIALS AND METHODS: We conducted a retrospective analysis of 402 patients hospitalized for biliary diseases between January 2018 and January 2025. After excluding patients without comparable ECGs or with co…

A Case of Cholecystitis Camouflaging Cholangiocarcinoma.

Cholangiocarcinoma is a malignancy that is hard to detect and resect, due mostly to its location as well as a lack of current screening tests. When found, it is often in the advanced stage as patients are usually asymptomatic during the early course of the disease; the overall prognosis is modest in patients diagnosed at this stage. Here, we discuss the case of a 48-year-old female with no significant past medical history or family history who presented to our hospital with symptoms of acute cholecystitis with a supporting ultrasound. She proceeded to get a laparoscopic cholecystectomy for the…

[Diagnosis and treatment of acute cholecystitis].

Diagnosis and treatment of acute cholecystitis Abstract. Acute cholecystitis is one of the most common acute surgical diseases. Typical clinical symptoms are pain in the upper abdomen, fever and leucocytosis. Ultrasonography may often be used to confirm the clinical diagnosis. There is a consensus that laparoscopic cholecystectomy is recommended as the treatment of choice and should be performed immediate after diagnosis independent of the onset of symptoms. The risk of complications is prevented by surgery. Surgery is the treatment of choice for acute cholecystits also in elderly patients wit…

A comparison of cholecystitis grading scales.

BACKGROUND: Previously, our group developed the Parkland grading scale for cholecystitis (PGS) to stratify gallbladder (GB) disease severity that can be determined immediately when performing laparoscopic cholecystectomy (LC). In prior studies, PGS demonstrated excellent interrater reliability and was internally validated as an accurate measure of LC outcomes. Here, we compare PGS against a more complex cholecystitis severity score developed by the national trauma society, American Association for the Surgery of Trauma (AAST), which requires clinical, operative, imaging, and pathologic inputs,…

Acute cholecystitis in patients with stroke.

BACKGROUND: Although acute cholecystitis after stroke is rare, an immediate diagnosis and treatment is required. MATERIAL AND METHODS: In the past five years, we observed six patients with acute cholecystitis during the initial hospitalization for stroke. RESULTS: Three patients had cardiac embolism, two had subarachnoid hemorrhage, and one had intra-cerebral hemorrhage. Four had calculous cholecystitis and two had acalculous cholecystitis. One of the patients with acalculous cholecystitis had hemorrhagic cholecystitis. The most commonly presented symptom was fever (50%), whereas only one pa…

Schistosomiasis as a cause of acute cholecystitis.

Schistosomal cholecystitis is remarkably uncommon disease which is associated with concomitant gallstone disease. The disease prevails with low socio-economic conditions of population. It is still not clear that the widespread fibrocalcific reaction in the wall of gallbladder and the cystic duct, observed for all the reports, predisposes to cholelithiasis or gallstones coexist. In the present study, we report a case of the Saudi male patient affected with schistosomal granulomatous lithiasic cholecystitis. The patient's  gallbladder was badly inflamed therefore performing a safe cholecystectom…

Clinical Cholecystitis in the Absence of the Gallbladder.

The congenital absence of the gallbladder (CAG) is a rare condition with an incidence of 13-65 cases/ 100,000 in the general population. This occurs when the gallbladder and the cystic duct fail to bud from the common bile duct during the fifth week of gestation. Most commonly, the patients with congenital absence of the gallbladder are asymptomatic. When symptomatic, they present as biliary colic, dyspepsia, jaundice or very rarely as acute cholecystitis. We present a case of a 27-year-old female who presented with acute right upper quadrant abdominal pain. Further evaluation with an ultrasou…

A Rare Case of Emphysematous Cholecystitis.

Emphysematous cholecystitis is an acute infection of the gallbladder wall caused by gas-forming organisms. It is infrequent with insidious onset and diagnosed by the use of radiographs detecting presence of air within the gallbladder wall or lumen. The report describes the case of a 42-year-old alcoholic male who presented with sudden onset of pain in the right upper quadrant of abdomen, fever and bilious vomiting of two days duration. The patient did not have symptoms of jaundice. Emergency partial cholecystectomy was done and the culture directed antibiotics were given. The patient was follo…

Patients' experiences with cholecystitis and a cholecystectomy.

Nurses commonly care for patients with cholecystitis, a major health problem with a growing prevalence. Although considerable research has been done to compare patient outcomes among surgical approaches for cholecystitis, few studies have examined the experiences of patients with cholecystitis and the subsequent cholecystectomy surgery. A qualitative study with a phenomenological approach was initiated to better understand the experience of hospitalized patients with cholecystitis through their cholecystectomy surgery. Face-to-face semistructured interviews were conducted with patients diagnos…

Diagnosis and treatment of xanthogranulomatous cholecystitis.

BACKGROUND: Cardiac arrhythmias as a late complication following congenital heart surgery are encountered more and more frequently in clinical practice. The use of new electrophysiological methods of visualisation and mapping improves the efficacy of radio-frequency (RF) ablation of these arrhythmias. AIM: To assess patterns of atrial arrhythmias following congenital heart surgery and to examine the efficacy of RF ablation using the electro-anatomical CARTO system. METHODS: Electrophysiological diagnostic study and RF ablation were performed in 24 consecutive patients (mean age 36+/-18 years

[Cholecystic fistula with atypical symptoms].

We report a patient with spontaneous cholecystocolonis fistula secondary to cholelithiasis. A 93 year-old woman was admitted because of weight loss, diarrhoea and upper abdominal pain. Ultrasound examination revealed air in the biliary tract and cholescientigraphy revealed a fistula between the gallbladder and right colon. Using endoscopic retrograde cholangiopancreatography a calculus was extracted from the bile duct and the symptoms disappeared.

Study on carcinogenesis in chronic cholecystitis.

Not only bile but also chronic cholecystitis may play a role in gallbladder carcinogenesis. Numerous studies have revealed a close correlation between chronic inflammation and neoplasia. The experiments were conducted on paraffin sections, obtained from 377 surgically resected gallbladders with chronic cholecystitis. Immunohistochemical reaction was conducted on deparaffinized sections, using a monoclonal antibody against 8-hydroxydeoxyguanosine (8-OHdG), a biomarker of oxidative DNA damage. An increase was found in the expression of 8-hydroxydeoxyguanosine in chronic cholecystitis. The level …
