Application and improvement of ipsilateral clavicle turnover during proximal humeral tumorous bone defect repair.

PURPOSE: To investigate the clinical effects and complications that may arise as a result of ipsilateral clavicle turnover during tumorous bone defect repair of the proximal humerus, and to explore the feasibility of performing combined free fibula transplantation and internal fixation in reducing complications METHODS: We treated 13 patients with clavicle turnover or free fibular transplantation from September 1998 to February 2020. There were 7 males and 6 females with the average age of 26.2 years (7-50 years). The histological diagnoses were osteosarcoma in 6 patients (Enneking IIB), giant…

Surgical clavicle reconstruction after aneurysmal bone cyst resection in a child: A simple method.

The clavicle is an infrequent location for primary tumors in general, and aneurysmal bone cyst (ABC) of the clavicle is particularly rare. The challenge of the functional and esthetic result in the treatment of these lesions in the pediatric population is high when considering the reconstruction of critical bone defects. In this article, we present the case of a seven-year-old boy with an ABC in the middle third of the clavicle, treated by resection and reconstruction with free autograft of the fibula stabilized by using an intramedullary titanium nail. We offer a description of the used techn…

Epidemiology of Clavicle Fractures Among US High School Athletes, 2008-2009 Through 2016-2017.

BACKGROUND: Little is known about the epidemiology of clavicle fractures in United States (US) high school athletes. Sports participation among high school students has increased steadily, placing increased numbers at risk of sports-related injury. PURPOSE: To describe the epidemiology of clavicle fractures among high school athletes, including injury rates by sex, sport, and type of play and trends in operative versus nonoperative treatment. STUDY DESIGN: Descriptive epidemiology study. METHODS: The study data set included all athlete-exposure (AE) and clavicle fracture data collected from…

Radiographic outcomes of single versus dual plate fixation of acute mid-shaft clavicle fractures.

INTRODUCTION: The clavicle exhibits considerable movement in three planes making rigid fixation challenging. The addition of a second plate may be considered to improve fixation rigidity, but risks compromising the blood supply to the fracture site. The purpose of this study is to assess if extraperiosteal dual plate fixation increases the rate of non-union, reoperation, and complications at 1 year for surgically treated acute mid-shaft clavicle fractures. METHODS: Between June 1998 and June 2013, surgically treated mid-shaft clavicle fractures undergoing open reduction internal fixation with…

Closed vs open nailing for displaced middle third fracture of clavicle. Does it matter?

BACKGROUND: Intramedullary nailing for fractures of middle third clavicle has become increasingly popular. Though open nailing has been widely described, closed nailing finds less mention. OBJECTIVES: In this study we compared closed nailing with open nailing in fractures of middle third of clavicle to assess if the former holds any advantage. MATERIAL AND METHODS: 34 patients with closed nailing were compared to 31 patients with open nailing in terms of operative time, length of incision, pain, time to union and functional outcome. RESULTS: Only operative time and length of incision were s…

Posterior sternoclavicular Salter-Harris fracture-dislocation in a patient with unossified medial clavicle epiphysis.

Sternoclavicular injuries are relatively rare, composing less than 1 % of all musculoskeletal fractures or dislocations. When sternoclavicular injuries do occur, they typically present as an isolated dislocation of the sternoclavicular joint without associated fracture of the clavicle or manubrium. However, in patients with unfused medial clavicle physis, sternoclavicular joint injuries can present as a fracture-dislocation through the unfused physis. These physeal injuries are important to recognize as the displaced epiphysis can block reduction of the sternoclavicular joint. We present a cas…

Radiological and functional outcomes 2.7 years following conservatively treated completely displaced midshaft clavicle fractures.

OBJECTIVES: It is unclear whether all completely displaced midshaft clavicle fractures require primary surgical intervention. The aim of this study was to elucidate the radiological and clinical outcomes after conservative treatment, and to identify subgroups at risk of an inferior outcome. DESIGN: Retrospective case series. SETTING: Level II trauma center. PATIENTS: Between 2005 and 2008, 122 patients were conservatively treated for a completely displaced midshaft clavicle fracture of whom 92 were eligible for inclusion in this study. Of these, 59 completed the study after a median of 2.7 …

Late onset venous thoracic outlet syndrome following clavicle non-union fracture: A case report.

A 59-year-old woman was admitted three times over a six-month period with recurrent upper extremity deep venous thrombosis (UEDVT). It was determined that this patient was suffering from an unusual presentation of Paget-Schröetter syndrome secondary to a 20-year-old non-union of a midshaft clavicle fracture. Following thrombolysis the patient underwent resection and plate fixation of the clavicle fracture non-union. Despite the anatomic proximity of the subclavian vessels to the clavicle, vascular complications from fracture are rare. Treatment of midshaft clavicle fractures is often non-opera…

Angular stable fixation of displaced distal-third clavicle fractures with superior precontoured locking plates.

HYPOTHESIS: We reviewed the outcome of angular stable plates in addressing displaced lateral-third clavicle fractures. We investigated union, shoulder function, request for implant removal, and return to sport. Our hypothesis was that these implants provide predictable union and return to sports without the negative consequence of leaving plates in situ, reducing the requirement for a second surgery. METHODS: We undertook a retrospective review of a consecutive series of patients who underwent this surgery between 2007 and 2010. Nineteen patients with a mean follow-up of 25 months were includ…

Does whole-spine lateral radiograph with clavicle positioning reflect the correct cervical sagittal alignment?

PURPOSE: To evaluate the differences of cervical alignment between standing cervical lateral radiograph and whole-spine lateral radiograph with clavicle position. METHODS: We prospectively evaluated 101 asymptomatic adults from whom standing cervical lateral radiograph with hands on both side followed by whole-spine lateral radiographs with clavicle position were obtained from April 2012 to December 2013. On two radiographs, cervical sagittal alignment from C2 to C7 was analyzed by Gore angle (GA) and Cobb angle (CA); head position was evaluated using the translation distance (TD, distance of…

Comparison between conservative and surgical treatment of midshaft clavicle fractures: outcome of 151 cases.

INTRODUCTION: Midshaft clavicle fractures comprise up to 15% of all adult upper extremity fractures and account for 76% of all clavicle fractures. The treatment of choice remains controversial. The aim of our retrospective study was to compare the outcome of the surgical and conservative procedure in a trauma care unit (single center study). MATERIAL AND METHODS: In a cohort of 151 (mean age 36,1y/male 115/female 36) cases, between 2005 and 2009, 70 patients (46.4%) were treated conservatively (mean age 40.8y) and 81 (53.6%) underwent either surgical treatment with a locking compression plate…

Emergency bedside sonographic diagnosis of subclavian artery pseudoaneurysm with brachial plexopathy after clavicle fracture.

Clavicle fractures are common; however, complications are unusual. Two such complications, subclavian artery pseudoaneurysm and brachial plexopathy, are rare events that can cause significant morbidity and mortality. We report the case of a 53-year-old man who presented with shoulder swelling and right arm weakness for 1 week. Three weeks before, he had fallen and fractured his right clavicle. On presentation to our emergency department, his examination revealed a brachial plexopathy and a large supraclavicular mass. An emergency bedside triplex sonogram was performed to characterize the mass …

Function plateaus by one year in patients with surgically treated displaced midshaft clavicle fractures.

BACKGROUND: Based on short-term (1 year or less) followup, primary fixation of displaced midshaft clavicle fractures reportedly results in better function compared with that reported for nonoperative methods. Whether better function persists beyond 1 year is unclear. QUESTIONS/PURPOSES: For displaced midshaft clavicle fractures, do the better mean Disabilities of the Arm, Shoulder and Hand (DASH) and Constant-Murley Shoulder (CSS) scores for operative versus nonoperative treatment at 1 year change between 1- and 2-year followup? PATIENTS AND METHODS: We previously reported 132 patients in a …

Surgical treatment of neurogenic thoracic outlet syndrome secondary to the clavicle haematogenic subacute osteomyelitis.

A rare case of neurogenic thoracic outlet syndrome secondary to subacute osteomyelitis (SOM) of clavicle is presented. It was treated successfully with scalenectomy combined with clavicle and first rib resection and reconstruction of the clavicle using vascularised seventh rib transfer. Various issues involved in the diagnosis and management of such a case are discussed.

[Missed clavicle fracture, deep vein thrombosis following bicycle accident with head and chest trauma].

A 57-year-old patient suffered head and chest trauma following a bicycle accident. After 12 days in hospital with early mobilization the patient developed a deep vein thrombosis 5 days after being discharged and the family physician diagnosed a non-displaced clavicle fracture. The patient complained that no thrombosis prophylaxis had been carried out during the stay in hospital and the clavicle fracture had been overlooked. The expert opinion from the arbitration board determined that no health limitations had occurred as a result of the missed clavicle fracture. There were no indications for …

Complications of clavicle fracture and acromioclavicular joint rupture. What the general surgeon should know.

Fractures of the clavicle and acute dislocations of the acromioclavicular joint are common seen in the emergency room. Displaced or comminuted clavicle fractures and acromioclavicular joint ruptures (types III-VI) are associated with complications, such as subclavian vessels injury, hemopneumothorax, scapula-thoracic dissociation or brachial plexus paresis. Herein we discuss the usually complications seen after these injuries and underline whatever a trauma surgeon should know in order to avoid fatal situations.

Bilateral clavicle non-unions treated with anteroinferior locking compression plating (LCP): a case report.

The author reports the case of a 30-year-old female patient with bilateral atrophic non-union of the clavicle; the latter both healed after internal fixation with a locking compression plate fixed on the anteroinferior aspect of the clavicle, combined with autologous cancellous bone grafting. The advantages of the anteroinferior positioning of the plate on the clavicle are presented.

Musculocutaneous nerve injury after coracoid process transfer for clavicle instability. Report of three cases.

During the last 10 years, we have treated 54 cases of fracture or dislocation of the lateral clavicle by transposing the coracoid process onto the clavicle. Recently, a postoperative musculocutaneous nerve injury was observed in 3 cases, all of which recovered spontaneously within 5 months. Our technique seems to entail a certain risk of causing stretch injury to the musculocutaneous nerve. Apart from resting the affected arm for a month or two, the condition does not require specific treatment.
