OBJECTIVE: To observe the effect of different manipulative reduction for children's distal radioulnar fracture and dorsal dislocation.
METHODS: From June 2013 to June 2014, 80 children with distal radioulnar fracture and dorsal dislocation were treated by bone setting manipulative reduction including 51 males and 29 females with an average age of 6.5 years old ranging from 3 to 14 years old. Time from injury to treatment was 1 h to 6 d, 31 cases were on the right, 49 cases were on the left. Among them, 45 cases were type I of overlapping displacement, 35 cases were type II. The displacement o

We undertook a retrospective study of 39 children with displaced supracondylar fractures treated by delayed closed reduction with fluoroscopic guidance. After 3-5 days of side-arm traction, swelling had usually diminished sufficiently to allow the elbow to be safely hyperflexed to stabilize the fracture after elective closed reduction. This method yielded 92% good or excellent results. There were no vascular problems or Volkmann's contractures.

Forty-one patients with 42 elbow dislocations, of which 13 had acute simple dislocations, 21 had fracture-dislocations and 8 had neglected elbow dislocations, were evaluated with regard to limitation of motion, pain, instability and residual neurovascular deficit. All patients with acute simple dislocation were treated with closed reduction, but the duration of immobilization before commencement of active motion varied. Open-reduction was indicated for failed closed reduction and neglected-dislocations. Internal fixation as a primary procedure was only performed for displaced fracture-dislocat

OBJECTIVES: Posterior iliac crescent fracture with associated sacroiliac joint disruption (PICFSID) is a type of traumatic posterior pelvic injury associated with instability. Posterior iliac fracture location and obliquity along with other details determine the treatment. Open reduction and internal fixation has been previously recommended for the majority of crescent fractures. Our objectives were to determine whether all crescents can be categorised according to the previously described Day crescent classification scheme, and to see which crescent types are amenable to percutaneous fixation

Avulsion fracture of the tibial tuberosity usually caused by sudden violent contraction of the quadriceps muscles is uncommon and predominantly occurs in adolescents. Illustrative is the case history of a 16-year old boy with an Ogden I-B avulsion fracture successfully treated by open reduction and internal screw fixation. In these cases, anatomic reduction should be the primary goal. In specific cases this can be achieved by closed reduction, however, usually open reduction and fixation is indicated.

Pediatric supracondylar humerus fractures are the most commonly encountered type of elbow fractures in children that require surgical fixation. Many pediatric supracondylar humerus fractures can be treated with closed reduction and percutaneous skeletal fixation. In difficult fractures, adjunct pin techniques, such as joystick wires and leverage pins, can be used to help attain a satisfactory and stable reduction before an open approach is used. After the fracture is reduced, optimal pinning, with the use of either crossed or lateral-entry techniques, and fixation that achieves maximal spread

A 49-year-old man suffered a closed oblique fracture of the middle third of his left femur. Closed reduction and internal fixation by intramedullary (IM) nailing were performed. Per-operative fluoroscopic imaging and initial postoperative X-rays were judged normal and the patient followed the usual rehabilitation protocol. At 3-month follow-up the patient still demonstrated poor knee function and pain. A plain X-ray and a CT scan of the left knee revealed a displaced fracture of the medial femoral condyle. Analysis of the postoperative imaging suggests that the fracture occurred during the ins

PURPOSE: We propose a new classification of subcondylar fractures of the mandible based on ramal height shortening and degree of fracture angulation and present our treatment experience using this classification system.
PATIENTS AND METHODS: Eighty patients with subcondylar fractures of the mandible were evaluated. We categorized the fractures into 3 classes: Class 1 (minimally displaced), fracture with ramal height shortening: <2 mm and/or degree of fracture displacement: <10°; Class 2 (moderately displaced), fracture with ramal height shortening: 2 to 15 mm and/or degree of fracture displac

BACKGROUND: Displaced tibial shaft fractures are common in adolescent patients, yet there is no standardized management strategy. We compared surgical fixation and closed reduction and casting (CRC) of these fractures to assess treatment outcomes and determine predictors of failure.
METHODS: We retrospectively reviewed all patients aged 12 to 18 who presented with a displaced tibial shaft fracture that required reduction over an 8-year period. Exclusion criteria included open fractures and lack of follow-up to radiographic union or to 6 months from the index procedure. Fractures were initiall

OBJECTIVE: To investigate the treatment method and effectiveness of fresh closed fracture-dislocation of the midtarsal joint.
METHODS: Between April 2004 and April 2011, 73 patients (75 feet) with fresh closed fracture-dislocation of the midtarsal joint were treated with closed reduction combined with open reduction and internal fixation. There were 56 males (58 feet) and 17 females (17 feet), aged from 19 to 62 years (mean, 35.8 years). Injuries were caused by falling from height in 35 cases, by sprain in 4 cases, by machine twist in 5 cases, by heavy pound in 9 cases, and by traffic acciden

INTRODUCTION: Proximal humerus fractures are uncommon in children, accounting for <5% of pediatric fractures, with glenohumeral dislocation being an even rarer occurrence. These injuries are challenging due to the complex shoulder anatomy and the risk of growth disturbances, as the proximal humeral physis contributes ~80% of humeral growth. While many fractures can be managed conservatively, severe displacement or associated dislocations, such as Neer-Horwitz Type IV fractures, may require surgical intervention. Treatment options include K-wires, elastic stable intramedullary nailing, and plat

INTRODUCTION: In children and adolescents distal forearm physeal fractures are common. Usually distal forearm physeal injuries of are common injuries in children and adolescents. Epiphyseal injuries to the distal radius are common in children, but involvement of the distal ulna is rare. Fracture of the distal radius with dislocation of the DRUJ is known as a True Galeazzi fracture dislocation and an epiphyseal separation of the distal ulna occurred instead of dislocation of DRUJ or both)[10] is called Galeazzi equivalent lesions. Galeazzi fractures in children are less common than in adults. [

BACKGROUND: Combined symphyseal-condylar mandible fractures are relatively common but difficult to manage in children. This study investigated the cause and management of symphyseal-condylar fractures in pediatric patients.
METHODS: This study presents a retrospective review and expert opinion of pediatric symphyseal-condylar mandibular fracture management at the authors' institution between 1990 and 2019. National data from the Healthcare Cost and Utilization Project Kids' Inpatient Database (2000 to 2016) were used to determine whether institutional data had national applicability.

BACKGROUND: Pure Posteromedial tibiotalar dislocation unaccompanied by fractures represents an exceptionally rare orthopedic injury, accounting for approximately 0.065% of ankle injuries and 0.46% of ankle dislocations. Such injuries typically result from high-energy trauma, with limited reports involving low-energy mechanisms in athletic activities. This case illustrates the successful management of a closed posterior ankle dislocation in a young student, emphasizing the importance of prompt intervention and structured rehabilitation.

A 57-year-old right-hand-dominant woman was involved in a motor vehicle collision. Upon examination, her right hand was markedly deformed and swollen, with limited range of movement. Plain radiographs revealed dorsal dislocations of the index, long, ring, and small finger carpometacarpal joints and an avulsion fracture of the dorsal aspect of the capitate. Closed reduction was unsuccessful. Closed reduction under general anesthesia was successful on the carpometacarpal joint of the ring and small fingers, however, the long and index fingers remained irreducible. An open approach revealed that

We are reporting the one-year follow-up for a case of traumatic atlantoaxial dislocation associated with an odontoid fracture. This injury combination is rare and serious because of its resulting instability. After an unsuccessful attempt at closed reduction with traction, an open reduction with occipitocervical fixation was performed using a posterior approach. Based on our experience and a review of the published literature, the method for managing such an injury is discussed. If closed reduction with traction is successful, subsequent treatment is based on the algorithms for isolated odonto

Supracondylar fracture of femur after total knee arthroplasty (TKA) presents a challenge for orthopedic surgeons. It is controversial to treat such a fracture by using closed method or open method. Nonunion, malunion, knee rigidity and other complications in prolonged immobilization were noted in the closed method. Implant migration, infection, nonunion and malunion were also reported in open reduction treatment because of the difficulty to achieve rigid fixation. In this periprosthetic fracture, retrograde intramedullary Huckstep nailing provides a better chance to get rigid fixation compared

BACKGROUND: Proximal humeral fractures are relatively common in pediatric patients. These injuries are usually treated nonoperatively in younger children or children with minimally displaced fractures. However, closed reduction or open reduction followed by percutaneous pinning is recommended for older children with displaced fractures. Percutaneous pinning has several advantages, but there are limited reports of a safe and reliable surgical technique in the literature.
