Development of a hybrid artificial intelligence framework for pharmacotherapy optimization.

INTRODUCTION: Pharmacotherapy optimization in multimorbid patients is increasingly complex due to polypharmacy, fragmented data, expanding electronic health records, and workforce constraints. Conventional clinical decision support systems remain largely rule-based and often fail to adequately incorporate patient-specific context. While artificial intelligence offers new opportunities, stand-alone models remain insufficiently reliable for high-risk pharmacotherapy decision support. AIM: To develop a relevance-driven, clinician-supervised hybrid AI framework for pharmacotherapy optimization.

A scoping review of explainable artificial intelligence for medical multimodal data.

Multimodal Artificial Intelligence (AI) models-integrating diverse data such as imaging and clinical records-are advancing rapidly in healthcare, yet a significant disconnection persists between these complex predictive architectures and the explainable AI (XAI) techniques used to interpret them. We conducted a scoping review over 4 bibliographic databases to investigate the use of explainability methods in cross-modal medical AI studies. From 82 included studies, we found that the landscape remains dominated by independent feature attribution (assigning importance scores to individual modalit

The methodology of Dynamic Uncertain Causality Graph for intelligent diagnosis of vertigo.

Vertigo is a common complaint with many potential causes involving otology, neurology and general medicine, and it is fairly difficult to distinguish the vertiginous disorders from each other accurately even for experienced physicians. Based on comprehensive investigations to relevant characteristics of vertigo, we propose a diagnostic modeling and reasoning methodology using Dynamic Uncertain Causality Graph. The symptoms, signs, findings of examinations, medical histories, etiology and pathogenesis, and so on, are incorporated in the diagnostic model. A modularized modeling scheme is present

Reframing diagnostic reasoning: the Bayesian imperative in shoulder examination.

Despite the widespread use of clinical shoulder tests such as the Jobe or Neer test, diagnostic reasoning often remains binary and insuffici … neous range-based summaries, Bayesian modeling provides a coherent and evidence-based framework for clinical reasoning that explicitly accounts for diagnostic uncertainty. As such, it offers a more robust approach for interpreting clinical test results in musculoskeletal care.

Dual-stage anatomical perception and reasoning framework for automated cobb angle estimation.

Accurate and automated Cobb angle measurement is essential for the diagnosis and management of Adolescent Idiopathic Scoliosis (AIS). However, existing deep learning approaches often ignore the intrinsic anatomical dependencies of the spine, which results in physiologically implausible estimations. We propose a two-stage anatomical perception and reasoning framework based on keypoint detection for Cobb angle measurement. The anatomical perception stage is built upon a Local-to-Global Aggregation Backbone (LGAB), which combines convolutional neural networks and Transformers to jointly capture l

Investigating Miranda waiver decisions: An examination of the rational consequences.

Millions of custodial suspects waive their Miranda rights each year without the benefit of legal counsel. Miranda understanding, appreciation, and reasoning abilities are essential to courts' acceptance of Miranda waivers (Grisso, 2003; Rogers & Shuman, 2005). The question posed to forensic psychologists and psychiatrists in the disputed Miranda waivers is whether a particular waiver decision was knowing, intelligent, and voluntary. Despite the remarkable development of Miranda research in recent decades, studies have generally focused on understanding and appreciation of Miranda rights, but w

Cognitive Function in Adults with Enduring Anorexia Nervosa.

Anorexia Nervosa (AN) is a severe and often enduring disorder characterized by restriction of food intake, low body weight, fear of weight gain, and distorted body image. Investigations on cognition performance in AN patients have yielded conflicting results. Using an established and sensitive computerized cognitive test battery, we aimed to assess core aspects of cognitive function, including attention span, information processing, reasoning, working and episodic memory, in AN patients and controls. Patients were recruited from the Danish Prospective Longitudinal all-comer inclusion study in

Give a Practice, Not a Verdict: Making Feedback Usable in Clinical Teaching.

Clinical teachers often notice something developmentally important in a learner's work before they have enough evidence to make a global judgement about the learner. A resident's presentation may include the correct facts but make the assessment difficult to hear; a plan may be reasonable but leave risk unnamed; a note may contain required elements but obscure clinical reasoning. These moments can be hard to address because broad interpretations may feel unfair, whereas overly hedged feedback may leave the learner without a clear way forward. This article offers a practical translation pathway…

Large Language Model - Enhanced Decision Tree Framework for Identifying Multiple Sclerosis Diagnoses from Clinical Documentation.

BACKGROUND: Early diagnosis and intervention are crucial in multiple sclerosis (MS), yet diagnostic delays are common. Large language models (LLMs) such as generative pre-trained transformers (GPTs) may help streamline diagnostic workflows by extracting MS diagnostic signals from clinical notes. OBJECTIVE: To derive MS diagnosis status from the first neurology note using a computable algorithm based on the 2017 McDonald criteria and applying GPT-4 for node-level reasoning within a structured decision framework. METHODS: We analyzed first neurology notes from 125 randomly selected patients (i

Implications of Variability in Clinical Bedside Swallowing Assessment Practices by Speech Language Pathologists.

Speech language pathology (SLP) clinical bedside swallowing assessments (CBSA) are a cornerstone of quality care for patients in acute hospi … so demonstrate consistent use of a core set. It is apparent that SLPs prioritise the application of clinical reasoning to tailor their CBSA to the patient over following a highly structured item-based protocol. The variability in component use likely reflects a complex clinical reasoning process that draws on a wide variety of information combined with expert knowle …

How Patients' Inquiry Shapes Meaning in Primary Care Consultations: A Qualitative Study Using Own-Point-of-View Video-Stimulated Recall.

RATIONALE: Medical consultations are collaborative encounters in which physicians and patients jointly address health concerns and make decisions. While most research has focused on physicians' clinical reasoning, decision-making is increasingly viewed as a form of situated cognition, distributed across individuals and their interactions. However, the patient's perspective on how they make sense of their health problems remains underexamined. AIM AND OBJECTIVES: The aim of this study was to investigate how patients experience and make sense of their health problems before and during a primary…

Bias mitigation skills in diagnostic reasoning: utilizing a situated cognition approach.

PROBLEM: Diagnostic errors are common in healthcare and often stem from a failure to consider the social and contextual factors that influence clinician decision-making. Systemic inequities and clinician bias disproportionately contribute to diagnostic inaccuracies among marginalized communities. Medical schools are well-positioned to address this challenge by providing curricula that explore the impact of social context on diagnostic reasoning and equip students with strategies to mitigate harms associated with clinician bias. APPROACH: The authors developed a novel curriculum grounded in si

Agentic artificial intelligence in cognitive screening: A translational roadmap for dementia care.

Conventional dementia screening tools, whether paper tests or static artificial intelligence (AI) models, capture only a snapshot of cognition and need to be adminstered periodically. Yet decline is dynamic, shaped by environment, comorbidities, and life history. Current approaches rarely adapt, integrate feedback, or provide transparent reasoning that clinicians can trust. We propose agentic AI systems: modular agents built on large language models (LLMs) that collaborate, adapt, and mirror interdisciplinary care. The Cognitive Agent Lab exemplifies this framework, emphasizing functional aspe

The role of feared possible selves in the relationship between inferential confusion and obsessive-compulsive symptoms: A replication and extension in a clinical sample.

OBJECTIVES: The feared possible self refers to an imagined version of self that one is afraid of being or becoming. Previous evidence has shown that dysfunctional reasoning (i.e., inferential confusion) is associated with obsessive-compulsive disorder (OCD) symptoms, which is partially mediated by a feared self. However, the evidence is reliant on non-clinical samples and a general measure of the feared self. METHODS: Using a cross-sectional design, the current study attempted to replicate and extend this literature in a sample clinically diagnosed with OCD (n = 350) to assess the pathway fro

Effectiveness of a high-fidelity simulation program based on flipped learning for nurses' emergency management of seizures: a mixed-methods design.

BACKGROUND: A seizure is a transient occurrence of signs and/or symptoms caused by abnormal excessive or synchronous neuronal activity in the brain. Nurses play a critical role in recognizing and managing seizures, which encompasses airway management, hemodynamic monitoring, and safe administration of antiepileptic medications. High-fidelity simulation (HFS) based on flipped learning integrates self-directed pre-learning with realistic clinical scenarios to enhance clinical performance, problem-solving abilities, and clinical reasoning. Clinical nurses have limited access to HFS based on flipp…
