Deprivation scores and NHS practice payment trends in England 2018-2019 to 2023-2024:a multivariable analysis.

BACKGROUND: Funding shortfalls persist for practices in the most deprived areas, despite capitation formula adjustments. AIM: To evaluate whether deprivation scores predicted practice payment trends between 2019 and 2024. DESIGN AND SETTING: Multivariable analysis was undertaken of English general practices (2018-2019 to 2023-2024), excluding practices with <750 patients or average payments >£500 per patient per year, using published data. METHOD: A quadratic mixed-effects model was fitted, using cluster-robust standard errors. The outcome was log-transformed average NHS practice payments p

Rural and urban clinician views on COVID-19's impact on substance use treatment for individuals on community supervision in Kentucky.

BACKGROUND: The COVID-19 pandemic produced system-level changes within the criminal legal system and community-based substance use disorder (SUD) treatment system with impacts on recovery efforts. This study examines rural and urban clinicians' perspectives of COVID-19 on SUD treatment delivery for people on community supervision. METHODS: Virtual qualitative interviews were conducted between April and October 2020 with 25 community supervision clinicians employed by Kentucky's Department of Corrections (DOC), who conduct assessments and facilitate community-based treatment linkages for indiv

HIV knowledge and access to testing for people with and without disabilities in low- and middle-income countries: evidence from 37 Multiple Indicator Cluster Surveys.

INTRODUCTION: Disability and HIV are intricately linked, as people with disabilities are at higher risk of contracting HIV, and living with HIV can lead to impairments and disability. Despite this well-established relationship, there remains limited internationally comparable evidence on HIV knowledge and access to testing for people with disabilities. METHODS: We used cross-sectional data from 37 Multiple Indicator Cluster Surveys. Surveys were from six UNICEF regions, including East Asia and Pacific (n = 6), East and Central Asia (n = 7), Latin America and the Caribbean (n = 6), Middle East

Characterizing the mental health and functioning of Canadian respiratory therapists during the COVID-19 pandemic.

Introduction: Healthcare professionals (HCPs) appear to be at increased risk for negative psychological outcomes [e.g. depression, anxiety, post-traumatic stress disorder (PTSD), moral distress] and associated impacts on functioning throughout the COVID-19 pandemic. HCPs working on designated COVID-19 units may be further impacted than their colleagues not on these units given added demands of patient care and risk of contracting COVID-19. Little is known, however, about the mental health and functioning of specific professional groups beyond nurses and physicians, including respiratory therap

Adolescents and Young Adults: The Pediatrician's Role in HIV Testing and Pre- and Postexposure HIV Prophylaxis.

Most sexually active youth in the United States do not believe that they are at risk for contracting HIV and have never been tested. Creating safe environments that promote confidentiality and respect, obtaining an accurate sexual and reproductive health assessment, and providing nonstigmatizing risk counseling are key components of any youth encounters. Pediatricians can play a key role in preventing and controlling HIV infection by promoting risk-reduction counseling and offering routine HIV testing and prophylaxis to adolescent and young adult (youth) patients. In light of persistently high

Provider challenges in responding to retrafficking of juvenile justice-involved domestic minor sex trafficking survivors.

BACKGROUND: Domestic minor sex trafficking (DMST) survivors are disproportionately involved in the juvenile justice system, but frequently run away and experience retrafficking. However, little research explores how practitioners who work with juvenile justice-involved DMST survivors address such dynamics. OBJECTIVE: This study examines challenges related to chronic runaway behaviors and related retrafficking of juvenile justice-involved DMST survivors from the perspective of practitioners. PARTICIPANTS AND SETTING: 35 in-depth interviews were conducted with social service and justice system

The Mask of the Warrior: unraveling deep-seated health vulnerabilities in veteran identities.

As service members transition from deployment to civilian life, they are also expected to reintegrate into society. An important part of this process is to "soften up" veteran or warrior identities and open up the self for both existing and new identities, mindsets, and ways of life. Past research has shown that the warrior mindset, in particular, can have negative health implications in the long run. The mindset can be costly, not only for the individual and their loved ones, but also for the healthcare services and other agencies. This article draws from a recent interview study with 24 depl

Health Care Costs and Thromboembolic Events in Hydroxyurea-Treated Patients with Polycythemia Vera.

BACKGROUND: Patients with polycythemia vera (PV) are at increased risk of thromboembolic events (TEs), which are key contributors to reduced overall survival compared with the age- and sex-matched general population. In addition to aspirin and phlebotomy to maintain hematocrit level < 45%, many patients receive cytoreduction with hydroxyurea (HU), which is associated with improved survival and may reduce the risk of cardiovascular events and TEs. However, 1 in 4 patients become resistant to or intolerant of HU. In the general population, prophylaxis and treatment following arterial and venous

Obstetric management of high-order multiple pregnancies.

High-order multiples are increasingly common as a result of assisted reproductive technologies and represent pregnancies at exceptional risk. This article discusses the antepartum management of high-order multiples, which has in general been highly individualized and poorly studied. Care for high-order multiples should include preterm birth prevention education, the frequent assessment of maternal symptoms and cervical status by a consistent provider, individualized modification of activity, attention to maternal nutrition, ultrasonography for the assessment of fetal anatomy and intra-uterine

A brief prehospital stroke severity scale identifies ischemic stroke patients harboring persisting large arterial occlusions.

BACKGROUND AND PURPOSE: The Los Angeles Motor Scale (LAMS) is a brief 3-item stroke severity assessment measure designed for prehospital and Emergency Department use. METHODS: The LAMS and NIHSS were scored in under-12-hour acute anterior circulation ischemic stroke patients. Stroke severity ratings were correlated with cervicocerebral vascular occlusion on CTA, MRA, and catheter angiography. Receiver operating curves, c statistics, and likelihood ratios were used to evaluate the predictive value for vascular occlusion of stroke severity ratings. RESULTS: Among 119 patients, mean age was 67

Information Loss in Scalar Monetary Aggregation: A Tensorial Langevin Framework for Financial Shock Propagation and Policy Targeting.

We develop a tensor-based dynamical framework for monetary flows in multi-sector, multi-agent economies and quantify the information destroyed when the monetary state is reduced to a scalar aggregate. The state is a third-order tensor encoding capital flows across sectors, agent classes, and time; deviations from equilibrium obey a tensor-indexed Langevin (multivariate Ornstein-Uhlenbeck) equation with a coupling operator and channel-specific friction rates. Using standard Lyapunov theory, we assemble a stability and convergence framework for the induced vectorized system, with a bound stated
