AIM: To determine whether there are characteristic immunohistological changes in the colonic mucosa in acute graft versus host disease (GvHD).
METHODS: Consecutive allogeneic (n = 11) and autologous (n = 11) bone marrow transplant recipients underwent endoscopic biopsy of sigmoid mucosa before transplant and on day 30 post-transplant. Immunohistochemical staining and quantitation of intraepithelial and lamina propria mononuclear cells were undertaken using a panel of monoclonal antibodies and a Streptavidin-biotin alkaline phosphatase staining technique.

During 1993, four consecutive adult male patients underwent emergency surgery for free perforation of nonspecific sigmoid colon ulcers. All four patients presented with peritonitis and in each patient the Hartmann's procedure was performed. In each patient there was a single perforation but two of the patients presented with multiple erosions and ulcerations, and the other two- with solitary ulcers. A patient with adenocarcinoma of the sigmoid colon producing alpha-fetoprotein (AFP) is described. Hepatoid differentiation is suggested by the morphological appearance and production of hepatic-type AFP. The possible histogenesis of such a tumor is discussed.

It is well documented that use of seat belts will reduce the incidence and severity of injuries associated with motor vehicle accidents. However, seat belts themselves can produce serious injury, particularly if worn incorrectly. Three cases of an unusual sigmoid colon injury produced by seat belt trauma are reported. Each patients physical exam findings, as well as the anatomy and histology of the injury, are presented to illustrate the deceptive nature of this injury, and to stress the importance of an accurate diagnosis and management.

A case of perianal invasive Paget's disease associated with a sigmoid colon carcinoma is presented. The chief complaint was perianal irritation for a year. Histologic examination yielded a correct diagnosis and abdominosacral resection with wide excision of the cutaneous component was performed. Histology of the resected specimen revealed the Paget's disease to be invasive of the dermis, the sigmoid colon carcinoma to be in Dukes' A stage and the two lesions to be different. The patient has been disease-free for more than 5 years after the operation.

A case of endometrioid carcinoma arising from endometriosis of the sigmoid colon is reported. The histogenesis of the endometriosis in this patient was thought to be artificial secondary implantation of the endometrial tissue into the invaded wall of the sigmoid colon from the ovarian endometriosis during an operative procedure.

We assessed the safety of Hartmann's procedure in terms of postoperative morbidity and mortality associated not only with the original operation, but also with reestablishment of colorectal continuity and long-term survival. We studied 107 consecutive patients in whom Hartann's procedure was performed either electively (96) or as an emergency (11) between 1970 and 1975. The effect of dopamine on human sigmoid motility has been studied in 26 subjects. To record mechanical activity of the sigmoid colon, two small, air-filled balloons mounted on a probe introduced through a sigmoidoscope were used.
