Kangaroo Mother Care in Developing Countries With Poor Hygiene or Limited Healthcare Infrastructure: A Systematic Review.

Kangaroo Mother Care (KMC) is an affordable, evidence-based intervention widely endorsed for improving survival and health outcomes in preterm and low-birth-weight infants, particularly in resource-constrained healthcare settings. Concerns persist regarding its use in areas with suboptimal sanitation, mainly due to fears of increased newborn infection rates. This systematic review evaluates KMC's effectiveness and safety in contexts characterized by poor hygiene and limited resources. Using the PICO framework, we conducted comprehensive searches across PubMed, Cochrane Library, Scopus, Web of

Neonatal Bloodstream Infections: A Narrative Review on Diagnostic and Therapeutic Challenges and Prospects in Developing Countries.

BACKGROUND: Bloodstream infections (BSIs) pose a significant global health challenge, particularly in developing countries. Neonates are highly vulnerable due to underdeveloped immune systems and immature physical barriers, a risk amplified by poor hygiene and limited healthcare access in low- and middle-income countries (LMICs), leading to an estimated incidence of 5.5 cases per 1000 live births. This review aims to provide insight into the diagnostic and therapeutic challenges of neonatal BSIs in developing countries and to identify prospects for improving their diagnosis and treatment. MET…

Intravenous lidocaine: A transformative approach to reducing opioid use in pediatric burn patients in developing countries.

BACKGROUND: Effective postoperative pain control in pediatric burn patients is essential for recovery. Despite advances in pain management, opioid use remains high, contributing to adverse effects and risks of dependency. Intravenous (IV) lidocaine has shown analgesic benefits in adult and pediatric postsurgical contexts but remains underexplored in burned children. This study evaluates the impact of lidocaine on opioid consumption and its safety profile in this population. OBJECTIVE: To determine whether continuous IV lidocaine infusion reduces opioid use in pediatric burn patients during th

Cardiomyopathy and Sudden Cardiac Death Among the Athletes in Developing Countries: Incidence and Their Prevention Strategies.

The incidence of cardiomyopathy in athletes contributes significantly to the public health burden in developing countries. Most effective management strategies primarily rely on the modification of risk factors, and it is less expensive compared to other advanced investigations. Moreover, limited data is available concerning the incidence of adverse events including cardiac arrest and the strategies to prevent them, especially in this population subset. Therefore, devising preventative strategies that can easily be implemented in athletes and provide a cost-effective approach is warranted. We …

A Systemic Review and Meta-analysis of the Leading Pathogens Causing Neonatal Sepsis in Developing Countries.

BACKGROUND: Neonatal sepsis is one of the major public health problems globally, particularly, in developing countries. Klebsiella, Staphylococcus aureus, Coagulase-negative Staphylococcus, and Escherichia coli are the common pathogens for neonatal sepsis in developing countries. However, the pooled estimate of common pathogens causing neonatal sepsis in developing countries is still unknown. Therefore, this study is aimed at computing the pooled proportion of the leading cause of pathogens for neonatal sepsis in developing countries. METHODS: We strictly followed the Preferred Reporting Item…

Associations of mental disorders and neurotropic parasitic diseases: a meta-analysis in developing and emerging countries.

BACKGROUND: Although they are declining worldwide, neurotropic parasitic diseases are still common in developing and emerging countries. The aim of this study was to estimate the pooled prevalence and pooled association measures of comorbidities between mental disorders (anxiety, depression, bipolar disorder, and schizophrenia) and neurotropic parasitic diseases (malaria, cysticercosis, toxoplasmosis, human African trypanosomiasis, Chagas disease, and human toxocariasis) in developing and emerging countries. METHODS: As the first meta-analysis on this topic, this study was performed in accord

Need for Grass Root Innovation in Developing Countries: Case for Stationary Binder Clips in Scalp Hemostasis.

OBJECTIVE: The primary training in any surgical practice starts with tissue handling and effective hemostasis. Neurosurgical procedures start with an incision in the scalp and require summative use of mechanical hemostats and bipolar coagulation to achieve hemostasis. Though Raney clips are the most popular and effective in maintaining hemostasis, their high cost and nonreusability become deterrents for routine use in resource-stricken environments. METHODS: We have compared stationery binder clips of different sizes with Raney clips on the parameters of effectiveness, availability, and cost.

Minimally invasive transcatheter aortic valve implantation for the treatment of rheumatic heart disease in developing countries.

The treatment of rheumatic heart disease presents a medical and surgical challenge, particularly in developing countries, where the disease is prevalent. Most of these countries find it prohibitively expensive to import heart valve prostheses that are required for treatment and are largely manufactured in the United States and Europe. Even if the valves were available, the absence of facilities to conduct open-heart surgery for valve replacement results in many people from developing countries dying needlessly of the disease. To address this problem, transcatheter aortic valve implantation, an…

Incidence and case-fatality of varicella-zoster virus infection among pediatric cancer patients in developing countries.

UNLABELLED: Limited evidence is available about varicella-zoster virus (VZV) infection among pediatric cancer patients in developing countries, which raises questions about the generalizability of VZV vaccine recommendations for pediatric cancer patients (derived from developed countries) to these settings. We assessed the incidence and case-fatality of VZV infection at three institutions in developing countries (Argentina, Mexico, and Nicaragua). Individuals eligible for our study were aged <20 years and actively receiving cancer-directed therapy. We estimated a summary incidence rate (IR) an…

Scaling up cancer care for children without medical insurance in developing countries: The case of Mexico.

BACKGROUND: In 2006, the Mexican government launched the Fund for Protection Against Catastrophic Expenditures (FPGC) to support financially healthcare of high cost illnesses. This study aimed at answering the question whether FPGC improved coverage for cancer care and to measure survival of FPGC affiliated children with cancer. PROCEDURE: A retrospective cohort study (2006-2009) was conducted in 47 public hospitals. Information of children and adolescents with cancer was analyzed. The coverage was estimated in accordance with expected number of incident cases and those registered at FPGC. Th

Strategies for the improvement of cardiac care services in developing countries: what does the future hold?

Cardiovascular disease is the leading cause of death in developing countries. The atherosclerotic or degenerative diseases are not only on the rise, they are occurring at a younger age compared with those in developed nations. Some of these developing countries continue to have a high prevalence of rheumatic heart disease, with a large number of new cases with acute rheumatic fever. It appears that the prevalence of congenital heart disease has remained the same in both developing and developed countries, although the actual data are missing. Most of the developing countries have an increasing…

Community-acquired bacterial bloodstream infections in developing countries in south and southeast Asia: a systematic review.

Information about community-acquired bacteraemia in developing countries in south and southeast Asia is scarce. We aimed to establish the case fraction of bacteraemia in febrile patients admitted to hospital. We searched four databases and identified studies of south and southeast Asia published between 1990 and 2010 that prospectively assessed patients admitted to hospital and from whom a blood culture was taken. We reviewed 17 eligible studies describing 40,644 patients. Pathogenic organisms were isolated from 3506 patients (9%; range 1-51%); 1784 (12%) of 14,386 adults and 1722 (7%) of 26,2…

Evaluation of vaccines against enteric infections: a clinical and public health research agenda for developing countries.

Enteric infections are a major cause of morbidity and mortality in developing countries. To date, vaccines have played a limited role in public health efforts to control enteric infections. Licensed vaccines exist for cholera and typhoid, but these vaccines are used primarily for travellers; and there are two internationally licensed vaccines for rotavirus, but they are mainly used in affluent countries. The reasons that enteric vaccines are little used in developing countries are multiple, and certainly include financial and political constraints. Also important is the need for more cogent ev…

Home visits by community health workers to prevent neonatal deaths in developing countries: a systematic review.

OBJECTIVE: To determine whether home visits for neonatal care by community health workers can reduce infant and neonatal deaths and stillbirths in resource-limited settings. METHODS: We conducted a systematic review up to 2008 of controlled trials comparing various intervention packages, one of them being home visits for neonatal care by community health workers. We performed meta-analysis to calculate the pooled risk of outcomes. FINDINGS: Five trials, all from south Asia, satisfied the inclusion criteria. The intervention packages included in them comprised antenatal home visits (all trial

Supplementary vitamin E, selenium, cysteine and riboflavin for preventing kwashiorkor in preschool children in developing countries.

BACKGROUND: Protein Energy Malnutrition is an important cause of child morbidity and mortality in middle- and low-income countries. It has been suggested that excessive free radical activity may be responsible for the clinical manifestation of kwashiorkor. Antioxidants may be able to curb excessive free radical activity and prevent the development of kwashiorkor in susceptible children. OBJECTIVES: To evaluate the benefits of supplementation of vitamin E, selenium, cysteine and riboflavin (alone or in combination) in preventing kwashiorkor. SEARCH STRATEGY: We conducted searches of CENTRAL 2

HIV infection and pregnancy status among adults attending voluntary counseling and testing in 2 developing countries.

OBJECTIVES: This study investigated the impact of HIV voluntary counseling and testing (VCT) on reproduction planning among 1634 adults in 2 sub-Saharan countries. METHODS: Data were obtained from a multisite randomized controlled trial. RESULTS: At 6 months post-VCT, the women more likely to be pregnant were younger (odds ratio [OR] = 2.5; 95% confidence interval [CI] = 1.0, 6.5), not using contraceptives (OR = 0.1; 95% CI = 0.1, 0.3), and HIV infected (OR = 3.0; 95% CI = 1.3, 7.0). An interaction emerged linking pregnancy intention at baseline and HIV serostatus with pregnancy at follow-up
