Peripheral nerve blocks for primary and secondary headache disorders: review of current evidence and a practical approach.

Headache is prevalent, disabling, and a frequent neurological referral in the healthcare system. Clinic-based procedures have evolved in recent years to play an important role in headache medicine, with growing evidence on the safety, tolerability and efficacy of peripheral nerve blocks (PNBs). Despite novel headache therapies, PNBs are still widely used in headache services to treat primary and secondary headache disorders, including cluster headache and other trigemino-autonomic cephalalgias, migraine, occipital neuralgia, and other less frequent headache disorders. We aim to provide an upda…

[SUNCT syndrome--case report and review of the literature].

The SUNCT syndrome is a recently described headache disorder characterised by shortlasting, unilateral headache attacks with conjunctival injection and tearing. Our report presents a further case and compares the findings in our patient with those in the cases described earlier. In addition, we review the literature and discuss possible aetiological and pathogenetic factors as well as the differential diagnosis of SUNCT syndrome, trigeminal neuralgia and cluster headache syndromes.

Headache diagnosis.

Headache is an extremely common symptom in primary care practice. Despite the ubiquity of the pain, differential diagnosis of headache is not difficult, provided the clinician obtains a comprehensive history. A complete physical examination and specific testing may be required to rule out other underlying causes, but headache itself falls into 3 main classes that are readily identified. Migraine headache occurs most commonly in women, is of moderate to severe intensity, and is often accompanied by nausea and increased sensitivity to light and sound. Cluster headache describes multiple recurren…

Primary headache disorders: From pathophysiology to neurostimulation therapies.

Primary headache disorders including migraine, cluster headache, and tension-type headache are among the most common disabling diseases worldwide. The unclear pathogenesis of primary headache disorders has led to high rates of misdiagnosis and limited available treatment options. In this review, we have summarized the pathophysiological factors for a better understanding of primary headache disorders. Advances in functional neuroimaging, genetics, neurophysiology have indicated that cortical hyperexcitability, regional brain dysfunction, central sensitization and neuroplasticity changes play v…

Nitric oxide-related drug targets in headache.

Nitric oxide (NO) is a very important molecule in the regulation of cerebral and extra cerebral cranial blood flow and arterial diameters. It is also involved in nociceptive processing. Glyceryl trinitrate (GTN), a pro-drug for NO, causes headache in normal volunteers and a so-called delayed headache that fulfils criteria for migraine without aura in migraine sufferers. Blockade of nitric oxide synthases (NOS) by L-nitromonomethylarginine effectively treats attacks of migraine without aura. Similar results have been obtained for chronic the tension-type headache and cluster headache. Inhibitio…

Sex hormones and primary headaches other than migraine.

The relation between sex hormones and migraine has been examined in a series of studies, leading to the definitions of pure menstrual migraine and menstrually-related migraine. The relation between sex hormones and other types of primary headache has been studied less extensively, but there is at least some evidence that hormones in general, and menstruation, pregnancy, or menopause in particular, also impact these disorders. This article reviews the available literature on changes of tension-type headache, cluster headache, other trigeminal autonomic cephalalgias, and hemicrania continua duri…

Genetics in primary headaches.

This tutorial describes different methods and results of genetic studies of primary headaches. A positive family history is imprecise, because it does not specify the number of affected, family size or relation to the proband. Nor does it include an interview of the possibly affected family members. Calculation of the familial aggregation after confirmation of the diagnosis by a physician is more precise. Compared to the general population, first-degree relatives of probands with migraine without aura, migraine with aura, chronic tension-type headache and cluster headache has a significantly i…

Children's Headache: Drawings in the Diagnostic Work Up.

OBJECTIVES: This study aims to evaluate the drawings effectiveness in childhood headache assessment. BACKGROUND: Headache is a common cause of pain in children. Although drawings have been used in childhood to recognize psychological insights and pain perception, they were rarely used for headache characterization. METHODS: We collected drawings from 67 subjects with cephalalgia during a 22-month timeframe. The clinical diagnosis was made according to the 2nd edition of The International Headache Classification. Drawings were independently categorized as migraine or tension-type headache (TT

Expression of melatonin receptors in trigeminal and sphenopalatine ganglia: potential targets for primary headache disorders.

BACKGROUND: Primary headache disorders such as migraine and cluster headache exhibit circadian and circannual variations in attack onset. Melatonin plays a central role in regulating biological rhythms and likely influences the timing of headache attacks. Recent evidence suggests melatonin may be a promising treatment for migraine and cluster headache; however, underlying mechanisms need to be elucidated. Because of the importance of the trigeminovascular system (TVS) and trigeminal-parasympathetic ganglia in these disorders, we proposed that melatonin receptors (MT1 and MT2) are expressed in …

Management of chronic headache.

BACKGROUND: Headache remains the most common cause of neurological consultation in clinical practice for which correct diagnosis and treatment are essential. OBJECTIVE: This article provides a review of headache presentation and management, with an emphasis on chronic headaches and the differentiation between migraine and tension-type headache (TTH). DISCUSSION: By far the most important diagnostic tool for proper headache diagnosis is the taking of a concise and representative history of the headaches. Migraine and TTH exist along a continuum and identification of the patient's position on

Neurophthalmologic signs in headache syndromes.

Visual and oculomotor changes may be the only abnormalities in patients complaining of headache. Ocular signs are common during episodes of migraine and cluster headache. Temporal arteritis may be an extracranial cause of ocular signs. Intracranial disorders include painful ophthalmoplegia, pseudotumor cerebri and various causes of increased intracranial pressure. Neurophthalmologic evaluation in headache patients must include assessment of visual acuity and visual fields, examination of pupils (including pharmacologic testing), ophthalmoscopic examination and auscultation for bruits.

SUNCT syndrome. Diagnosis morbi. Shortlasting Unilateral Neuralgiform headache attacks, with Conjunctival injection, Tearing and rhinorrhoea.

The present communication represents an updating of the clinical features of SUNCT. The characteristics of SUNCT have been weighed against other orbital/periorbital headache syndromes towards which differential diagnosis is mandatory. In this group, there are various headaches: cluster headache syndrome, first branch trigeminal neuralgia, and idiopathic stabbing headache (jabs and jolts syndrome). SUNCT emerges as a fairly clear-cut, clinical syndrome. A set of diagnostic criteria of SUNCT syndrome is also given.

Indomethacin-responsive trigeminal autonomic cephalgias: a review of key characteristics and pathophysiology.

Trigeminal autonomic cephalgias (TACs) are a well-defined subset of uncommon primary headaches that share comparable onset, pathophysiology and symptom patterns. TACs are characterised by the presentation of one-sided and high-intensity trigeminal pain together with unilateral cranial autonomic signs, which can include lacrimation, rhinorrhea, and miosis. The International Classification of Headache Disorders 3rd Edition recognises four different headache entities in this group, with cluster headache as the most recognised among them. Hemicrania continua (HC) and paroxysmal hemicrania (PH) are…

Multiple cranial nerve blocks for the transitional treatment of chronic headaches.

BACKGROUND: Multiple cranial nerve blocks of the greater and lesser occipital, supraorbital, supratrochlear and auriculotemporal nerves are widely used in the treatment of primary headaches. We present efficacy and safety data for these procedures. METHODS: In an uncontrolled open-label prospective study, 119 patients with chronic cluster headache, chronic migraine, short lasting unilateral neuralgiform attack disorders, new daily persistent headaches, hemicrania continua and chronic paroxysmal hemicrania were examined. All had failed to respond to greater occipital nerve blocks. Response was…

Trigeminal autonomic cephalalgias: from pathophysiology to clinical aspects.

The strictly unilateral headaches, more commonly known as trigeminal autonomic cephalalgias (TACs), are characterised by severe, strictly unilateral pain in the territory of the distribution of the trigeminal nerve, associated with autonomic manifestations. The recent International Headache Society classification lists the strictly unilateral headaches as cluster headache (CH), episodic and chronic paroxysmal hemicrania, and short-lasting unilateral neuralgiform headache attacks with conjunctival injection and tearing. CH is the most common and best-defined of the TACs, whose pathophysiologies…
