Collagen glucosyltransferase activity (EC 2.4.1.66) was quantified in experimentally-induced liver carcinoma, murine schistosomiasis mansoni-induced liver fibrosis and compared to the level of enzyme activity in control liver samples. Enzyme activity in hepatoma and fibrotic tissues were 12 and 5 times the mean level of enzyme activity in the control liver tissue respectively. A series of 25 women with genuine stress incontinence were treated by para-urethral injection of glutaraldehyde cross-linked collagen. The technique was simple and associated with few complications. Follow-up at 3 months revealed 20 patients (80%) to have been symptomatically improved or cured. An increase in urethral closure pressure together with a decrease in urine flow rate and increase in detrusor pressure was found post-operatively. Premature vascular aneurysms and fragility of cerebral arteries are commonly associated with type III collagen mutations and physical signs suggesting a generalized abnormality of connective tissue. Sometimes these traits are clearly genetically transmitted. Ten patients with bilateral nanophthalmos underwent sclerectomies for uveal effusion. Ultrastructural examination of the sclera revealed abnormal collagen in seven patients. Microangiopathic changes in capillary architecture associated with collagen diseases can be visualized by vital capillary microscopy. In our study of 100 patients with collagen disease we found typical capillaroscopic images in 64%. Differentiation in subclasses showed the highest diagnostic value of this method in systemic sclerosis (77%) and rheumatoid arthritis (68%). It was lower in patients with Sharp- and CREST-syndrome (55%) and very low in systemic lupus erythematosus (33%). In summary, vital capillary microscopy is a valid method in non-invasive diagnostic of collagen diseases.

Auto-immunity to type II collagen is now recognised as a good experimental model of arthritis. The possibility of a post-immunization treatment with collagen to suppress or abrogate the arthritic response is particularly interesting in the therapeutical trends suggested by such a possibility. In human pathology, if the pathogenic role of anti-native collagen type II auto-antibodies is well demonstrated in relapsing polychondritis, in the contrary, this hypothesis does not seem relevant to rheumatoid arthritis. In this disease, the role of the auto-immunity to native collagen appears secondary.

There are at least 7 variants of Ehlers-Danlos syndrome (EDS). One family with type 1, one with type 3 and one isolated case with an undefined type of EDS are described. The relative proportion of collagen type I and type III in the aponeurosis of twenty-four patients with Dupuytren's disease was determined and compared with the aponeurosis of normal persons. The presence of considerable amounts of type III collagen was found in the Dupuytren's disease patients. The sera of the patients were screened for circulating anti-collagen antibodies using a sensitive radioimmunoassay. Relapsing polychondritis is a disorder of unknown cause characterized by the destruction of cartilage. To test the hypothesis that immunologic mechanisms are involved in the pathogenesis of relapsing polychondritis, we analyzed the serum of 15 patients for the presence of antibodies to cartilage.
