Prevalence of allergic contact dermatitis in Thailand.

BACKGROUND: Contact dermatitis is a universal condition. Patch testing is an effective diagnostic tool for clinically suspected allergic contact dermatitis. Previous studies have shown that the prevalence of specific allergens varies by geographic area. OBJECTIVE: To determine the relative frequency of positive reactions to different sensitizers in Thai patients with clinically suspected contact dermatitis. METHODS: The results of patch tests performed from January 1, 2004, to December 31, 2006, at the Siriraj Hospital Contact Dermatitis Clinic were analyzed. A total of 323 consecutive patie

[In vitro testing for allergic contact dermatitis].

Patch testing is the standard method to identify allergic contact dermatitis. Simple, safe and accurate in vitro methods identifying contact allergies would offer both theoretical and practical advantages. Using the lymphocyte transformation test (LTT), allergen-specific lymphocytes central to the pathogenesis of Type IV allergies can be identified. Various studies have assessed the value of LTT in the diagnosis of contact allergies. The current assessment is that the test can be helpful in distinguishing between irritant and allergic reactions. LTT is especially useful in demonstrating drug a

Clues to an accurate diagnosis of contact dermatitis.

An accurate diagnosis of allergic contact dermatitis can be achieved by a combination of historical, morphologic, and diagnostic steps. Clues in the history and physical examination can point to an irritant as the source of contact dermatitis. While irritants and allergens share many common features both immunologically and clinically, there are grounds for the distinction. Knowledge of occupational factors is necessary to assess the source of contact dermatitis. A common pitfall is the failure to appreciate the role of endogenous factors in the clinical presentation and overall care of the de

Contact dermatitis to cosmetics, fragrances, and botanicals.

Cosmetics, fragrances, and botanicals are important causes of allergic contact dermatitis. Identifying and avoiding the causative allergens can pose a challenge to both the patient and the dermatologist. The site of involvement can give the investigator clues to the cause of the eruption in many cases. Fragrances and preservatives are the two most clinically relevant allergens in cosmetics. Botanicals are being added to cosmetics because of consumer demand and are now being recognized as sources of allergy as well. Patch testing allows for the detection of allergens that are potentially releva

Allergic contact dermatitis to a sanitary pad.

A 13-year-old girl presented with a recurring vulval dermatitis that started several days after first using a sanitary pad. The rash settled but recurred with subsequent use of the same sanitary pads. Patch testing revealed a marked sensitivity (3+) to colophony (rosin). She denied previous reactions to adhesive tape/sticking plaster. During patch testing she also developed reactions to a black permanent waterproof marking pen used to mark the site of patch tests. The ink of this pen also contained colophony.

A metallurgical approach to metal contact dermatitis.

It is well-known that some metals/alloys are sensitizing on skin contact, but it is not so well appreciated that sensitization is dependent on the reaction of the metal/alloy with sweat. The first step in skin sensitization by metals/alloys is a corrosion process and the formation of soluble metal ions. The nature of this process has implications with respect to occurrence of metal contact dermatitis, to testing techniques and to classification of metals/alloys as skin sensitizers.

[Analysis of contact dermatitis by cytokines. I].

The study of the participation of T cells and the kinetic studies of cytokines by the RT-PCR method in the challenge phase of contact hypersensitivity reaction (CHR) were performed using skin tissues collected at regular intervals from mice sensitized with 2,4-dinitro-1-fluorobenzene (DNFB). The results obtained from these studies and the further analyses using anti-CD4 and CD8 antibodies showed that the revelation of phenotype of lymph node cells at the reaction site and T lymphocytes at the CHR site were found to play important roles. Furthermore, we examined cytokines formed at the CHR site

The rise and fall of allergic contact dermatitis.

When a simple chemical hapten contacts the skin, it triggers a complex cellular and molecular reaction that eventuates in the cutaneous reaction clinically recognized as allergic contact dermatitis. This review summarizes the crucial role of the cytokines and adhesion molecules that direct the traffic of cells into, and out of, the skin after contact with an allergen. Particularly stressed are the molecular interactions between antigen-bearing Langerhans cells and responding CD4(+) T cells: this interaction results in the amplification of the memory cells that add specificity to the immune res

Allergic contact dermatitis to fragrance: a review.

BACKGROUND: Allergy to fragrance is the most common cause of cosmetic contact dermatitis and therefore constitutes a significant clinical problem. The widespread use of fragranced materials in skin care and household products is probably the most important reason for the high incidence of fragrance sensitization. OBJECTIVE: This report will summarize the history of fragrance, review how to detect and evaluate fragrance allergy, discuss the problems inherent in patch testing with the fragrance mix and its constituents, describe systemic contact dermatitis from ingestion of certain flavors, and

Mechanisms of resolution of allergic contact dermatitis.

Photoallergic and allergic contact dermatitis are examples of type IV hypersensitivity reactions that involve T cell-mediated immune responses against haptens that come into contact with the skin. These two types of allergies differ in that for routine contact allergens, the hapten is usually a chemically reactive species that readily couples to host proteins; for photoallergic reactions, UV light (320 - 400 nm) is necessary to generate ("photoactivate") the chemically reactive hapten. From this point on, both photoallergic and allergic contact dermatitis are likely to proceed along the same p

Contact dermatitis caused by cetrimide in antiseptics.

Cetrimide and chlorhexidine are widely used antiseptics. Irritant contact dermatitis from cetrimide has been reported, but only rarely and without pathologic study. We report the clinical and pathologic findings from a series of 18 cases of contact dermatitis caused by antiseptic solutions containing 3% cetrimide and 0.3% chlorhexidine. The patients consisted of 10 males and 8 females, aged from 2 to 62 years. Most patients developed pruritic, burning or painful rashes over the flexors and genitalia after 1 to 4 weeks of daily use of undiluted antiseptics for an underlying pruritic dermatosis.

Cosmetics as causes of allergic contact dermatitis.

This article reviews the correlation between the nature of cosmetic products and the location of lesions in cases of contact dermatitis. It also reviews the nature of the most common allergenic products and substances and offers guidelines for testing cosmetics in cases of contact allergy.
