Vitamin D in tuberous sclerosis complex-associated tumors.

Mammalian target of rapamycin inhibitors (mTORi) have been used to treat pediatric tuberous sclerosis complex (TSC)-associated tumors, particularly in cases with contraindications to surgery or difficulties in complete tumor resection. However, some patients experience side effects and tumor regression after discontinuation of the treatment. Therefore, there is an urgent need to develop drugs that can be used in combination with mTORi to increase their efficacy and minimize their side effects. 1,25-Dihydroxyvitamin D3 (1,25-D), which has anticancer properties, may be a promising candidate for

[Anaesthesia in Electrochemotherapy].

Electrochemotherapy (ECT) is a symptom control method for inoperable or exulcerating cutaneous metastases or skin cancer. With the help of electroporation, an enhancement of the efficacy of the administered chemotherapeutic agent, bleomycin or cisplatin, is achieved, which leads to a local reduction of the metastases and thereby has a low impact on the systemic health.ECT can be performed under local, regional or general anaesthesia, whereby the form of anaesthesia depends on the number and extent of the metastases as well as the affected body site. For general anaesthesia, there are some spec

[Vertebroplasty and kyphoplasty : A critical statement].

BACKGROUND: Despite optimal drug-conservative therapy, a relevant percentage of patients with vertebral compression fractures (WKF) do not experience any relevant improvement in their pain symptoms. Vertebroplasty (VP) and kyphoplasty (KP) are described in the literature as percutaneous interventional procedures for the treatment of WKF. OBJECTIVE: Assessment of the effectiveness of the VP and KP in the treatment of WKF and discussion of the procedures in the context of the current literature. MATERIAL AND METHODS: Presentation of the fundamentals of VP and KP and their further developments.

Focus on the Novel Cardiovascular Drug LZC696: from Evidence to Clinical Consideration.

LCZ696, a first-in-class angiotensin receptor neprilysin inhibitor (ARNI), is comprised of the angiotensin receptor blocker valsartan and the neprilysin inhibitor pro-drug sacubitril (AHU377). After oral administration, AHU377 is rapidly metabolized to the active neprilysin inhibitor LBQ657. LCZ696 exerts its effects of diuresis, natriuresis, vasodilation and aldosterone secretion inhibition through simultaneous renin-angiotensin-aldosterone system (RAAS) blockade and natriuretic peptides system (NPS) enhancement. Powerful evidence including PARAMETER and PRARDIGM-HF trials have shown that LCZ

Clinical Practice. Postmenopausal Osteoporosis.

Key Clinical Points Postmenopausal Osteoporosis Fractures and osteoporosis are common, particularly among older women, and hip fractures can be devastating. Treatment is generally recommended in postmenopausal women who have a bone mineral density T score of -2.5 or less, a history of spine or hip fracture, or a Fracture Risk Assessment Tool (FRAX) score indicating increased fracture risk. Bisphosphonates (generic) and denosumab reduce the risk of hip, nonvertebral, and vertebral fractures; bisphosphonates are commonly used as first-line treatment in women who do not have contraindications. Clinical indications for acupuncture in chronic post-traumatic headache management.

Chronic post-traumatic headache (PTH) is one of the most common complaints after mild traumatic brain injury, yet evidence to date is insufficient to direct conventional treatment of headaches with this etiology. Therefore, the current guidelines recommend a symptomatic approach for the three patterns of PTHs: migraine-like, tension-like, and mixed symptomatology. To improve response rates and minimize the potential for polypharmacy, adverse effects, and risk of dependency, effective nonpharmacologic options should be employed to support faster and safer patient rehabilitation. Current evidenc

Safety and efficacy of vigabatrin for the treatment of infantile spasms.

In 2009, vigabatrin became the first FDA approved medication for the treatment of infantile spasms in the United States. There are few well-designed prospective studies comparing the drug to placebo or other modalities used in the treatment of infantile spasms. The available data have demonstrated that vigabatrin is efficacious in the treatment of infantile spasms regardless of underlying etiology, but that it is particularly beneficial in patients with a diagnosis of tuberous sclerosis. Adrenocorticotropic hormone (ACTH), the only other medication with robust efficacy data, has been used as f

[Arrhythmia in patients with ischemic heart disease and indications for surgical treatment].

The authors analysed 171 patients with ischemic heart disease (IHD) and life-threatening and non-threatening arrhythmias before and after surgical treatment. The control group was composed of 34 patients with IHD (of a similar severity of the principle disease) but without arrhythmias. Life-threatening arrhythmias of the third arrhythmic group persisted even after effective revascularization of the myocardium and resection of the aneurysm. To reveal the zones of elective instability in this group of patients preoperative electrophysiological examination must be conducted with programmed stimul

Open surgical ablation of ventricular tachycardia: Utility and feasibility of contemporary mapping and ablation tools.

BACKGROUND: Ventricular tachycardia (VT) catheter ablation success may be limited when transcutaneous epicardial access is contraindicated. Surgical ablation (SurgAbl) is an option, but ablation guidance is limited without simultaneously acquired electrophysiological data. OBJECTIVE: We describe our SurgAbl experience utilizing contemporary electroanatomic mapping (EAM) among patients with refractory VT storm. METHODS: Consecutive patients with recurrent VT despite antiarrhythmic drugs (AADs) and prior ablation, for whom percutaneous epicardial access was contraindicated, underwent open Surg

A New Perspective on the Nitrate-Phosphodiesterase Type 5 Inhibitor Interaction.

BACKGROUND: Nitrates and nitrate-containing compounds are vasodilators used for the treatment of angina and heart failure. Phosphodiesterase type 5 inhibitors used for the treatment of erectile dysfunction are also vasodilators, and when taken together with nitrates, synergistic effects that enhance hypotensive effects may occur. Phosphodiesterase type 5 inhibitors are therefore contraindicated in patients taking organic nitrates. METHODS AND RESULTS: A literature review was performed to provide a historical overview of different phosphodiesterase type 5 inhibitors and nitrates and their inte

Potassium Iodide in Dermatology- Recent Advances in Mechanism of Action, Preparation, Uses and Adverse Effects.

Potassium iodide (KI) is a simple yet versatile drug. It has been successfully used for the treatment of various diseases in dermatology, but there are very limited studies on the exact mechanism of action, preparation, and dosage of potassium iodide, causing concerns regarding its toxicity. This article reviews the multimodal mechanism of action of KI and standardises the method of preparation of a saturated solution of potassium iodide (SSKI) on an OPD basis. It also lists the existing and newer indications, optimal dosages, adverse effects, and contraindications of potassium iodide, thereby

Transcatheter Arterial Chemoembolization with Drug-Eluting Beads for the Treatment of Hepatocellular Carcinoma: Recommended Selection for Small-Caliber (<100 μm) Beads.

Drug-eluting beads transarterial chemoembolization (DEB-TACE) is an alternative to conventional lipiodol-based TACE (cTACE) to treat hepatocellular carcinoma (HCC). With the advancement in pharmacology, small-caliber DEB-TACE (<100 μm) has been introduced since 2016. For the treatment of hepatic neoplasms or HCC, there is a tendency to use smaller beads by DEB-TACE to achieve more extensive tumor necrosis and a significant reduction in liver toxicity in comparison with that caused by cTACE. However, the indications and potential complications of small-caliber DEB-TACE remain uncertain and have

[China expert consensus on center guided home-based cardiac rehabilitation].

Cardiac rehabilitation (CR) is a structured evidence-based intervention technology in chronic stage of patients with cardiovascular disease (CVD), including patient education, health behavior modification, drug treatment and exercise training. Evidence has showed that CR therapy further improved secondary prevention outcomes in patients with CVD. However, its utilization rate in China is very low. Among eligible patients, only a few have received CR treatment. Therefore, it is extremely urgent to propose new strategies to improve CR participation. Center guided home-based cardiac rehabilitatio

Identification and prioritization of novel anti-Wolbachia chemotypes from screening a 10,000-compound diversity library.

Lymphatic filariasis and onchocerciasis are two important neglected tropical diseases (NTDs) that cause severe disability. Control efforts are hindered by the lack of a safe macrofilaricidal drug. Targeting the Wolbachia bacterial endosymbionts in these parasites with doxycycline leads to a macrofilaricidal outcome, but protracted treatment regimens and contraindications restrict its widespread implementation. The Anti-Wolbachia consortium aims to develop improved anti-Wolbachia drugs to overcome these barriers. We describe the first screening of a large, diverse compound library against Wolba

[Pediatric caudal anesthesia: importance and aspects of safety concerns].

Caudal block is a safe procedure commonly used for pediatric perioperative analgesia. Complications are extremely rare but nevertheless local and systemic contraindications must be excluded. Optimal safety and quality result when strict attention is paid to technical details. A local anesthetic (LA) containing epinephrine allows early detection of inadvertent intravascular LA administration; therefore an epinephrine/LA mixture is recommended at least for the test dose. In terms of safety the choice of LA itself is probably of secondary importance. Clonidine as an adjuvant has an excellent risk
