[Digestive and predigestive functions in patients with eating disorders].

Patients with eating disorders (anorexia nervosa, AN; bulimia nervosa, BN) frequently exhibit gastrointestinal symptoms and altered gastrointestinal functions, especially delayed gastric emptying. These symptoms are regarded as secondary to the disordered eating behaviour, vomiting or laxative misuse. They often improve during successful therapy. There are, however, studies showing that in addition predigestive functions (smell, taste, cephalic phase of digestion) and the hormonal regulation of digestion may be changed in eating disorders. This underlines the possibility that, in a subpopulati…

[Neurogastroenterology: focus on pediatric digestive motility diseases].

The enteric nervous system exercises a key role on the gastrointestinal tract (GIT) motility, sensibility, secretions and absorption. This "Little brain of the gut" consists of numerous autonomic neurones located in the GIT, influenced by luminal and intrinsic factors. A new science, the neurogastroenterology, explores the modulation of the GIT functions and the interactions between the central, autonomic and enteric nervous systems forming the brain-gut axis. It works to understand the role of the glial and Cajal's cells, of chemical mediators, hazards of the GIT ontology, influence of inflam

[Role of endoscopic capsule in the diagnosis of digestive diseases].

The recent development of the endoscopic capsule allows for the endoscopic study of the whole small intestine on a safe and non-invasive basis. Digestive hemorrhage of unknown origin is the main indication of the technique (sensitivity 89%, specificity 95%). In these cases, a high diagnostic accuracy is seen in patients with active bleeding (92.3%), which nevertheless decreases notably in patients with previous non-active bleeding (12.9%). Capsule endoscopy has also been proposed in patients with a clinical and laboratory suspicion of Crohn's disease involving the small bowel which has not bee

[Digestive and hemorrhage complications of low-dose aspirin].

INTRODUCTION: The gastro-intestinal (GI) toxicity associated with high dose aspirin has been fully demonstrated, but remains poorly elucidated at low doses i.e., less than 500 mg/day. Such toxicity is relatively difficult to study because lesional and/or bleeding GI complications are not always well described in studies. The objective of this review is to compile a documented inventory of GI complications induced by low-dose aspirin. METHODOLOGY: This review is based on a detailed review of randomized studies, case-control and cohort studies which aim to study the GI toxicity of low-dose aspi

Implantation of migrated biliary stents in the digestive tract.

BACKGROUND: Biliary stents constitute an alternative for the palliative treatment of benign or malignant biliary obstruction, biliary strictures, choledocholithiasis, biliary fistulas from lateral lesions of the biliary duct or cystic duct leaks due to slippage of clip closure. Obstruction resulting in cholangitis is common. Proximal migration to the biliary duct or distal migration to the duodenum with subsequent passage per rectum are relatively frequent, but impaction and perforation of the bowel are rare. CASE OUTLINES: Two cases are reported. In one patient a migrated stent impacted in t

Is the food-entrainable circadian oscillator in the digestive system?

Food-anticipatory activity (FAA) is the increase in locomotion and core body temperature that precedes a daily scheduled meal. It is driven by a circadian oscillator but is independent of the suprachiasmatic nuclei. Recent results that reveal meal-entrained clock gene expression in rat and mouse peripheral organs raise the intriguing possibility that the digestive system is the site of the feeding-entrained oscillator (FEO) that underlies FAA. We tested this possibility by comparing FAA and Per1 rhythmicity in the digestive system of the Per1-luciferase transgenic rat. First, rats were entrain

Social position of adolescents with chronic digestive disorders.

OBJECTIVE: To investigate the consequences of having a chronic digestive disorder on the social position of adolescents. METHODS: Five diagnostic groups, including inflammatory bowel disease (IBD), chronic liver diseases, congenital digestive disorders, coeliac disease and food allergy (total n = 758, ages 12-25 years), were each compared with a population-based control group in a multicentre study using a cross-sectional design. Social position was assessed by a mailed questionnaire measuring 24 aspects, categorized as education, leisure activities, friendship, labour participation, financia

Digestive haemorrhage in patients with anti-Anisakis antibodies.

OBJECTIVE: Anisakis simplex parasite causes anisakidosis, a disease that often mimics other gastrointestinal diseases such as peritonitis, appendicitis, Crohn's disease, ulcer, etc. Patients with digestive haemorrhage caused by ulcers, varices or Mallory syndrome were analysed for antibodies to the worm A. simplex. METHODS: Antibody detection was carried out by enzyme-linked immunosorbent assay (ELISA) and immunoblot using crude extracts of antigen and excretory/secretory products. Total immunoglobulin (Ig), IgG, IgM, IgA and IgE were studied. RESULTS: Eighty-seven patients were studied. The

[Radiologic diagnosis of congenital anomalies of the digestive tract].

UNLABELLED: The aim of the study is to analyze the frequency of the congenital anomalies of the digestive tract in the period of 1993-1998. The patients and methods: Our group included 67 patients (47 males and 20 females; age range 1-12 months) suspected of digestive tract anomalies. First, all of them have been examined by pediatric surgeon. At the first place, we used the most conventional radiological procedures for digestive tract examination. They were followed by US in cases with suspicion on hypertrophic pyloric stenosis and biliary atresia (bile-duct atresia). CT and MR were performed

[Application of ultrasound method for hemorrhagic digestive disease].

Ultrasonography for the digestive diseases are consisted of abdominal ultrasonography (US), endoscopic ultrasonography (EUS) and color Doppler endoscopic ultrasonography (CDEUS). These play a supplementary role in comparison with the roentgenography and endoscopy. The information of the ultrasonography is different from these examinations. By US the collateral shunts of esophago-gastric varices are observed. EUS is useful for diagnosis of the properties of esophago-gastric varices and judgement of effects of treatment for these varices, gastric ulcer and vessels in ulcer base and hemorrhagic b

Hydatid liver cyst perforation into the digestive tract.

Complications of liver hydatid disease include those related to the compression of adjacent viscus, to the infection of the cyst's contents or to the perforation of the cyst. Perforations into hollow abdominal organs are rare, and can occur secondary to the infection of the cyst or to a primary pathology of the perforated organ. We report on 2 cases of perforation of a liver cyst into the duodenum, 1 due to the presence of a duodenal ulcer, and 1 secondary to a hydatid abscess. The surgical treatment of the lesions and their complications was, in both cases, successful.

[Acute low digestive hemorrhage in testicular choriocarcinoma].

We report a case in which acute intestinal hemorrhage was the initial manifestation in the evolution of choriocarcinoma of the testis. The diagnosis of bleeding was difficult and required laparotomy. Metastatic lesion of the small bowel should be considered a possible cause of intestinal bleeding in patients with testicular choriocarcinoma.

[Treatment of prostheto-digestive using arterial allograft].

From October 1988 to March 1995, we operated 22 patients for fistulization between the prosthesis and the digestive tract to remove the in situ allograft. The delay between the initial operation and treatment for fistulization was 7.3 +/- 4 years. In these patients who had undergone multiple operations (2.5 +/- 1.9 operations per patient), the infected prosthesis was made of Dacron in 21 cases and polytetrafluoroethylene in one. The procedure was planned beforehand in 21 cases who benefited from a complete preoperative work-up and was required in an emergency situation in 6 for digestive bleed

[Ectopic varices, a rare cause of digestive hemorrhage].

From January 1986 to September 1995, 4 patients were hospitalized in our ward for gastrointestinal bleeding from ectopic varices. The patients were all female, aged 30 to 65 years. The etiology of portal hypertension in these patients was alcoholic cirrhosis, cirrhosis in Wilson's disease and previous alveolar echinococcosis treated by right hepatectomy, complicated by post-operative portal thrombosis. Clinical presentation in all 4 cases was lower gastrointestinal bleeding. Diagnosis was by emergency arteriography in 3 cases; no source was found in one case with recurrent hemorrhage. The 4 pa

[Colonic angiodysplasia: an important cause of digestive hemorrhage].

Angiodysplasia of the colon is one of the most common causes of lower gastrointestinal bleeding in elderly patients. The diagnosis can be made by colonscopy or angiography. The best treatment, endoscopic coagulation or surgical resection, is still being debated. Over a period of nearly seven years, 5 patients with angiodysplasia of the colon were treated in our Department. The treatment was right hemicolectomy for all patients with localization at the caecum or ascending colon; in the only patients with segmoid localization a segmentary resection was performed. During a mean follow-up of 22.8

[Digestive stenoses following ulcero-necrotizing enterocolitis].

Over a 10-year period, the authors followed the course of 112 neonates treated for primary acute necrotizing enterocolitis. Ten cases of gastrointestinal stricture were diagnosed, regardless of initial treatment, after an interval of 20 days to 6 months. The site of the stricture was colonic in 6 patients, ileal in three other patients and ileal and colonic in the remaining patient. The stricture was treated surgically in 9 patients (resection-anastomosis in 6 cases, resection with gastrointestinal diversion in 2 cases and isolated gastrointestinal diversion in 1 case).
